【摘要】：目的:本課題通過對分期穴位埋線和口服西藥兩者治療原發(fā)性痛經(jīng)的臨床療效進(jìn)行對比,科學(xué)的評價(jià)分期穴位埋線治療原發(fā)性痛經(jīng)的臨床療效及安全性,為分期穴位埋線法治療原發(fā)性痛經(jīng)的推廣而提供依據(jù)。方法:將60例原發(fā)性痛經(jīng)患者隨機(jī)分成埋線組和西藥組,分別30例。埋線組于經(jīng)前期(月經(jīng)來潮前5-7天)和經(jīng)后期(月經(jīng)干凈5天內(nèi))分別埋線一次,經(jīng)前期取穴:關(guān)元、十七椎、子宮、次毼、地機(jī)。經(jīng)后期以膈俞、肝俞、脾俞、腎俞、三陰交為主。西藥組口服布洛芬緩釋膠囊,月經(jīng)來潮前2-3天開始口服,每次0.3g,一天兩次,連續(xù)服用至月經(jīng)期,經(jīng)期感疼痛緩解或消失即可停藥。1個(gè)月經(jīng)周期為1個(gè)療程,連續(xù)治療3個(gè)療程。3個(gè)療程結(jié)束后觀察近期療效,并停止治療3個(gè)月后隨訪遠(yuǎn)期療效。采用痛經(jīng)積分進(jìn)行療效判定,并且比較兩組臨床療效結(jié)果。結(jié)果:埋線組和西藥組近期痛經(jīng)積分對比治療前,差異均有統(tǒng)計(jì)學(xué)意義(P0.01)。埋線組遠(yuǎn)期痛經(jīng)積分與治療前比較,差異具有統(tǒng)計(jì)學(xué)意義(P0.01),西藥組遠(yuǎn)期痛經(jīng)積分與治療前比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。近期臨床療效比較,埋線組治愈13例,顯效9例,有效6例,無效2例;西藥組治愈5例,顯效7例,有效8例,無效10例。遠(yuǎn)期臨床療效比較,埋線組治愈15例,顯效11例,有效2例,無效2例;西藥組治愈5例,顯效4例,有效6例,無效15例。結(jié)論:(1)分期穴位埋線治療原發(fā)性痛經(jīng)療效確切,近期療效可,隨著埋線效應(yīng)的累加、機(jī)體素質(zhì)的改善,遠(yuǎn)期療效也可觀。(2)埋線通過對機(jī)體的良性調(diào)節(jié)發(fā)揮作用,線體吸收需要時(shí)間,早期起效會較慢,但作用卻持久穩(wěn)定,不易復(fù)發(fā)。(3)1個(gè)月經(jīng)周期僅需要治療2次,與口服西藥比較,大大減少治療次數(shù),同時(shí)可以保證治療的連貫性和有效性,亦無需擔(dān)心副作用,操作簡便,經(jīng)濟(jì)安全。(4)布洛芬緩釋膠囊即刻鎮(zhèn)痛效果顯著,迅速緩解癥狀,但作用多數(shù)是不可持久的,停藥后易反彈,且服藥有副作用的可能。(5)分期穴位埋線治療原發(fā)性痛經(jīng)值得臨床推廣運(yùn)用。
[Abstract]:Objective: to evaluate the clinical efficacy and safety of primary dysmenorrhea treated by stage catgut embedding at acupoints and western medicine by oral administration, and to evaluate scientifically the clinical efficacy and safety of the treatment of primary dysmenorrhea. Methods: 60 patients with primary dysmenorrhea were randomly divided into catgut embedding group and western medicine group. 30 cases were treated with catgut embedding in the premenstrual period (5-7 days before the onset of menstruation) and in the late menstrual period (within 5 days of menstruation) respectively. The acupoints were collected in the premenstrual period: Guan Yuan, 17 vertebrae, uterus, secondary phlegm, ground machine. Shenshu, Sanyinjiao mainly. The western medicine group takes ibuprofen sustained release capsule orally, 2-3 days before menstruation, 0.3g each time, twice a day, continuously take until the menstrual period, the menstrual pain can be relieved or disappeared. 1 menstrual cycle is a course of treatment. Three consecutive courses of treatment. After the end of three courses of treatment to observe the short-term effect, and to stop treatment for 3 months follow-up long-term effect. Dysmenorrhea score was used to determine the curative effect. Results: there were significant differences in the scores of dysmenorrhea between the catgut embedding group and the western medicine group before treatment (P 0.01). The long-term dysmenorrhea score of the catgut embedding group was compared with that of the control group. There was no significant difference in long-term dysmenorrhea score between western medicine group and before treatment. In recent clinical effect comparison, 13 cases were cured, 9 cases were effective, 6 cases were effective, 2 cases were ineffective in western medicine group, 5 cases were cured in western medicine group, 5 cases were cured in western medicine group, 5 cases were cured in western medicine group, 5 cases were cured in western medicine group, and 5 cases were cured in western medicine group. There were 7 cases of remarkable effect, 8 cases of effective and 10 cases of failure. Compared with the long-term clinical effect, 15 cases were cured, 11 cases were effective, 2 cases were effective, 2 cases were ineffective, 5 cases were cured, 4 cases were effective and 6 cases were effective in western medicine group. Conclusion the curative effect of stage catgut implantation on primary dysmenorrhea is definite, and the short-term effect can be achieved. With the accumulation of catgut embedding effect and the improvement of body quality, the long-term effect is considerable. (2) Catgut embedding plays a role in the benign regulation of the body. Line body absorption takes time, early effect will be slower, but the effect is persistent and stable, not easy to recur.) 1 menstrual cycle only needs to be treated twice, compared with oral western medicine, greatly reduce the number of times of treatment. At the same time, it can ensure the continuity and effectiveness of the treatment, and it is easy to operate, safe and economical. The immediate analgesic effect of ibuprofen sustained release capsule is remarkable, and the symptoms can be relieved quickly, but most of the effects are not sustainable. The treatment of primary dysmenorrhea is worth popularizing in the treatment of primary dysmenorrhea.
