【摘要】：目的:觀察在口服乙酰半胱氨酸片治療的基礎(chǔ)上,健脾益肺膏對(duì)慢性阻塞性肺疾病穩(wěn)定期肺脾氣虛型患者的臨床療效。方法:采用隨機(jī)對(duì)照分組的方法,以肺脾氣虛型慢性阻塞性肺疾病為研究對(duì)象,采用具有前瞻性對(duì)照的方案。將納入的60例患者,隨機(jī)分為中藥健脾益肺膏聯(lián)合乙酰半胱氨酸片的治療組,和單用乙酰半胱氨酸片治療的對(duì)照組。每組各30例,以8周為治療觀察時(shí)間。通過觀測(cè)患者治療前后中醫(yī)癥狀評(píng)分、肺功能、運(yùn)動(dòng)耐力(6分鐘步行距離,6MWT),從而評(píng)估健脾益肺膏的療效。結(jié)果:1、健脾益肺膏治療穩(wěn)定期慢性阻塞性肺疾病(肺脾氣虛型)患者疾病療效肯定,治療組和對(duì)照組的有效率分別是90%、66.7%。兩組患者經(jīng)治療后對(duì)比,差異明顯,說明治療組在疾病療效方面明顯優(yōu)于對(duì)照組。2、穩(wěn)定期慢性阻塞性肺疾病患者(肺脾氣虛型)通過口服健脾益肺膏可以很好的改善患者的中醫(yī)主癥咳嗽,咳痰,喘息氣促。3、兩組患者治療前的肺功能相比無明顯差異,能夠相互比較。經(jīng)治療后,兩組患者的肺功能與之前相比明顯好轉(zhuǎn),但治療組患者的肺功能改善情況明顯要比對(duì)照組好。說明健脾益肺膏能夠更好地改善患者的肺功能,提高患者的生活質(zhì)量,改善患者的臨床預(yù)后。4、兩組患者治療后6分鐘步行實(shí)驗(yàn)跟治療前相比,步行距離較前提高,對(duì)照組治療后提高程度不如治療組。5、兩組患者治療前、治療后,患者的肝腎功能、尿常規(guī)、血常規(guī)均在正常值范圍內(nèi),說明治療前后對(duì)患者身體無明顯影響,安全性好。結(jié)論:針對(duì)慢阻肺肺脾氣虛型患者,在乙酰半胱氨酸片治療的基礎(chǔ)上聯(lián)用健脾益肺膏,能更好地改善患者的臨床療效,且無明顯毒副作用,安全有效,值得在臨床上推廣應(yīng)用。
[Abstract]:Objective: to observe the clinical effect of Jianpi Yifei ointment on chronic obstructive pulmonary disease (COPD) patients with chronic obstructive pulmonary disease (COPD) in stable stage of lung and spleen qi deficiency on the basis of oral acetylcysteine tablets.Methods: a randomized controlled group was used to study chronic obstructive pulmonary disease with deficiency of lung and spleen qi.60 patients were randomly divided into two groups: the treatment group treated with Jianpi Yifei ointment and acetylcysteine tablet and the control group treated with acetylcysteine tablet alone.30 cases in each group were treated for 8 weeks.The curative effect of Jianpi Yifei ointment was evaluated by observing the scores of TCM symptoms, pulmonary function, exercise endurance and walking distance of 6 minutes before and after treatment.Results the curative effect of Jianpi Yifei ointment on stable chronic obstructive pulmonary disease was confirmed. The effective rate of treatment group and control group was 90% and 66.7% respectively.After treatment, the difference between the two groups was obvious.It shows that the curative effect of the treatment group is obviously better than that of the control group. The patients with stable chronic obstructive pulmonary disease (lung and spleen qi deficiency) can improve the cough and phlegm of the main symptoms of traditional Chinese medicine by taking oral Jianpi Yifei ointment.There was no significant difference in lung function between the two groups before treatment and could be compared with each other.After treatment, the pulmonary function of the two groups was obviously improved compared with the former, but the improvement of pulmonary function in the treatment group was better than that in the control group.The results showed that Jianpi Yifei ointment could improve the lung function, improve the quality of life, and improve the clinical prognosis of the patients. The walking distance of the two groups was higher than that before treatment 6 minutes after treatment.Before and after treatment, the liver and kidney function, urine routine and blood routine of the patients in the control group were within the normal range, indicating that there was no obvious effect on the patients' body before and after treatment, and the safety was good.Conclusion: the combination of acetylcysteine tablets and Jianpi Yifei ointment can improve the clinical efficacy of patients with chronic obstructive pulmonary disease and spleen qi deficiency, and it is safe and effective. It is worth popularizing in clinic.
