【摘要】：目的:觀察評價疏肝降逆法治療肝郁型高泌乳素血癥的臨床療效,并初步探討其作用機制,為本病尋求一種有效、安全、經(jīng)濟簡單的治療途徑。方法:將2015年3月至2016年12月就診于黑龍江中醫(yī)藥大學(xué)附屬第二醫(yī)院婦科門診,且符合納入標(biāo)準(zhǔn)的61例肝郁型高泌乳素血癥的患者隨機分為治療組(口服丹梔逍遙散加減)31例,對照組(口服溴隱亭)30例。兩組患者均以一個月為一療程,連續(xù)治療3個療程,觀察兩組患者用藥前后臨床癥狀、中醫(yī)證候改善情況,血清內(nèi)分泌激素(PRL、FSH、LH、E2、P)變化,副反應(yīng),復(fù)發(fā)率。結(jié)果:兩組患者臨床總有效率比較,治療組80.65%,對照組83.33%,經(jīng)統(tǒng)計學(xué)分析,兩者并無顯著性差異(P0.05);兩組患者中醫(yī)證候療效比較,治療組痊愈10例,顯效15例,有效4例,無效2例,對照組痊愈4例,顯效7例,有效11例,無效8例,治療組明顯優(yōu)于對照組,且差異性顯著(P0.05);治療組在經(jīng)行腹脹痛、乳房脹痛、口干口苦等癥狀方面的改善,顯著優(yōu)于對照組(P0.01);兩組患者月經(jīng)周期情況治療前后組內(nèi)比較有顯著性差異(P0.05);兩組患者血清PRL治療前后組內(nèi)比較有非常顯著性差異(P0.01),組間比較無顯著性差異(P0.05);兩組患者FSH、LH治療前后組內(nèi)比較無顯著性差異(P0.05),E2、P治療前后組內(nèi)比較有顯著性差異(P0.05);在副反應(yīng)方面,對照組有7人出現(xiàn)惡心、頭暈等不良反應(yīng),治療組未出現(xiàn),組間比較有顯著性差異(P0.05);在副反應(yīng)方面,治療組復(fù)發(fā)率低于對照組。結(jié)論:疏肝降逆法治療肝郁型高泌乳素血癥有較好的臨床療效,能夠有效的降低患者血清PRL水平,改善患者臨床癥狀,且用藥安全,遠期效果好,是臨床治療肝郁型高泌乳素血癥的有效途徑。
[Abstract]:Objective: to observe and evaluate the clinical efficacy of the method of soothing the liver and lowering the inverse in the treatment of hyperprolactinemia of liver depression type, and to explore the mechanism of its action so as to seek an effective and safe method for this disease. Methods: from March 2015 to December 2016, we visited the gynecological outpatient department of the second affiliated Hospital of Heilongjiang University of traditional Chinese Medicine. In addition, 61 patients with hyperprolactinemia of liver depression type were randomly divided into treatment group (31 cases treated by oral administration of Danzhi Xiaoyao Powder plus or subtraction of Dan Zhi Xiaoyao San). Control group (oral bromocriptine 30 cases. Two groups of patients with one month as a course of treatment for 3 consecutive courses of treatment, observe the two groups before and after the treatment of clinical symptoms, TCM syndromes improvement. Results: the total effective rate of the two groups was 80.65% compared with that of the two groups. In the control group, there was no significant difference between the two groups (P 0.05). There were 10 cases of cure, 15 cases of remarkable effect, 4 cases of effective, 2 cases of ineffective in treatment group, 4 cases of cure in control group, 7 cases of remarkable effect, 11 cases of effective and 8 cases of failure. The treatment group was obviously superior to the control group, and the difference was significant (P 0.05). The improvement of symptoms such as abdominal distending pain, breast distending pain and mouth dryness in the treatment group was significantly better than that in the control group (P 0.01). There was significant difference in menstrual cycle between the two groups before and after treatment. There was a very significant difference in serum PRL between the two groups before and after treatment (P 0.01), but there was no significant difference between the two groups (P 0.05). There was no significant difference between the two groups before and after FSHLH treatment. In the side effects, there were 7 cases of nausea, dizziness and other adverse reactions in the control group, but not in the treatment group, there was significant difference between the two groups (P 0.05). In the side effects, the recurrence rate of the treatment group is lower than that of the control group. Conclusion: the treatment of liver depression hyperprolactinemia has a good clinical efficacy, can effectively reduce the serum PRL level of patients. It is an effective way to treat hyperprolactinemia of liver depression type by improving clinical symptoms, safety and long-term effect.
