【摘要】：目的運用“中醫(yī)傳承輔助平臺”系統(tǒng)挖掘柴可群教授治療胃癌的用藥數(shù)據(jù),總結(jié)柴可群教授治療胃癌的臨床經(jīng)驗,使之能夠進一步指導(dǎo)中醫(yī)藥在胃癌治療中的運用。方法采用回顧性調(diào)查的方法,收集柴可群教授2014年01月至2017年01月診治的胃癌病案,將所用病案資料依次錄入“中醫(yī)傳承輔助平臺”系統(tǒng)(V2.50),并運用其集成的規(guī)則分析、改進互信息法、復(fù)雜系統(tǒng)熵聚類及無監(jiān)督的熵層次聚類等數(shù)據(jù)挖掘方法,分析柴可群教授治療胃癌的用藥及總結(jié)經(jīng)驗。結(jié)果對108例胃癌患者,480個處方進行分析,統(tǒng)計處方中藥物的使用頻次,藥物之間的關(guān)聯(lián)規(guī)則,演化得到核心組合23個、新處方13個。結(jié)論通過對柴可群教授治療胃癌用藥的分析總結(jié),得出柴可群教授在治療胃癌特點:基于頻數(shù)分析可知以扶正藥為主,祛邪藥為輔,適時佐以理氣藥,其中扶正藥以補氣、補陰較多,祛邪藥則以化痰散結(jié)、清熱解毒、化瘀散毒多見;基于關(guān)聯(lián)規(guī)則分析得出的核心處方是太子參,茯苓,白術(shù),甘草,半夏,黃芩,薏米仁;基于熵聚類的組方規(guī)律分析得出13新處方可歸類為以下幾種:①顧護脾胃,②疏肝和胃,③通降胃腑,④化痰解毒,⑤溫陽化瘀,⑥經(jīng)方化裁。認(rèn)為胃癌的基本病機是“正虛為本、癌毒為患、情志失暢”,辨治胃癌主要有以下幾個特點:①病證結(jié)合,中西合參;②扶正培本、抗癌祛邪;③整體局部,三因制宜;④分期而治,貴在對癥;⑤通降胃腑,疏理氣機;⑥溫陽通絡(luò),顧及氣陰;⑦調(diào)暢情志,注意飲食;⑧處方執(zhí)簡,用藥輕靈。
[Abstract]:Objective to excavate the data of professor Chai Kequn in the treatment of gastric cancer by using the system of "Chinese Medicine inheriting and assisting platform", and summarize the clinical experience of Professor Chai Kequn in the treatment of gastric cancer. Methods retrospective investigation was used to collect the medical records of gastric cancer diagnosed and treated by Professor Chai Kequn from January 2014 to January 2017. The data of medical records are input into the system of "Chinese Medicine inheritance and Assistance platform" in turn, and the data mining methods such as mutual information method, entropy clustering of complex systems and unsupervised entropy hierarchical clustering are improved by using the integrated rule analysis. Results 480 prescriptions of 108 patients with gastric cancer were analyzed, the frequency of drug use in prescriptions, association rules between drugs were counted, and 23 core combinations were obtained. Conclusion the characteristics of professor Chai Kequn in the treatment of gastric cancer are as follows: based on the analysis of frequency, we can know that the main medicine is to strengthen the qi, the other is to dispel evil, and to assist in the treatment of gastric cancer. Among them, tonifying qi, tonifying yin, expelling evil drugs are phlegm dispersing, clearing away heat and detoxification, removing blood stasis and dispersing poison, the core prescription based on association rule is Radix Pseudostellariae, Poria cocos, Atractylodes macrocephala, Glycyrrhiza uralensis, Pinellia ternata, Scutellaria baicalensis, and Yimiren; Based on the rule analysis of entropy clustering, the 13 new prescriptions can be classified as the following kinds: 1: 1, protecting the spleen and stomach, 2 soothing the liver, and 3 opening up the stomach to reduce phlegm and detoxify the stomach. Fang Hua considers that the basic pathogenesis of gastric cancer is "positive deficiency is the basis." Differentiation and treatment of gastric cancer are mainly characterized by the following characteristics: 1. Combination of disease and syndromes, combination of Chinese and Western medicine, combination of ginseng and 2 tonics, anti-cancer, anti-evil, anti-cancer, anti-evil and anti-cancer, three should be treated by stages, mainly in treating the stomach by five channels and lowering the stomach entrails, as a result of the treatment of the disease by stages.
