【摘要】：目的:評價(jià)腎衰方改善CKD4期慢性腎衰患者腎功能及鈣磷代謝的臨床療效。方法:選取慢性腎臟病4期(脾腎氣虛、瘀血阻滯型)患者60例,隨機(jī)分為兩組:對照組和治療組,前者給予西醫(yī)一體化治療,后者在前者的基礎(chǔ)上加用腎衰方,治療6個(gè)月后,觀察兩組患者臨床療效、中醫(yī)證候積分、腎功能、24 h尿蛋白定量、鈣、磷、1,25(OH)2D3、甲狀旁腺、血清FGF-23的變化。結(jié)果:治療后,治療組臨床療效明顯優(yōu)于對照組(P0.05),在降低中醫(yī)證候積分、減少24h尿蛋白定量、降低尿素氮、肌酐、提高腎小球?yàn)V過率方面優(yōu)于對照組(P0.05)。治療組在降低血磷、改善繼發(fā)性甲狀旁腺功能亢進(jìn)方面優(yōu)于對照組(P0.05)。結(jié)論:腎衰方在治療脾腎虧虛、瘀血阻滯型CKD4期患者較對照組有明顯的臨床療效,能減少蛋白尿、改善腎功能、降低血磷,改善甲狀旁腺功能亢進(jìn),能延緩慢性腎功能衰竭,具有一定的臨床意義。
[Abstract]:Objective: to evaluate the clinical effect of Shenshuai recipe on improving renal function and calcium and phosphorus metabolism in patients with chronic renal failure (CKD4). Methods: 60 patients with chronic kidney disease (spleen and kidney qi deficiency and blood stasis block) were randomly divided into two groups: control group and treatment group. The former was treated with integrated western medicine, while the latter was treated with Shenshuai recipe on the basis of the former. After 6 months of treatment, the clinical efficacy, TCM syndromes, renal function 24 h urinary protein quantification, calcium, phosphorus, 125 OHH 2D3, parathyroid gland were observed in the two groups. Results: after treatment, the clinical efficacy of the treatment group was significantly better than that of the control group (P 0.05). The treatment group is superior to the control group in reducing blood phosphorus and improving secondary hyperparathyroidism. Conclusion: Shenshuai prescription can treat deficiency of spleen and kidney. Compared with the control group, blood stasis block type CKD4 patients have obvious clinical effect, which can reduce proteinuria, improve renal function, reduce blood phosphorus, improve hyperparathyroidism, delay chronic renal failure, and have certain clinical significance.
