【摘要】：目的觀察腕踝針配合彈力帶訓(xùn)練對(duì)膝骨性關(guān)節(jié)炎的臨床療效,并通過分析疼痛程度、等速肌力、平衡能力的變化,為臨床尋求一個(gè)簡(jiǎn)單有效的治療方案。方法本研究采用簡(jiǎn)單隨機(jī)對(duì)照方法,將60例符合納入標(biāo)準(zhǔn)的膝骨性關(guān)節(jié)炎患者隨機(jī)分為腕踝針配合彈力帶訓(xùn)練組、單純彈力帶訓(xùn)練組各30例,進(jìn)行臨床觀察。治療前后對(duì)患者進(jìn)行膝關(guān)節(jié)綜合評(píng)定(包括關(guān)節(jié)活動(dòng)度測(cè)試、等速肌力測(cè)試、動(dòng)態(tài)跌倒風(fēng)險(xiǎn)指數(shù)、VAS疼痛評(píng)分表)。結(jié)果1、兩組治療后較治療前關(guān)節(jié)活動(dòng)度、肌力、動(dòng)態(tài)跌倒風(fēng)險(xiǎn)指數(shù)和VAS疼痛評(píng)分均有明顯改善,治療后優(yōu)于治療前(P0.01)。2、臨床療效比較：腕踝針配合彈力帶訓(xùn)練組：臨床控制7例、顯效14例、有效8例、無(wú)效1例,總有效率96.7%；單純彈力帶訓(xùn)練組：臨床控制4例、顯效9例、有效14例、無(wú)效3例,總有效率90%,腕踝針配合彈力帶康復(fù)訓(xùn)練組療效優(yōu)于單純彈力帶康復(fù)訓(xùn)練組(P0.05)。3、兩組治療后肌力、動(dòng)態(tài)跌倒風(fēng)險(xiǎn)指數(shù)和VAS評(píng)分改善程度比較,腕踝針配合彈力帶訓(xùn)練組優(yōu)于單純彈力帶訓(xùn)練組,差異顯著(P0.05)；但兩組治療后關(guān)節(jié)活動(dòng)度測(cè)試結(jié)果相比較無(wú)顯著差異(P0.05)。結(jié)論1、應(yīng)用腕踝針配合彈力帶康復(fù)訓(xùn)練治療膝骨性關(guān)節(jié)炎的療效確切,并可顯著提高短期療效。2、通過配合腕踝針對(duì)于阻斷、降低膝骨性關(guān)節(jié)炎的疼痛更為顯著,同時(shí)在消除疼痛因素干擾的情況下促使彈力帶訓(xùn)練中膝關(guān)節(jié)的屈／伸肌的訓(xùn)練獲得更大成效、進(jìn)一步提高關(guān)節(jié)穩(wěn)定性。3、通過腕踝針配合彈力帶訓(xùn)練,能夠有效降低中老年膝骨性關(guān)節(jié)炎患者的跌倒風(fēng)險(xiǎn),提高平衡控制能力。
[Abstract]:Objective to observe the clinical effect of wrist ankle acupuncture combined with elastic band training on knee osteoarthritis, and to find a simple and effective treatment scheme by analyzing the changes of pain degree, isokinetic muscle strength and balance ability.Methods 60 patients with osteoarthritis of knee were randomly divided into two groups: carpal and ankle acupuncture combined with elastic band training group (30 cases) and elastic band training group (30 cases).Before and after treatment, the patients were evaluated with comprehensive knee joint (including joint motion test, isokinetic muscle strength test, dynamic fall risk index and VAS pain score).Results 1. After treatment, the joint motion, muscle strength, dynamic fall risk index and VAS pain score of the two groups were significantly improved, which was better than that of the pre-treatment group (P 0.01). The clinical efficacy was compared: carpal and ankle acupuncture combined with elastic belt training group: 7 cases were controlled clinically.The total effective rate was 96.7% in 14 cases, effective in 8 cases, ineffective in 1 case, and the total effective rate was 96.7.The simple elastic band training group showed clinical control in 4 cases, remarkable effect in 9 cases, effective in 14 cases, ineffective in 3 cases.The total effective rate was 90. The curative effect of wrist ankle acupuncture combined with elastic band rehabilitation training group was better than that of simple elastic band rehabilitation training group (P0.05. 3). The improvement degree of muscle strength, dynamic fall risk index and VAS score were compared between the two groups after treatment.Wrist ankle acupuncture combined with elastic band training group was better than simple elastic band training group, the difference was significant (P 0.05), but there was no significant difference between the two groups after treatment.Conclusion 1. The treatment of knee osteoarthritis with wrist ankle acupuncture combined with elastic band rehabilitation training is effective, and can significantly improve the short-term curative effect. The pain of knee osteoarthritis can be reduced more significantly by the combination of wrist and ankle to block the osteoarthritis.At the same time, under the condition of eliminating the disturbance of pain factors, the training of flexion / extensor muscle of knee joint in elastic band training is more effective, and the stability of joint is further improved by means of wrist and ankle acupuncture combined with elastic band training.It can effectively reduce the fall risk and improve the balance control ability in the middle-aged patients with knee osteoarthritis.
