【摘要】：頑固性頭痛為臨床常見疾病,其特點表現(xiàn)為緩慢起病、纏綿反復(fù)、經(jīng)久不愈,加之現(xiàn)代社會競爭激烈、發(fā)展迅速,更易受精神、情緒、外界環(huán)境等因素影響而誘發(fā),嚴重影響人們社會功能的發(fā)揮。據(jù)統(tǒng)計[1],人群發(fā)病率達5%左右,女性多見。頑固性頭痛屬于中醫(yī)頭痛范疇,中醫(yī)藥作為我國的特色治療方式,在其治療方面積累了大量寶貴經(jīng)驗,為許多被頭痛長期困擾的患者帶來了福音,對各種原因?qū)е碌念B固性頭痛因療效顯著、癥狀改善明顯而受到肯定。導(dǎo)師李燕梅教授為河南省腦病頭痛知名專家,河南省中西醫(yī)結(jié)合學(xué)會腦病分會主任委員,從事臨床診療及教學(xué)科研工作近40年,擅長各種腦部疾病的中西醫(yī)診療,對古、近、現(xiàn)代眾多醫(yī)家的學(xué)術(shù)理論都有深入研究,且在研習(xí)經(jīng)典著作的基礎(chǔ)上逐步揣摩自己的行醫(yī)思路,尤其在頑固性頭痛的診療方面尤有自己獨到的經(jīng)驗。本文通過對大量相關(guān)文獻的搜集、整理及研究,總結(jié)導(dǎo)師運用中醫(yī)經(jīng)典理論指導(dǎo)臨床治療頑固性頭痛的學(xué)術(shù)思想及臨證經(jīng)驗,希望為中醫(yī)藥在頑固性頭痛的治療方面提供一些新思路及方法。本文分為學(xué)術(shù)思想與經(jīng)驗總結(jié)部分及文獻綜述部分,在經(jīng)驗總結(jié)方面,導(dǎo)師認為頑固性頭痛的發(fā)生與先、后天均有影響;發(fā)病的根本為本虛標實;合邪致病為頭痛頑固的關(guān)鍵;邪深在里、頑結(jié)不散形成頭痛頑固的根本;外因牽宿根,頭痛易反復(fù);調(diào)理在平時,貴在堅持;血分藥在治療中貫穿始末;因病勢利導(dǎo),邪除不傷正;認為頑固性頭痛的發(fā)病與五臟密不可分;將頑固性頭痛分為肝氣郁結(jié)、陰寒凝滯、火熱蘊結(jié)、瘀血滯絡(luò)、痰濁凝滯、肝腎陰虛、脾腎陽虛、氣血兩虧證,治療上靈活辨證采用理肝散結(jié)、溫寒散滯,祛瘀通絡(luò)、滌痰散滯、滋補肝腎、溫脾壯腎、補瀉兼施等治法,并注重用藥透、散、引,使病邪由深到淺,由內(nèi)而外,為很多頭痛患者帶來了福音。文獻綜述部分,對頑固性頭痛的中西醫(yī)研究進展做了系統(tǒng)總結(jié)。
[Abstract]:Intractable headache is a common clinical disease, which is characterized by slow onset, lingering and repeated, long lasting, coupled with fierce competition in modern society, rapid development, and more susceptible to mental, emotional, external environment and other factors. According to statistics [1], the incidence rate of the population is about 5%, and women are more common. Intractable headache belongs to the category of headache of traditional Chinese medicine, Chinese medicine is the characteristic treatment method in our country. It has accumulated a great deal of valuable experience in its treatment, has brought good news to many patients who have suffered from headache for a long time, and has a remarkable curative effect on intractable headache caused by various causes. Professor Li Yanmei, a well-known expert on encephalopathy headache in Henan Province and chairman of the encephalopathy branch of the Henan Society of Integrated Chinese and Western Medicine, has been engaged in clinical diagnosis, treatment, teaching and scientific research for nearly 40 years. Chinese and Western medicine, which are good at various brain diseases, have in-depth research on the academic theories of ancient, modern and modern doctors, and have gradually figured out their own thinking of practicing medicine on the basis of studying classical works. Especially in the diagnosis and treatment of intractable headache have their own unique experience. This article through a large number of related literature collection, collation and research, Summing up the academic thoughts and clinical experience of clinical treatment of intractable headache guided by the classical theory of traditional Chinese medicine. It is hoped to provide some new ideas and methods for the treatment of intractable headache by TCM. This article is divided into two parts: academic thought and experience summary and literature review. The day after tomorrow has the influence; the root of the disease is the essence of the vacuity; the combination of pathogenic factors is the key of headache stubbornness; the evil is deep in the inside, the stubborn formation of the headache is the root of the headache; the external cause leads to the root of the headache, the headache is easy to repeat; the adjustment in peacetime, it is important to insist; Blood separation drugs throughout the treatment; because of disease snobbery, evil does not hurt the right; think that the incidence of intractable headache and the five viscera inseparable; stubborn headache divided into liver-qi stagnation, Yin cold stagnation, fire heat accumulation knot, blood stasis, phlegm stagnation, phlegm stagnation, Liver and kidney yin deficiency, spleen and kidney yang deficiency, deficiency of both qi and blood syndrome, the treatment of flexible differentiation of the use of liver-dispersing, warm cold scattered stagnation, dispelling blood stasis, phlegm stagnation, nourishing the liver and kidney, warming the spleen and strengthening the kidney, reinforcing diarrhea and so on, and pay attention to drug penetration, powder, induced, From deep to shallow, from inside to outside, it brings good news to many headache patients. In the part of literature review, the research progress of intractable headache in traditional Chinese and western medicine is systematically summarized.
