【摘要】：目的：1觀察甘丹油治療嬰幼兒濕疹的臨床療效。2通過小鼠實驗研究觀察甘丹油外用時對濕疹模型小鼠的消炎止癢作用,并且觀察中藥甘丹油對小鼠背部濕疹皮損中IL-2、1NF-α、IFN-γRa和小鼠血中CD4、CD8的免疫影響。方法：1動物的藥效學(xué)研究部分,每日涂抹2,4-二硝基氯苯(2,4-DNCB)于動物的耳、背部皮膚,造其濕疹造模,另外用0.5%的4-氨基吡啶(4-AP)造其瘙癢模型,采用中藥甘丹油進(jìn)行治療,觀察小鼠舔體反應(yīng)次數(shù),比較小鼠兩耳厚度、重量差、耳部皮膚組織中炎癥細(xì)胞數(shù)值的變化,用免疫組化法檢測各組小鼠背部皮損組織中的含量多少,及檢測各組小鼠血中CD4.CD8淋巴細(xì)胞的數(shù)值高低。2 臨床病例研究122例嬰幼兒濕疹患者隨機(jī)分為治療組61例、對照組61例。治療組采用中藥甘丹油治療,對照組采用丁酸氫化可的松軟膏治療。比較兩組病情總積分改變,療效前后對比,皮疹形態(tài)表現(xiàn)分值的改變。結(jié)果：1中藥甘丹油治療嬰幼兒濕疹療效確切。2中藥甘丹油外用對濕疹造模的小鼠其皮損處有明顯的消炎作用,其機(jī)理為潤膚止癢,調(diào)節(jié)小鼠體內(nèi)分泌紊亂的細(xì)胞因子等。結(jié)論：1臨床病例研究結(jié)果表明,對照組各項指標(biāo)明顯優(yōu)于治療組,但從其復(fù)發(fā)率、副作用方面考量治療組優(yōu)于對照組。體現(xiàn)中藥甘丹油治療嬰幼兒濕疹療效較好,可作為理想的治療嬰幼兒濕疹藥物。2小鼠藥效學(xué)結(jié)果顯示中藥甘丹油外用治療致小鼠耳廓腫脹模型、背部濕疹造模均有顯著的抑制作用,具有抗炎消腫作用；對2,4-DNCB致小鼠背部皮膚瘙癢模型有明顯的抑制作用,其止癢作用與給藥濃度有一定關(guān)系,其中以中中組療效最好。體現(xiàn)中藥甘丹油外用的機(jī)理可能是通過抗炎消腫而改善濕疹的皮損,為臨床治療嬰幼兒濕疹和亞急性、慢性濕疹提供理論基礎(chǔ)。
[Abstract]:Objective to observe the clinical efficacy of Gandan oil in the treatment of infant eczema by observing the anti-inflammatory and antipruritic effect of Gandan oil on eczema model mice by experimental study in mice.The immunological effects of Gandan oil on IL-2NF- 偽 IFN- 緯 Ra and CD _ 4 CD _ 8 in mice blood were observed.Methods in the part of pharmacodynamics study on the animals, the mice were treated with 2% 4-dinitrochlorobenzene 24-DNCBs daily on the ear, back skin and eczema. 0.5% 4-aminopyridine 4-AP) was used to make the pruritus model, and the traditional Chinese medicine Gandan oil was used to treat the pruritus model.To observe the times of licking reaction, to compare the thickness and weight of the two ears, to compare the changes of inflammatory cells in the skin tissue of the ears, and to detect the content of the skin lesions in the back of the mice by immunohistochemical method.The clinical case study of 122 cases of infantile eczema were randomly divided into treatment group (61 cases) and control group (61 cases).The treatment group was treated with gandan oil and the control group with hydrocortisone butyrate ointment.The changes of the total score, the curative effect before and after treatment, and the morphologic features of rash were compared between the two groups.Results the therapeutic effect of Gandan oil on infant eczema was definite .2 the external application of Gandan oil had obvious anti-inflammatory effect on the skin lesions of mice model of eczema. The mechanism was to moisturize the skin and relieve itching and regulate the cytokines secreting in mice.Conclusion the results of the clinical case study showed that the control group was superior to the treatment group, but the treatment group was superior to the control group in terms of its recurrence rate and side effects.The results showed that Gandan oil was effective in treating infant eczema, and could be used as an ideal drug for treating infant eczema. The pharmacodynamics of mice showed that the model of auricle swelling was induced by external use of Gandan oil, which was an ideal drug for the treatment of infant eczema.All the models of eczema on the back had obvious inhibitory effect, which had anti-inflammatory and swelling effect, and had obvious inhibitory effect on the mice skin pruritus model induced by 2C4-DNCB, and its antipruritus effect was related to the concentration of drug administration, and the middle group had the best curative effect.The mechanism of external application of Gandan oil may be to improve the skin lesions of eczema through anti-inflammation and detumescence, which provides a theoretical basis for the clinical treatment of infantile eczema, subacute and chronic eczema.
