【摘要】：目的:觀察探討祛瘀通絡(luò)方預(yù)防術(shù)后深靜脈血栓的臨床療效。方法:收集山東中醫(yī)藥大學(xué)附屬醫(yī)院2015年3月—2016年1月關(guān)節(jié)骨科住院病例60例并進行統(tǒng)計,其中30例病人從術(shù)后第二天開始予以口服自制中藥祛瘀通絡(luò)方日一劑直至出院,其余30例病人不予中藥。所有病人均進行術(shù)后常規(guī)治療及康復(fù)鍛煉,觀察病人術(shù)后血漿纖維蛋白原、D-二聚體的變化,患肢腫脹程度以及髖關(guān)節(jié)Harris評分。結(jié)果:通過對病人進行的血液檢查結(jié)果的統(tǒng)計分析,兩組患者血漿纖維蛋白原及D-二聚體等指標(biāo)均有所降低,且實驗組降低比率更大,兩組數(shù)據(jù)經(jīng)過t檢驗,差異顯著(P0.01)。實驗組患者在術(shù)后患肢腫脹程度和髖關(guān)節(jié)功能恢復(fù)方面都優(yōu)于對照組,兩組數(shù)據(jù)經(jīng)過Ridit分析,具有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:祛瘀通絡(luò)方在預(yù)防術(shù)后深靜脈血栓方面療效顯著,與低分子肝素皮下注射及患者功能鍛練等治療聯(lián)合應(yīng)用,能最大程度的保障患者術(shù)后的安全,該方法無創(chuàng)傷無痛苦,值得推廣。
[Abstract]:Objective: To observe the clinical efficacy of the prevention of deep vein thrombosis after surgery. Methods: Quyu Tongluo Fang Hospital of Shandong University of Traditional Chinese Medicine in March 2015 January 2016 - joint Department of orthopedics 60 cases and statistics, including 30 cases of patients treated with traditional Chinese medicine preparation started from Quyu Tongluo Recipe on the agent until discharged from the second day after operation, the remaining 30 patients not Chinese. All the patients underwent routine postoperative treatment and rehabilitation exercise, postoperative changes of plasma fibrinogen, two D- dimer, limb swelling degree and Harris hip score. Results: through the statistical analysis of blood test results of patients of the two groups of plasma fibrinogen and D- two dimers index were decreased, and the ratio of the experimental group decreased more, two sets of data through the t test, significant difference (P0.01). The patients in the experimental group limb swelling degree and after hip surgery Joint function recovery is better than the control group, two groups of data through Ridit analysis, with statistical significance (P0.05). Conclusion: Quyu Tongluo Decoction in the prevention of postoperative deep vein thrombosis curative effect significantly, should be combined with low molecular weight heparin subcutaneous injection and patients with functional exercise therapy, patients can maximize security this method is non-invasive, no pain, it is worthy of promotion.
