【摘要】：目的:觀察運(yùn)用消幽復(fù)胃湯治療Hp耐藥性慢性胃炎脾胃濕熱證患者的Hp根除情況及臨床癥狀的改善情況,以探討其臨床價(jià)值。方法:本臨床觀察運(yùn)用治療前后自身對(duì)照的方法來(lái)進(jìn)行研究,將40例經(jīng)西醫(yī)正規(guī)根除Hp方案治療但Hp未根除,且經(jīng)中醫(yī)系統(tǒng)辨證為脾胃濕熱證者納入研究中,并以消幽復(fù)胃湯為治療藥物。每次服用劑量為150ml,于每日早飯前及晚飯后各30min服用,規(guī)定療程為4周。療程結(jié)束后,觀察并記錄患者治療前后在中醫(yī)證候積分上的具體變化情況。并至少在患者停藥4周之后運(yùn)用13C-UBT檢測(cè)患者的Hp,以觀察其Hp的根除率情況,全部數(shù)據(jù)收集完畢后,運(yùn)用SPSS17.0統(tǒng)計(jì)軟件進(jìn)行系統(tǒng)分析。結(jié)果:納入本研究的40例患者中有2例脫落,最終剩余38例患者完成研究。在Hp根除方面,共有20例患者的Hp被成功根除,根除率達(dá)到52.63%,且經(jīng)檢驗(yàn)分析后,得出χ2 = 27.14,且p0.01,可判斷治療前后Hp的根除情況具有顯著差異性,具備統(tǒng)計(jì)學(xué)意義。在中醫(yī)證候的臨床療效方面,其有效率為94.7%。在中醫(yī)證候的各項(xiàng)積分及總積分方面,通過(guò)統(tǒng)計(jì)學(xué)系統(tǒng)分析,P值均0.01,可判斷差異具備統(tǒng)計(jì)學(xué)意義。結(jié)論:1.消幽復(fù)胃湯對(duì)Hp耐藥性慢性胃炎脾胃濕熱證患者體內(nèi)的Hp具備一定的根除作用。2.消幽復(fù)胃湯能夠明顯改善Hp耐藥性慢性胃炎脾胃濕熱證患者的中醫(yī)臨床癥狀,對(duì)胃脘有灼熱感、口臭口苦、惡心欲嘔及大便黏膩等癥狀的改善更為明顯。
[Abstract]:Objective: to observe the eradication of HP and the improvement of clinical symptoms in patients with chronic gastritis with HP resistance.Methods: the clinical observation was carried out by using self-control method before and after treatment. Forty cases of HP eradication regimen were treated by western medicine, but HP was not eradicated, and the syndrome of spleen and stomach dampness and heat was identified as the syndrome of spleen and stomach dampness and heat by TCM system.And with Xiaoyou Fuwei decoction as the treatment drug.Each dose is 150ml. Take 30min before breakfast and after supper. The course of treatment is 4 weeks.After the course of treatment, observe and record the patients before and after treatment on the specific changes in TCM syndromes.At least 4 weeks after drug withdrawal, 13C-UBT was used to detect HP in order to observe the eradication rate of HP. After all the data were collected, SPSS17.0 statistical software was used for systematic analysis.Results: two of the 40 patients involved in the study had dropped off, and the remaining 38 patients had completed the study.In the aspect of HP eradication, a total of 20 patients were successfully eradicated, and the eradication rate was 52.63. After test and analysis, it was concluded that 蠂 2 = 27.14, and p0.01, it can be judged that the eradication of HP before and after treatment has significant difference and has statistical significance.The effective rate of TCM syndromes is 94. 7%.In the aspects of integral and total integral of TCM syndromes, the difference can be judged statistically by statistical systematic analysis of P value of 0.01.Conclusion 1.Xiaoyou Fuwei decoction has a certain eradication effect on HP in patients with HP resistance chronic gastritis, spleen and stomach dampness and heat syndrome.Xiaoyou Fuwei decoction can obviously improve the clinical symptoms of TCM in patients with chronic gastritis with HP resistance and chronic gastritis with dampness and heat of spleen and stomach. The improvement of symptoms such as burning heat in stomach, bitter breath, nausea and nausea, sticky stool and so on is more obvious.
