【摘要】：目的:Meta分析溫補心腎陽氣中藥干預緩慢型心律失常的臨床療效,為溫補心腎陽氣中藥治療緩慢型心律失常應用于臨床提供循證醫(yī)學證據(jù)。方法:制定科學全面的檢索策略,嚴格制定文獻的納入標準和排除標準,用計算機檢索和手動檢索相配合的方法檢索2011-2015年公開發(fā)表的中文文獻,關于溫補心腎陽氣法配合西藥常規(guī)治療緩慢型心律失常的RCT試驗。并將最終納入的文獻進行資料的錄入和Jadad評分,評出每一篇納入文獻的質量,用Rev Man5.3統(tǒng)計軟件進行二分類資料或連續(xù)性資料的meta分析。結果:本次分析一共納入15篇文獻,包括2篇高質量文獻,13篇低質量文獻。共納入病例1354例,其中治療組(溫補心腎陽氣中藥配合基礎西藥)698例患者,對照組(基礎西藥或加安慰劑)656例患者。Meta分析的結果顯示:(1)在提高患者臨床有效率方面:治療組療效優(yōu)于對照組,差異有統(tǒng)計學意義(OR=3.78,95%CI=2.75~5.18,Z=8.23,P0.00001);(2)在改善中醫(yī)證候療效方面:治療組療效優(yōu)于對照組,差異有統(tǒng)計學意義(OR=6.39,95%CI=3.20~12.76,Z=5.26,P0.00001);(3)在提高患者24小時平均心率方面:治療組療效優(yōu)于對照組,差異有統(tǒng)計學意義(MD=5.22,95%CI=4.41~6.03,假設檢驗Z=12.69,P0.00001);(4)在提高患者24小時最慢心率方面:治療組療效優(yōu)于對照組,差異有統(tǒng)計學意義(MD=5.65,95%CI=4.92~6.38,假設檢驗Z=15.16,P0.00001)。結論:溫補心腎陽氣中藥配合常規(guī)西藥治療能夠明顯提高緩慢型心律失常患者臨床有效率,改善患者癥狀,提高患者24小時平均心率及最低心率,臨床療效顯著,且安全性較好。
[Abstract]:Objective to analyze the clinical efficacy of Chinese medicine for warming heart and kidney yang qi in treating bradyarrhythmia, and to provide evidence for clinical application of traditional Chinese medicine for warming heart and kidney yang qi in the treatment of slow arrhythmia. Methods: a scientific and comprehensive search strategy was formulated. To strictly formulate the inclusion criteria and exclusion criteria for literature, and to search the Chinese literature published in 2011-2015 by means of computer retrieval and manual retrieval. The RCT test of tonifying the heart and kidney yang qi combined with the routine western medicine for the treatment of bradyarrhythmia was conducted. The data input and Jadad score of the final literature were recorded, and the quality of each article was evaluated. Rev Man5.3 statistical software was used to analyze the two classification data or continuous data by meta. Results: 15 articles were included in this analysis, including 2 high quality literatures and 13 low quality articles. 1354 cases were included in this analysis. Among them, 698 patients in treatment group (warming heart and kidney yang qi Chinese medicine combined with basic western medicine) and 656 patients in control group (basic western medicine or placebo + placebo 656 patients. Meta analysis showed that: the curative effect of treatment group was better than that of control group) in improving the clinical effective rate of patients. The difference is statistically significant: the curative effect of the treatment group is better than that of the control group, and the difference is statistically significant. The treatment group is better than the control group, and the difference is statistically significant. The treatment group is better than the control group in improving the mean heart rate of 24 hours. The treatment group is superior to the control group in improving the mean heart rate of 24 hours. The difference was statistically significant (P < 0.05). The effect of the treatment group was better than that of the control group in increasing the slowest heart rate in 24 hours. The difference was statistically significant (P < 0.01). Conclusion: the clinical effective rate and symptoms of patients with chronic arrhythmia can be obviously improved by warming the heart and nourishing the kidney yang qi with the Chinese medicine of warming the heart and kidney yang qi, and the routine western medicine, and the hypothesis test is that the Chinese herbal medicine for warming the heart and kidney yang qi combined with the routine western medicine can obviously improve the clinical effective rate and improve the symptoms of the patients with bradyarrhythmia. The improvement of 24-hour mean heart rate and minimum heart rate was effective and safe.
