【摘要】：目的:觀察補腎調經湯治療腎虛型月經先后不定期的臨床療效,以期確立治療月經先后不定期的有效方法,并初步探討其作用機理,為臨床提供理論基礎。方法:將門診收集的60例腎虛型月經先后不定期的患者隨機分為治療組和對照組,每組各30例。一個療程后(即三個月經周期)觀察對比兩組治療前后臨床癥狀及體征變化;中醫(yī)證候改善程度及積分變化;促卵泡激素(FSH)、促黃體生成素(LH)、雌二醇(E2)、基礎體溫(BBT)及子宮內膜變化。結果:1.臨床療效比較:治療組總有效率為93.33%,對照組總有效率為66.67%,組間比較差異有統(tǒng)計學意義,治療組優(yōu)于對照組(P0.05)。2.中醫(yī)證候改善情況比較:治療組總有效率96.67%,對照組總有效率73.33%,兩組對比差異有統(tǒng)計學意義(P0.05)。3.中醫(yī)證候積分比較:治療后,治療組與對照組積分均較治療前降低,差異具有統(tǒng)計學意義,治療組與對照組比較差異具有統(tǒng)計學意義(P0.05)。4.中醫(yī)單項癥狀比較:治療后,治療組在改善各項中醫(yī)癥狀方面總有效率高于對照組,組間比較差異有統(tǒng)計學意義(P0.05)。5.實驗室檢查結果及BBT變化情況比較:治療后,治療組FSH、LH與治療前相比下降,差異有統(tǒng)計學意義(PO.05);對照組FSH、LH、E2與治療前相比,差異無統(tǒng)計學意義(P0.05)。治療后,兩組FSH、LH組間比較,差異有統(tǒng)計學意義(P0.05)。治療后兩組子宮內膜厚度與治療前比較差異有統(tǒng)計學意義(P0.05),治療組與對照組對比,差異有統(tǒng)計學意義(P0.05)。治療后基礎體溫兩組對比,差異有統(tǒng)計學意義(P0.05)。結論:補腎調經湯治療腎虛型月經先后不定期,可以更好的改善患者的臨床癥狀及體征、FSH及LH、BBT、子宮內膜厚度。其作用機制可能與調節(jié)機體下丘腦-垂體-卵巢軸有關。補腎調經湯治療腎虛型月經先后不定期臨床效果滿意,并且安全可靠。
[Abstract]:Objective: to observe the clinical effect of Bushen Tiaojing decoction on menstruation of kidney deficiency type, to establish an effective method of treating menstrual disorder, and to explore its mechanism, and to provide a theoretical basis for clinical practice.Methods: sixty patients with kidney deficiency menstruation were randomly divided into treatment group and control group, 30 cases in each group.After a course of treatment (that is, three menstrual cycles), the changes of clinical symptoms and signs before and after treatment, the degree of improvement of TCM syndromes and the changes of integral were observed and compared between the two groups.Follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E 2), basal body temperature (BBT) and endometrial changes.The result is 1: 1.Comparison of clinical efficacy: the total effective rate of the treatment group was 93.33 and the total effective rate of the control group was 66.67. The difference between the two groups was statistically significant, and the treatment group was superior to the control group (P 0.05).Comparison of the improvement of TCM syndromes: the total effective rate in the treatment group was 96.677.The total effective rate in the control group was 73.33. The difference between the two groups was statistically significant.Comparison of TCM syndromes: after treatment, the scores of the treatment group and the control group were lower than those of the control group, the difference was statistically significant, and the difference between the treatment group and the control group was statistically significant.Comparison of single symptoms of TCM: after treatment, the total effective rate of the treatment group in improving the symptoms of traditional Chinese medicine was higher than that of the control group, and the difference between the two groups was statistically significant (P 0.05).Comparison of the results of laboratory examination and the changes of BBT: after treatment, the FSH LH in the treatment group was lower than that before treatment, the difference was statistically significant (P 0.05), while in the control group, there was no significant difference between the two groups before and after treatment (P 0.05).After treatment, the difference between the two groups was statistically significant (P 0.05).There was a significant difference in endometrial thickness between the two groups after treatment and before treatment (P 0.05), and the difference between the treatment group and the control group was statistically significant (P 0.05).There was a significant difference in basic body temperature between the two groups after treatment (P 0.05).The mechanism may be related to the regulation of hypothalamus-pituitary-ovary axis.The treatment of kidney deficiency menstruation with Bushen Tiao Jing decoction has satisfactory clinical effect and is safe and reliable.
