【摘要】：目的：通過觀察益氣活血法預(yù)防全膝關(guān)節(jié)表面置換術(shù)后下肢深靜脈血栓形成的臨床療效,以更好的探討中醫(yī)藥防治下肢深靜脈血栓的作用機制,為中醫(yī)藥防治全膝關(guān)節(jié)表面置換術(shù)后下肢深靜脈血栓提供循證醫(yī)學(xué)支持。方法：根據(jù)病例納入標準及病例排除標準,于2015年1月至2016年1月共收集60例患者,隨機分為試驗組30例(口服益氣活血湯聯(lián)合靜滴血栓通注射液)和30例對照組(口服立伐沙班)。符合研究標準的患者術(shù)后均采用相同的基本預(yù)防措施,由同一組醫(yī)生完成手術(shù),均行單側(cè)膝關(guān)節(jié)表面置換術(shù),進行臨床觀察研究。結(jié)果：1、術(shù)后第一天,凝血酶原時間、D-二聚體較術(shù)前具有統(tǒng)計學(xué)意義(P0.01)。2、術(shù)后第3天紅細胞計數(shù)、紅細胞壓積、血紅蛋白濃度、纖維蛋白原、活化部分凝血酶時間、凝血酶時間較術(shù)前具有統(tǒng)計學(xué)意義(P0.01或P0.05)。3、術(shù)后第10天紅細胞計數(shù)、血紅蛋白、血小板及大腿腫脹率兩組組間結(jié)果有統(tǒng)計學(xué)意義(P0.05),說明試驗組優(yōu)于對照組；4、兩組患者術(shù)后第10天通過下肢靜脈彩超證實均無深靜脈血栓發(fā)現(xiàn),且兩組組間比較無顯著差異(P0.05)。結(jié)論：口服益氣活血湯聯(lián)合靜滴血栓通注射液不僅可以有效改善全膝關(guān)節(jié)表面置換術(shù)后血液的粘稠度,延長凝血時間,有效減低術(shù)后下肢深靜脈血栓的發(fā)生率；而且可以減少術(shù)后出血,減輕大腿腫脹率,降低貧血發(fā)生率。綜上所述,該法具有療效明確、價格低廉、患者接受度高、安全性好等特點,值的臨床大力推廣。
[Abstract]:Objective: to observe the clinical effect of supplementing qi and activating blood circulation method to prevent deep vein thrombosis of lower extremity after total knee arthroplasty, and to explore the mechanism of prevention and treatment of deep vein thrombosis of lower extremity by traditional Chinese medicine. To provide evidence-based medical support for the prevention and treatment of deep venous thrombosis of lower extremity after total knee arthroplasty with traditional Chinese medicine. Methods: from January 2015 to January 2016, 60 patients were collected according to the criteria of case inclusion and exclusion. They were randomly divided into trial group (30 cases) (oral Yiqi Huoxue decoction combined with intravenous drip of Xueshuantong injection) and control group (30 cases of control group). Patients who met the standard of study were treated with the same basic preventive measures after operation, and were operated by the same group of doctors. Results on the first day after operation, prothrombin time of D-dimer was significantly higher than that of pre-operation (P 0.01), erythrocyte count, hematocrit and hemoglobin concentration on the 3rd day after operation. Fibrinogen, activated partial thrombin time, thrombin time were significantly higher than those before operation (P0.01 or P0.05). The results of platelet and thigh swelling rates between the two groups were statistically significant, which indicated that the experimental group was superior to the control group (P 0.05). On the 10th day after operation, no deep venous thrombosis was found in both groups. There was no significant difference between the two groups (P 0.05). Conclusion: oral Yiqi Huoxue decoction combined with intravenous drip of Xueshuantong injection can not only effectively improve the viscosity of blood after total knee joint surface replacement, but also prolong the coagulation time. It can effectively reduce the incidence of postoperative deep venous thrombosis of lower extremities, and can reduce postoperative bleeding, reduce the rate of thigh swelling, and reduce the incidence of anemia. Good safety and other characteristics, value of the clinical vigorously promote.
