【摘要】：目的:對(duì)150例特發(fā)性面神經(jīng)麻痹患者進(jìn)行臨床流行病學(xué)調(diào)查研究,旨在探討該病的發(fā)病、中醫(yī)證型與抑郁狀態(tài)的相關(guān)性,為臨床治療該病提供理論指導(dǎo)。方法:根據(jù)流行病學(xué)調(diào)查方法隨機(jī)選取150例特發(fā)性面神經(jīng)麻痹患者,收集患者基本信息及相關(guān)中醫(yī)癥狀信息,并進(jìn)行漢密爾頓抑郁量表測(cè)試,如實(shí)填寫生活事件量表,從150例患者中得到121例抑郁狀態(tài)基礎(chǔ)上發(fā)生的特發(fā)性面神經(jīng)麻痹患者。歸納該121例患者的一般資料及測(cè)試結(jié)果,將所得結(jié)果運(yùn)用SPSS17.0進(jìn)行因子統(tǒng)計(jì)分析,歸納出特發(fā)性面神經(jīng)麻痹的發(fā)病、相關(guān)影響因素、中醫(yī)證型以及中醫(yī)證型與抑郁狀態(tài)的相關(guān)性。結(jié)果:1.患者在發(fā)病前存在不同程度的負(fù)性事件的不良精神刺激,存在情緒沮喪,興趣減退,精力下降,食欲減退,思維緩慢,社交障礙以及睡眠障礙等癥狀,與本病的發(fā)生有一定的相關(guān)性。2.根據(jù)因子分析所得到的4個(gè)公因子,將抑郁狀態(tài)基礎(chǔ)上發(fā)生的特發(fā)性面神經(jīng)麻痹歸納為四個(gè)中醫(yī)癥候群。3.不同性別、職業(yè)及文化程度患者的抑郁程度具有差異性,但是其關(guān)聯(lián)性并不高;不同病程和年齡段之間患者的抑郁程度無顯著性的差異。結(jié)論:1.通過本課題研究發(fā)現(xiàn),抑郁狀態(tài)基礎(chǔ)上發(fā)生的的特發(fā)性面神經(jīng)麻痹患者與職業(yè)、性別及文化程度等因素具有一定的關(guān)系。同時(shí),女性、文化程度高、腦力勞動(dòng)者的特發(fā)性面神經(jīng)麻痹患者抑郁程度相對(duì)較重。2.特發(fā)性面神經(jīng)麻痹患者的發(fā)病與抑郁狀態(tài)密切相關(guān)。3.抑郁狀態(tài)基礎(chǔ)上發(fā)病的特發(fā)性面神經(jīng)麻痹患者中醫(yī)證型主要有四類:風(fēng)寒侵襲證、外感風(fēng)熱證、痰濕內(nèi)盛證、瘀血阻絡(luò)證,其中風(fēng)寒侵襲證與抑郁狀態(tài)的相關(guān)程度最高。
[Abstract]:Objective: 150 cases of patients with idiopathic facial paralysis by clinical epidemiological investigation and research, to explore the pathogenesis of the disease and the relationship between TCM syndrome and depression, and provide theoretical guidance for clinical treatment of the disease. Methods: according to the epidemiological investigation method of randomly selected 150 patients with idiopathic facial nerve paralysis patients were collected, basic information and related information of TCM symptoms, and Hamilton depression scale test, fill in the life events scale, from patients with idiopathic facial nerve paralysis occurred in 121 cases of depression on the basis of from 150 patients. Summed up the general data and test results of the 121 patients, the results using SPSS17.0 statistical factor the analysis, summarized the pathogenesis of idiopathic facial paralysis, related factors, the correlation between TCM Syndrome Types and TCM syndrome and depression. Results: of the 1. patients before the onset. In the spirit of adverse negative events in different degrees of stimulation, there is depressed, appetite, energy loss, loss of appetite, slow thinking, social disorder and symptoms of sleep disorders, there are 4 factors.2. correlation obtained according to the factor analysis and the occurrence of this disease, depression will occur on the basis of idiopathic facial paralysis is divided into four TCM syndrome.3. in different gender, occupation and the degree of depression of patients with cultural differences, but its relevance is not high; the difference between the different course of disease and age of patients with depression history is no significant. Conclusion: 1. this study found that the in patients with idiopathic facial palsy and occupation depression based on the factors of gender and educational level has a certain relationship. At the same time, female, high education level, brainworkers idiopathic facial paralysis patients The degree of depression is relatively heavy.2. idiopathic facial nerve paralysis and the incidence of depression in patients with.3. is closely related to the depression on the basis of the pathogenesis of idiopathic facial nerve paralysis in patients with TCM syndrome type mainly has four kinds: cold invasion, external wind heat syndrome, phlegm syndrome, blood stasis syndrome, which is related to the degree of cold invasion syndrome and depression.
