【摘要】：目的:觀察手針聯(lián)合電針與單純手針治療腦卒中手痙攣患者的臨床療效差異。方法:將60例腦卒中手痙攣患者隨機分為手針組和手針聯(lián)合電針組,每組30例。手針組穴取患側(cè)肩毭、臂佈、肩毼、曲池、手三里、外關(guān)、伏兔、梁丘、血海、足三里、上巨虛、下巨虛、陰陵泉、三陰交、太溪、太沖以及各掌骨骨間背側(cè)肌中央;手針聯(lián)合電針組在手針組的基礎(chǔ)上于外關(guān)和第2、3掌骨骨間背側(cè)肌中央接以斷續(xù)波型的電針儀。兩組均留針40 min,每天治療1次,每周5次,4周為一療程,共治療3個療程。觀察兩組患者治療前后改良Ashworth分級(MAS)、Fugl-Meyer手指運動功能評定量表以及改良Barthel指數(shù)(BI)。結(jié)果:兩組患者治療后MAS較治療前分級均下降(均P0.05);治療后,手針聯(lián)合電針組患者MAS較手針組分級降低(P0.05);兩組患者治療后Fugl-Meyer手指運動功能評分和BI均較治療前提高(均P0.05);治療后,手針聯(lián)合電針組Fugl-Meyer手指運動功能評分和BI均高于手針組(均P0.05)。結(jié)論:手針聯(lián)合電針治療腦卒中手痙攣與單純手針相比,更能夠緩解腦卒中患者手的痙攣程度、促進手功能的恢復(fù),提高患者生存質(zhì)量。
[Abstract]:Objective: to observe the difference of clinical effect between hand acupuncture combined with electroacupuncture and simple hand acupuncture in the treatment of hand spasm after stroke methods: 60 patients with hand spasm were randomly divided into hand acupuncture group and hand acupuncture combined electroacupuncture group. There were 30 cases in each group. The acupoints of the hand acupuncture group were collected from the affected side of the shoulder, arm cloth, shoulder, bend pool, hand Sanli, Waiguan, Fu Rabbit, Liang Qiu, Xuehai, Zusanli, Shangzuxu, Lower Juxu, Yinling Spring, Sanyinjiao, Taixi, Taichong and the center of the intermetacarpal interosseous dorsal muscle, the hand acupuncture combined with electroacupuncture group and the hand acupuncture group received intermittent wave electroacupuncture at the center of the intermetacarpal interosseous dorsal muscle on the basis of the hand acupuncture group. Both groups were treated once a day for 40 minutes. Five times a week for 4 weeks as a course of treatment, Before and after treatment, the modified Ashworth grading and the modified Barthel index were observed. Results: after treatment, the MAS of the two groups was lower than that before and after treatment (all P 0.05; after treatment, P < 0.05). The MAS of the patients in the hand acupuncture combined with electroacupuncture group was lower than that of the hand acupuncture group, and the score of Fugl-Meyer finger motor function and the BI of the patients in the two groups were higher than those before the treatment (all P 0.05; after treatment), the patients in the two groups were significantly lower than those in the control group (P < 0.05). The Fugl-Meyer score and BI of hand acupuncture combined with electroacupuncture group were higher than those of hand acupuncture group (all P 0.05). Conclusion: the hand spasm of stroke patients treated by hand acupuncture combined with electroacupuncture can relieve the degree of hand spasm more than that of hand acupuncture alone. To promote the recovery of hand function and improve the quality of life of patients.
