【摘要】：目的:探討兒童過敏性紫癜臨床用藥規(guī)律,為臨床治療提供思路。方法:選擇山東中醫(yī)藥大學(xué)附屬醫(yī)院兒科病房2011年9月-2015年9月的兒童過敏性紫癜住院病例227例,錄入中醫(yī)傳承輔助系統(tǒng)軟件進(jìn)行分析,得出結(jié)論。結(jié)果:(1)所納入的227例病例中,男女比例為1.24:1,其中以5~9歲年齡段為主,病程多集中于1個(gè)月以內(nèi),皮膚型占74.4%,腹型占14.5%,關(guān)節(jié)型占6.2%,混合型占2.7%,腎型占2.2%。證型分布依次為:風(fēng)熱傷絡(luò)、濕熱痹阻、血熱妄行、氣不攝血、陰虛火旺。(2)常用前十位中藥依次為甘草、蒼術(shù)、紫草、防風(fēng)、生地黃、荊芥、薏苡仁、牛蒡子、苦參、黃柏;清熱藥及解表藥為主要用藥類型;藥物四氣以寒性藥物為主,五味以苦味藥物為主,藥物歸肝經(jīng)者居多;四妙散、消風(fēng)散、甘露消毒丹、犀角地黃湯、補(bǔ)中益氣湯為高頻處方。(3)得到以甘草、蒼術(shù)為代表的25個(gè)常用藥物組合及其關(guān)聯(lián)規(guī)則;得到關(guān)聯(lián)度在0.03以上的以防風(fēng)、蟬蛻為代表的藥對共31對;得到以淡豆豉、金銀花、蘆根為代表的藥物核心組合16對,進(jìn)一步聚合成新處方共8首。結(jié)論:本病用藥多以清熱藥、解表藥、補(bǔ)虛藥、利水滲濕藥為主,方藥功效主要為祛風(fēng)清熱、解毒活血、清熱祛濕、健脾益氣,提示風(fēng)、濕、熱、瘀為主要致病因素,臨床治法、方藥、藥物組合、新方聚合等均是圍繞此四類制定和配伍的。
[Abstract]:Objective: to explore the rule of clinical drug use in children with Henoch-Schonlein purpura and to provide some ideas for clinical treatment. Methods: a total of 227 cases of children with Henoch-Schonlein purpura from September 2011 to September 2015 in pediatric ward of affiliated Hospital of Shandong University of Chinese Medicine were selected. The results showed that the ratio of male to female was 1.24: 1 out of the 227 cases included in the study. Among them, the age group was 5 to 9 years old, and the course of disease was mostly within one month. The skin type accounted for 74.4%, abdominal type 14.5%, joint type 6.2%, mixed type 2.7%, kidney type 2.2. The distribution of syndrome type was as follows: wind and heat injury, dampness and heat obstruction, blood heat disorder, qi not absorbing blood, yin deficiency and fire flourishing, etc.) the top ten Chinese medicines were Glycyrrhiza, Atractylodes and Rhizoma Rhizome. Fangfeng, Rehmannia, Schizonepeta, Coix seed, Fructus Arctii, Sophora flavescens, Cortex Phellodendri; Heat-clearing drugs and antiepidermic drugs are the main drug types; the four drugs are mainly cold drugs, the five flavors are mainly bitter drugs, and the drugs belong to the liver Meridian, most of them belong to the liver Classic; Simiao San, Xiaofeng San, Gannuxiao Dan, Rhinoceros Corner Rehmannia decoction, Buzhong Yiqi decoction as high frequency prescription. 3) 25 common drug combinations and association rules represented by Licorice and Atractylodes were obtained, and Fangfeng with correlation degree above 0.03 was obtained. A total of 31 pairs of drug pairs represented by cicada slough were obtained. 16 pairs of drug core combinations, represented by black bean, honeysuckle and root, were obtained, and further aggregated into 8 new prescriptions. The efficacy of the prescription is mainly to dispel wind and clear heat, detoxify and activate blood, clear heat and remove dampness, invigorate spleen and invigorate qi, suggest that wind, dampness, heat and blood stasis are the main pathogenic factors, clinical treatment method, prescription medicine, drug combination, Xinfang polymerization and so on are around these four categories of formulation and compatibility.
