【摘要】：研究目的:本文通過對從脾論治相關疾病的現(xiàn)代文獻進行挖掘研究,探討建國以來從脾論治相關疾病的宏觀情況,深入分析腹瀉、痞滿、冠心病、紫癜的臨床診療規(guī)律和動物實驗研究內涵。研究資料與方法:以從脾論治相關疾病的現(xiàn)代文獻作為研究資料,通過構建數(shù)據(jù)庫以及數(shù)據(jù)平臺,對從脾論治相關疾病現(xiàn)代臨床類文獻的病、證、癥、方等內容以及動物實驗類文獻的病、證、指標內容進行規(guī)范,以關聯(lián)分析為主進行數(shù)據(jù)挖掘和可視化處理,探討并總結從脾治療相關疾病的臨床文獻和動物實驗文獻的內涵規(guī)律。研究成果與結論:1結合年代和發(fā)文量分析,文獻的增長情況良好,臨床從脾論治相關疾病的研究一直在穩(wěn)步發(fā)展。2根據(jù)宏觀情況分析,從脾論治疾病涉及到中醫(yī)的內、外、婦、兒以及耳鼻喉和骨傷科,中醫(yī)內科疾病中脾胃系疾病、氣血津液病以及肝膽病所占比例較大;從脾治療相關疾病的西醫(yī)病種在九大系統(tǒng)皆有所涉及,其中對消化系統(tǒng),內分泌系統(tǒng)和循環(huán)系統(tǒng)疾病的治療具有明顯優(yōu)勢。3選取了功能性消化不良、腹瀉、冠心病、紫癜進行分析,繪制力導向圖,得出現(xiàn)代臨床中的以上疾病分別和中醫(yī)學中的痞滿、泄瀉、胸痹、紫斑和血證聯(lián)系最為密切。在現(xiàn)代臨床中,它們的最常見證型分別是“肝郁脾虛”“脾陽虛”“心脾兩虛”和“脾腎兩虛、陰虛火旺”,這些與現(xiàn)代人的生活方式有很大關系;在治療方面,功能性消化不良中臨床應用最多的方劑是桂附理中湯;腹瀉(包括小兒腹瀉)應用最多的方劑是參苓白術散;紫癜以及冠心病臨床應用最多的方劑是歸脾湯。以上四種疾病,胸痹、泄瀉、痞滿的共有證型是肝郁脾虛,而紫斑與泄瀉證型聯(lián)系最為密切,共有證型是脾腎兩虛。4對功能性消化不良、腹瀉、冠心病、紫癜的動物實驗類文獻進行研究,繪制�；鶊D,得出了從脾論治冠心病主要是從氣與血的異常出發(fā),其病理產物是痰與瘀,與現(xiàn)代醫(yī)學的能量代謝、脂質代謝、免疫反應以及血管壁張力內容有關;紫癜主要的發(fā)生機制是脾統(tǒng)血功能的失常,與現(xiàn)代醫(yī)學的血液組成物質、血管生理功能以及人體的免疫機制有關;腹瀉和功能性消化不良都是由于脾的運化功能失常造成的,它們與現(xiàn)代醫(yī)學中的胃腸運動激素、腦-腸軸和免疫系統(tǒng)等相關指標均有密切聯(lián)系。以上四種疾病皆可根于脾虛,其中腹瀉與消化不良的發(fā)生根于脾的運化失司;紫癜的發(fā)生源于脾不統(tǒng)血;冠心病的發(fā)生是脾運化與統(tǒng)血功能異常共同作用的結果。
[Abstract]:Objective: to explore the macroscopical situation of the diseases related to spleen treatment since the founding of the people's Republic of China, and to analyze the diarrhea, fullness, coronary heart disease by digging up the modern literature on the treatment of diseases related to spleen.Clinical diagnosis and treatment of purpura and connotation of animal experiment.Research materials and methods: taking the modern literature on the treatment of related diseases from the spleen as the research data, through constructing the database and data platform, the diseases, syndromes and symptoms of the modern clinical literature on the treatment of the related diseases from the spleen were studied.Fang and other contents, as well as the disease, syndrome and index contents of animal experimental literature, are standardized, and data mining and visualization are mainly carried out by association analysis.To explore and summarize the connotation of clinical literature and animal experimental literature on spleen treatment related diseases.Research results and conclusions: 1 combined with the analysis of the years and the quantity of documents issued, the growth of the literature is good. The research on the clinical treatment of diseases related to the treatment of the spleen has been developing steadily .2 according to the macroscopic analysis, the treatment of diseases by the spleen involves the internal, external and female aspects of traditional Chinese medicine.Children, otolaryngology and orthopedics, diseases of spleen and stomach, diseases of qi, blood and body fluid, and diseases of liver and bile in traditional Chinese medicine have a large proportion. Western medicine diseases related to the treatment of spleen and spleen are involved in nine major systems, among which are the digestive system.The treatment of endocrine system and circulatory system diseases has obvious advantage. .3 selected functional dyspepsia, diarrhea, coronary heart disease, purpura to carry on analysis, draw force guide chart, get the above diseases in modern clinic and bulge full in traditional Chinese medicine, respectively.Diarrhea, chest obstruction, purple spots and blood syndrome are most closely linked.In modern clinical practice, their most common syndromes are "liver stagnation and spleen deficiency," "spleen Yang deficiency," "deficiency of heart and spleen," and "deficiency of spleen and kidney, deficiency of yin and fire", which are closely related to modern life style.The most commonly used prescription in functional dyspepsia is Guifulizhong decoction; the most commonly used prescription for diarrhea (including infantile diarrhea) is Shenling Atractylodes Powder; the prescription for purpura and coronary heart disease is Guipi decoction.The common syndromes of the above four diseases, chest obstruction, diarrhea, and fullness, are liver stagnation and spleen deficiency, while purple spot is most closely related to diarrhea syndrome. The common syndrome type is functional dyspepsia, diarrhea, coronary heart disease with deficiency of spleen and kidney.The animal experimental literature of purpura was studied, and the mulberry map was drawn. It was concluded that the treatment of coronary heart disease from spleen was mainly based on the abnormality of qi and blood, and its pathological product was phlegm and blood stasis, energy metabolism and lipid metabolism in modern medicine.The main mechanism of purpura is the abnormal function of spleen regulating blood, which is related to the blood composition of modern medicine, the physiological function of blood vessel and the immune mechanism of human body.Diarrhea and functional dyspepsia are caused by splenic dysfunctions which are closely related to gastrointestinal motility hormones brain-intestinal axis and immune system in modern medicine.The above four diseases can be rooted in spleen deficiency, in which diarrhea and dyspepsia take root in the spleen, purpura comes from spleen not regulating blood, and coronary heart disease is the result of splenic translocation and abnormal function of blood regulation.
