【摘要】：目的：觀察中藥金黃散外敷治療惡性腫瘤淺表包塊的臨床療效,為臨床惡性腫瘤治療提供一種新方法。方法：本課題主要選取84例腹膜轉(zhuǎn)移癌、肝癌、淋巴結(jié)轉(zhuǎn)移癌疾病患者為例,其中腹膜轉(zhuǎn)移癌30例,肝癌30例,淋巴結(jié)轉(zhuǎn)移癌24例,每天采用金黃散外敷腫瘤淺表包塊或者癌性疼痛部位至腫瘤治療無效或者本課題研究結(jié)束為止。觀察治療后惡性腫瘤包塊的有效率、控制率以及1個(gè)月、3個(gè)月、6個(gè)月療效維持有效率；腫瘤癌性疼痛緩解有效率、1個(gè)月、3個(gè)月、6個(gè)月療效維持有效率；腫瘤患者KPS評分療效變化情況等。結(jié)果：1.納入本課題符合診斷標(biāo)準(zhǔn)的惡性腫瘤患者84例,其中1例因局部皮膚嚴(yán)重瘙癢、皮疹,給予抗過敏、止癢等對癥處理后退出本試驗(yàn),最終符合標(biāo)準(zhǔn)的患者83例。2.金黃散外敷治療腫瘤有效率為10%～21.73%,控制率40%～56.52%,包塊療效維持1個(gè)月為80%-82%,三個(gè)月有效率為70%-78%,6個(gè)月有效率為40%～72%。其中腫瘤控制有效率以淋巴結(jié)轉(zhuǎn)移癌控制率最高,經(jīng)組間分析,淋巴結(jié)轉(zhuǎn)移癌與腹膜轉(zhuǎn)移癌經(jīng)統(tǒng)計(jì)學(xué)分析,差異具有統(tǒng)計(jì)學(xué)意義(P=0.0350.05),淋巴結(jié)轉(zhuǎn)移癌與肝癌經(jīng)統(tǒng)計(jì)學(xué)分析,差異具有統(tǒng)計(jì)學(xué)意義(P=0.015＜0.05)。3.金黃散外敷治療后腫瘤疼痛有效率90%-96%,1個(gè)月療效維持83%-85%,三個(gè)月有效率為75%～80%,6個(gè)月有效率為70%～75%。其中疼痛緩解以肝癌有效率最高,經(jīng)組間分析,肝癌與腹膜轉(zhuǎn)移癌經(jīng)統(tǒng)計(jì)學(xué)分析,差異具有統(tǒng)計(jì)學(xué)意義(P=0.023＜0.05),淋巴結(jié)轉(zhuǎn)移癌與肝癌經(jīng)統(tǒng)計(jì)學(xué)分析,差異具有統(tǒng)計(jì)學(xué)意義(P=0.025＜0.05)。4.金黃散外敷治療后腫瘤患者KPS評分改善率為43%-52%。結(jié)論：金黃散外敷惡性腫瘤不僅具有控制惡性腫瘤淺表包塊的作用,還具有緩解癌性疼痛和改善患者生存質(zhì)量的作用,值得臨床上進(jìn)一步推廣。
[Abstract]:Objective: to observe the clinical effect of Jinhuangsan on superficial mass of malignant tumor, and to provide a new method for the treatment of malignant tumor. Methods: 84 cases of peritoneal metastatic carcinoma and liver cancer were selected in this study. There were 30 cases of peritoneal metastatic carcinoma, 30 cases of liver cancer and 24 cases of lymph node metastasis. Apply Jinhuangsan to the superficial tumor mass or cancerous pain area every day until the tumor treatment is ineffective or until the end of this study. Observe the effective rate of malignant tumor mass after treatment. The control rate and the effective rate of 1 month, 3 months and 6 months were maintained, and the effective rate of cancer pain relief was maintained at 1 month, 3 months and 6 months. Results 1. 84 patients with malignant tumors who met the diagnostic criteria were included in the study. One patient withdrew from the trial because of severe local skin itching, rash, antiallergic, antipruritic, antipruritic, and other symptomatic treatments. The effective rate of external application of Jinhuangsan in the treatment of tumor was 100.21.73. the control rate was 4056.52.The effective rate was 80-82 for one month, the effective rate for three months was 70-78, and the effective rate for 6 months was 400.72.The effective rate of tumor control was transformed into lymph node. The cancer control rate is the highest. The difference between lymph node metastatic carcinoma and peritoneal metastatic carcinoma was statistically significant, and the difference between lymph node metastatic carcinoma and hepatocellular carcinoma was statistically significant. The difference was statistically significant (P < 0.015 < 0.05) .3.The effective rate of tumor pain after external application of Jinhuang Powder was 90-96.The curative effect maintained 83-85 for one month, 7580 for three months, 70th for 6 months. The highest effective rate was liver cancer, which was analyzed by inter-group analysis. The difference between hepatocellular carcinoma and peritoneal metastatic carcinoma was statistically significant (P < 0.023 < 0.05), and that between lymph node metastatic carcinoma and hepatocellular carcinoma was statistically significant. The difference was statistically significant (P < 0.025 < 0.05) .4.The improvement rate of KPS score in patients with tumor after external application of Jinhuang Powder was 43- 52.Conclusion: external application of Jinhuang Powder can not only control the superficial mass of malignant tumor. It also has the function of relieving cancer pain and improving the quality of life of patients, which is worth further popularizing in clinic.
