【摘要】：緊張型頭痛是臨床常見(jiàn)的僅次于偏頭痛的原發(fā)性頭痛,主要表現(xiàn)為雙側(cè)顳部、枕項(xiàng)部、頭頂、前額或全頭部的緊縮性、壓迫性疼痛。在社會(huì)生活節(jié)奏不斷加快的當(dāng)今,該病患病率呈上升趨勢(shì),成為影響患者生活質(zhì)量的常見(jiàn)疾病。該研究通過(guò)對(duì)比常規(guī)針刺治療緊張型頭痛,觀(guān)察排針平刺法對(duì)緊張型頭痛患者不同時(shí)間段的療效及對(duì)顱周肌肉壓痛的作用。目的：觀(guān)察排針平刺治療緊張型頭痛不同時(shí)間段的止痛效果,以及對(duì)顱周肌肉壓痛的作用。方法：從南京市中醫(yī)院腦病科頭痛門(mén)診及病房緊張型頭痛患者中篩選出60例,根據(jù)隨機(jī)編碼表,隨機(jī)分為排針針刺組(A組)和常規(guī)針刺組(B組),各30例。A組采用排針平刺法治療,枕穴組+顳穴組+頂穴組留針6小時(shí)；B組采用常規(guī)針刺,取頭維、太陽(yáng)、風(fēng)池、足三里、三陰交、太沖和頸夾脊穴,留針30分鐘。兩組均隔日治療一次,一共治療6次。記錄針刺后即刻、6h、1周及兩周后頭痛評(píng)分及顱周肌肉壓痛評(píng)分,將記錄到的數(shù)據(jù)用SPSS17.0軟件進(jìn)行分析處理。結(jié)果：在減輕頭痛程度上,兩組治療后較治療前均明顯改善,其中即刻、6h、1周時(shí)排刺組在緩解頭痛程度方面明顯優(yōu)于常規(guī)針刺組(P0.01),在2周時(shí)兩組評(píng)分無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)；在改善顱周肌肉壓痛方面,兩組2周治療后均較治療前改善,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論：(1)排針平刺法較常規(guī)針刺對(duì)緊張型頭痛有良好的即刻止痛效果,且療效穩(wěn)定持久,在減輕疼痛幅度上優(yōu)于常規(guī)針刺,但兩組總療效差異不具有統(tǒng)計(jì)學(xué)意義；(2)排針平刺與常規(guī)針刺均能夠改善患者的顱周肌肉壓痛,差異不具有統(tǒng)計(jì)學(xué)意義。
[Abstract]:Tension type headache is a common clinical migraine after primary headache, mainly for bilateral temporal, occipital head, forehead tight or whole head, compression pain. The accelerating pace of social life today, the prevalence of the disease is on the rise, as the common diseases affecting patients life the quality of the research. Through the comparison of the conventional acupuncture treatment of tension type headache, observation of curative effect of needle horizontal needling headache in patients with different time of tension type and effect on pericranial muscle tenderness. Objective: To observe the analgesic effect of acupuncture needling in treating tension type headache is not flat in the same period, and the role of pericranial muscle tenderness. Methods: from Nanjing City Hospital of traditional Chinese Medicine Department of encephalopathy headache clinic and ward patients with tension type headache in 60 cases were selected, according to the random encoding table, randomly divided into acupuncture acupuncture group (A group) and routine acupuncture group (B group), 30 patients in each group.A The pin horizontal needling therapy, pillow point group + temporal point group + top point group for 6 hours; B group was treated with conventional acupuncture, taking the first dimension, the sun, Fengchi, Zusanli, Sanyinjiao, Taichong and Jiaji points, 30 minutes for. The two groups were treated once a day, a total of 6 times of treatment. Recorded immediately after acupuncture, 6h score and pericranial muscle tenderness score of headache for 1 weeks and two weeks after the recorded data was analyzed by SPSS17.0. Results: in relieving headache degree, the two groups after treatment than before treatment were significantly improved, which immediately, 6h, 1 week row needling group to relieve headaches extent is superior to conventional acupuncture group (P0.01), at 2 weeks, scores of the two groups had no statistical difference (P0.05); in the improvement of pericranial muscle tenderness, two groups after 2 weeks of treatment were better than those before treatment, the difference was not statistically significant (P0.05). Conclusion: (1) thrust pin compared with the conventional method of acupuncture on tension type headache Have a good immediate analgesic effect and the curative effect is stable, ease the pain in amplitude is superior to conventional acupuncture, but the difference of the total effect of the two groups was not statistically significant; (2) pin horizontal needling and routine acupuncture can improve the patients with pericranial muscle tenderness, the difference was not statistically significant.
