基于fMRI技術(shù)針刺足太陽(yáng)膀胱經(jīng)飛揚(yáng)穴腦激活區(qū)特異性研究

【摘要】:目的:應(yīng)用血氧水平依賴(lài)功能磁共振成像技術(shù)(Bold-fMRI)獲得健康被試者針刺右側(cè)足太陽(yáng)膀胱經(jīng)飛揚(yáng)穴及其對(duì)應(yīng)非穴引起腦激活區(qū)數(shù)據(jù),結(jié)合中醫(yī)經(jīng)絡(luò)及腦神經(jīng)學(xué)知識(shí)試探尋針刺飛揚(yáng)穴產(chǎn)生療效的機(jī)制,通過(guò)對(duì)針刺飛揚(yáng)穴與對(duì)應(yīng)非穴引起腦激活區(qū)進(jìn)行比較,證明針刺飛揚(yáng)穴可引起相關(guān)腦區(qū)特異性激活,為腧穴特異性研究提供更直觀可視化影像學(xué)依據(jù)。方法:選取20名黑龍江中醫(yī)藥大學(xué)在讀健康男性右利手研究生作為本次實(shí)驗(yàn)被試者,使用飛利浦3.0T雙源雙梯度磁共振掃描儀收集被試者針刺右側(cè)足太陽(yáng)膀胱經(jīng)飛揚(yáng)穴及其非穴的腦Bold信號(hào)圖像數(shù)據(jù),通過(guò)SPM12軟件對(duì)圖像數(shù)據(jù)進(jìn)行后處理及統(tǒng)計(jì)學(xué)分析后,最終獲得針刺飛揚(yáng)穴及對(duì)應(yīng)非穴引起的腦激活區(qū)位置、體素值、激活程度等信息。結(jié)果:1.針刺健康被試者右側(cè)飛揚(yáng)穴引起正激活腦區(qū)主要包括:左側(cè)中央后回、小腦、顳上回、頂葉,右側(cè)小腦、島葉、中腦、丘腦、顳上回、額中回。2.針刺健康被試者右側(cè)飛揚(yáng)穴引起負(fù)激活腦區(qū)主要包括:左側(cè)邊緣葉、梭狀回、前扣帶回、中央后回、中腦,右側(cè)海馬回、杏仁核、額葉、小腦后葉、枕葉。3.針刺健康被試者右側(cè)飛揚(yáng)穴非穴引起的激活腦區(qū)主要包括:左側(cè)小腦前葉、島葉、頂下小葉、邊緣葉、額上回,右側(cè)額中回、島葉。4.針刺健康被試者右側(cè)飛揚(yáng)穴非穴無(wú)負(fù)激活腦區(qū)。結(jié)論:1.針刺健康被試者右側(cè)飛揚(yáng)穴可引起腦激活區(qū)呈現(xiàn)特異性反應(yīng)。2.針刺同一經(jīng)絡(luò)上不同腧穴可引起相似的大腦反應(yīng)。3.針刺飛揚(yáng)穴治療腰椎間盤(pán)突出癥引起疼痛的機(jī)制可能與其調(diào)節(jié)下丘腦、中腦、海馬回、杏仁核及扣帶回等多個(gè)腦區(qū)神經(jīng)元活動(dòng)有關(guān)。4.針刺飛揚(yáng)穴非穴引起的激活腦區(qū)可能與被試者"應(yīng)激狀態(tài)"有關(guān)。
[Abstract]:Objective: to obtain the data of brain activation region caused by acupuncture of the right foot and bladder through Feiyang point and its corresponding non-point by using blood oxygen level dependent functional magnetic resonance imaging technique (Bold-f MRI) in healthy subjects. Combined with the knowledge of meridian and neurology of traditional Chinese medicine, this paper tries to explore the mechanism of finding acupuncture at Feiyang point to produce curative effect. By comparing the brain activation area caused by acupuncture at Feiyang point and corresponding non-point, it is proved that acupuncture at Feiyang point can cause specific activation of relevant brain region. Methods: twenty healthy male right-handed graduate students in Heilongjiang University of traditional Chinese Medicine were selected as subjects. Philips 3.0T dual-source double-gradient magnetic resonance scanner was used to collect the brain Bold signal data of the right foot, which were punctured by the right foot, the bladder meridian point and its non-point. The image data were processed and statistically analyzed by SPM12 software. Finally, the information of the position, voxel value and activation degree of brain activation area caused by acupuncture at Feiyang point and its corresponding non-acupoints were obtained. Results 1. The positive activation area caused by acupuncture at the right Feiyang point of healthy subjects mainly includes: left posterior central gyrus, cerebellum, superior temporal gyrus, etc. Parietal lobe, right cerebellum, island lobe, midbrain, thalamus, superior temporal gyrus, middle frontal gyrus. 2. Acupuncture of right Feiyang point in healthy subjects mainly includes: left marginal lobe, fusiform gyrus, anterior cingulate gyrus, central and posterior gyrus, midbrain. Right hippocampus, amygdaloid nucleus, frontal lobe, posterior cerebellar lobe, occipital lobe, and occipital lobe. The active brain areas caused by acupuncture at right Feiyang point mainly include: left cerebellum anterior lobe, island lobe, inferior parietal lobule, marginal lobe, superior frontal gyrus, right middle frontal gyrus, right middle frontal gyrus. (4) Acupuncture of the right Feiyang point of the healthy subjects has no negative activation of brain area. Conclusion: 1. Acupuncture on the right side of the Feiyang point of the healthy subjects can cause a specific response of the brain activation region. 2. Acupuncture at different acupoints on the same meridian may cause the similarity. The mechanism of acupuncture at Feiyang point in the treatment of pain caused by lumbar disc herniation may be similar to its regulation of hypothalamus. The activity of neurons in the mesencephalon, hippocampal gyrus, amygdala and cingulate gyrus is related to .4.Activating brain area caused by acupuncture at Feiyang point may be related to the "stress state" of the subjects.
