【摘要】：目的:探討脾胃虛弱型慢性萎縮性胃炎胃粘膜上皮病變的中醫(yī)病機,觀察臨床療效,旨在完善慢性萎縮性胃炎的診治思路、為醫(yī)患提供優(yōu)選健康宣教方案。方法:系統(tǒng)回顧《內(nèi)經(jīng)》、《靈樞》、《傷寒論》、《金匱要略》、《難經(jīng)》等醫(yī)籍,挖掘胃粘膜上皮病變的中醫(yī)病機,總結(jié)健康宣教要點。以臨床符合納入標準60例慢性萎縮性胃炎患者為研討對象,按隨機對照的原則分為治療組及對照組,兩組病例的基本情況、癥狀、體征、胃鏡及病理等均無統(tǒng)計學差異。治療組服用六君子湯化裁方,每日1劑,早晚飯后半小時分服,每次200ml;對照組予胃復春片飯前半小時口服,一次4片,一日3次。一個療程為6個月,服藥1個療程后復查胃鏡及病理,對患者治療前后的癥狀、體征、胃鏡、病理進行統(tǒng)計學分析,判斷治療效果。結(jié)果:補益脾胃法的治療組在改善癥狀、胃鏡及病理分級療效上均優(yōu)于對照。結(jié)論:補益脾胃法治療脾胃虛弱型慢性萎縮性胃炎不僅可改善臨床癥狀,而且可改善明顯內(nèi)鏡下粘膜及病理組織學形態(tài)。
[Abstract]:Objective: to investigate the pathogenesis of chronic atrophic gastritis of spleen and stomach, observe the clinical effect and improve the diagnosis and treatment of chronic atrophic gastritis. Methods: a systematic review of the medical books such as Internal Classic >, Lingshu >, Treatise on febrile Disease, synopsis of Golden Chamber and difficult Sutra was carried out to excavate the pathogenesis of gastric mucosal epithelial lesions. The main points of health education were summarized. 60 patients with chronic atrophic gastritis were divided into treatment group and control group according to the principle of random control. There was no statistical difference in gastroscopy and pathology. The treatment group was treated with Liujunzi decoction, 1 dose per day, 200 ml in the morning and evening half an hour after the meal, while the control group was given orally half an hour before the meal, 4 tablets once a day, the control group was treated with Weifuchun tablet half an hour before the meal. Three times a day. The course of treatment was 6 months. After one course of treatment, the symptoms, signs, endoscopy and pathology of the patients before and after treatment were analyzed statistically. Results: the treatment group of tonifying spleen and stomach was superior to the control group in improving symptoms, gastroscopy and pathological grading. Conclusion: tonifying spleen and stomach can not only improve the clinical symptoms of chronic atrophic gastritis with asthenia of spleen and stomach, but also improve the clinical symptoms of chronic atrophic gastritis. Moreover, it can improve the mucosal and histopathologic morphology under endoscope.
