【摘要】：目的：從癥狀、皮損及體征變化等方面觀察、探討敏膚煎冷敷治療面部激素依賴性皮炎的臨床療效,分析此法可能的作用機(jī)理,比較該療法兩種不同處方的療效,期望能進(jìn)一步規(guī)范化、標(biāo)準(zhǔn)化中藥外敷治療面部激素依賴性皮炎的方法,以對(duì)面部激素依賴性皮炎的臨床治療提供更多的參考信息,發(fā)揮一定的臨床作用。方法：本研究采用數(shù)字隨機(jī)、對(duì)照的方法,將符合納入標(biāo)準(zhǔn)、簽署知情同意書的72例患者隨機(jī)分為治療組和對(duì)照組。治療組采用敏膚煎冷敷治療。對(duì)照組采用三黃洗劑加味冷敷患處。兩組病患均口服黃鶯教授治療過敏性疾病的中藥經(jīng)驗(yàn)方免煎顆粒,療程4周。觀察治療前后相應(yīng)指標(biāo)的改善情況。借助統(tǒng)計(jì)學(xué)軟件SPSS17.0對(duì)各組資料進(jìn)行統(tǒng)計(jì)分析,作出療效判定及結(jié)果分析。結(jié)果：①兩組治療前后分別對(duì)紅腫、灼熱、瘙癢、鱗屑、皮損面積等觀察指標(biāo)的積分進(jìn)行組內(nèi)比較,各項(xiàng)指標(biāo)積分比較P捚0.05,差異有統(tǒng)計(jì)學(xué)意義,說明兩觀察組對(duì)面部激素依賴性皮炎(風(fēng)熱證)在紅腫、灼熱、瘙癢、鱗屑、皮損面積等方面均有明顯改善。②兩組治療后綜合療效組間比較,治療組總有效率93.8%,對(duì)照組總有效率96.8%,P0.05,差異無統(tǒng)計(jì)學(xué)意義,說明兩組綜合療效相當(dāng)；③兩組單項(xiàng)觀察指標(biāo)的療效指數(shù)組間比較,瘙癢方面P=0.000.05,差異有統(tǒng)計(jì)學(xué)意義,說明敏膚煎在改善瘙癢方面優(yōu)于三黃洗劑加味。皮損面積方面,對(duì)照組的變異系數(shù)大于對(duì)照組,認(rèn)為對(duì)照組改善優(yōu)于治療組。紅腫、灼熱和瘙癢方面,P0.05,差異無統(tǒng)計(jì)學(xué)意義,說明兩觀察組對(duì)紅腫、灼熱、鱗屑等方面療效無差異。④治療組有1例未遵醫(yī)囑使用,自覺癥狀加重,囑冷開水濕敷后緩解,繼續(xù)使用無明顯不良反應(yīng),說明敏膚煎外用安全可靠。結(jié)論：1、敏膚煎與三黃洗劑加味在治療面部激素依賴性皮炎的紅腫,灼熱和鱗屑方面療效相當(dāng)。2、改善瘙癢方面,敏膚煎優(yōu)于三黃洗劑加味：改善皮損面積方面三黃洗劑優(yōu)于敏膚煎。3、敏膚煎短期療效顯著,藥物價(jià)格低廉,操作方便,副作用小,值得在臨床上推廣和進(jìn)一步研究。
[Abstract]:Objective: to investigate the clinical efficacy of Minfu decoction cold compress in treating facial hormone dependent dermatitis from the aspects of symptoms, lesions and signs, analyze the possible mechanism of this method, and compare the effects of two different prescriptions. We hope to further standardize and standardize the methods of external application of traditional Chinese medicine for the treatment of facial hormone dependent dermatitis, so as to provide more reference information for the clinical treatment of facial hormone dependent dermatitis. Methods: in this study, the digital randomized, controlled method will meet the inclusion criteria, 72 patients with informed consent were randomly divided into treatment group and control group. The treatment group was treated with Minfu decoction cold compress. The control group was treated with Sanhuang lotion plus modified cold compress. Diseased Chinese medicine experience prescription Fan-Fen granules, After 4 weeks of treatment, we observed the improvement of the corresponding indexes before and after treatment. The statistical software SPSS17.0 was used to analyze the data of each group, and the curative effect and the result were analyzed. Results before and after treatment, the two groups were treated with red swelling, burning heat, pruritus, and scales, respectively. The scores of the observed indexes, such as the area of skin lesion, were compared within the group, and the scores of the indexes were compared P 0.05, the difference was statistically significant, which indicated that the two groups had a significant difference in the treatment of facial hormone dependent dermatitis (wind-heat syndrome) in redness and swelling, burning heat, pruritus, scaly, and so on. The total effective rate of the treatment group was 93.8, the total effective rate of the control group was 96.8g / P 0.05, there was no significant difference between the two groups. It shows that the two groups have the same comprehensive curative effect and the curative effect index of the two groups is comparable. The pruritus is 0.000.05, the difference is statistically significant. It shows that Minfu decoction is better than Sanhuang lotion in improving itching. The area of skin lesion is better than that of Sanhuang lotion. The coefficient of variation of the control group was higher than that of the control group, and the improvement of the control group was better than that of the treatment group. There was no difference in the therapeutic effect of scale. 4 in the treatment group, 1 case did not follow the doctor's advice, the symptoms of the treatment group were aggravated, the cold and boiled water wet compress was ordered to relieve, and there was no obvious adverse reaction in the treatment group. ConclusionThe addition of Minfu decoction and Sanhuang lotion is effective in treating facial hormone dependent dermatitis, burning heat and scales, and improving pruritus, it is safe and reliable for external use of Minfu decoction, conclusion: 1: 1, Minfu decoction and Sanhuang lotion are effective in treating facial hormone dependent dermatitis. Minfu decoction is superior to Sanhuang lotion in improving the area of skin lesion. It is worth popularizing and further studying in clinic because of its obvious short-term curative effect, low price, convenient operation and small side effect.
