【摘要】：胃腸功能衰竭是一種嚴(yán)重的全身性炎癥反應(yīng),常并發(fā)多臟器功能不全綜合征,中西醫(yī)結(jié)合治療具有較好的療效。該研究收集名老中醫(yī)余國(guó)友主任醫(yī)師中醫(yī)藥治療89例胃腸功能衰竭的175張?zhí)幏?通過(guò)數(shù)據(jù)挖掘探析、統(tǒng)計(jì)其中藥及中藥類(lèi)別頻數(shù)、頻率,采用系統(tǒng)聚類(lèi)方法分析其用藥規(guī)律,總結(jié)余老的常用藥物及處方。研究結(jié)果顯示藥物使用頻次前3位的是厚樸、枳實(shí)、大黃,均為小承氣湯的組成成分;使用中藥中以補(bǔ)虛藥、理氣藥、清熱藥、化痰藥、化濕藥為最常用類(lèi)別,補(bǔ)虛藥里又以占比達(dá)2/3的補(bǔ)氣藥為主;同時(shí)挖掘出新處方和新的藥物組合,可作為臨床用藥參考。研究結(jié)果表明,余老臨床辨證胃腸功能衰竭多以脾胃氣虛為本,以熱毒、血瘀、氣滯、濕阻為標(biāo),論治胃腸功能衰竭常以健脾、理氣、瀉下通腑為主,依證配合化瘀解毒、化濕利濁、清熱化痰等方藥,辨證施治,隨癥加減,在治療過(guò)程中重視"胃氣"的調(diào)養(yǎng)和胃腸功能的恢復(fù)。
[Abstract]:Gastrointestinal failure is a severe systemic inflammatory reaction, often complicated with multiple organ dysfunction syndrome.This study collected 175 prescriptions for 89 cases of gastrointestinal failure treated by Yu Guoyou, chief physician of old Chinese medicine. Through data mining, the frequency and frequency of traditional Chinese medicine and its categories were counted, and the law of drug use was analyzed by systematic cluster method.General balance of old common drugs and prescriptions.The results showed that the top three drugs were Magnolia officinalis, Fructus Aurantii and Rhubarb, all of which were components of Xiaocheng Qi decoction.At the same time, the new prescription and the new combination of drugs can be used as a reference for clinical use.The results show that the syndrome differentiation of Yu Lao is based on deficiency of spleen and stomach qi, heat toxin, blood stasis, qi stagnation and dampness obstruction as the standard. The main treatment of gastrointestinal failure is to invigorate spleen, regulate qi, and purge the lower and lower entrails, and detoxify blood stasis and detoxify according to syndrome.The prescription of removing dampness and removing turbidity, clearing away heat and resolving phlegm, and so on, are treated with syndrome differentiation, plus or minus with syndrome, and the recuperation of "stomach qi" and the recovery of gastrointestinal function are emphasized in the course of treatment.
