【摘要】：目的:觀察評價清金化痰湯治療肺癌合并輕度肺部感染的臨床療效,試圖為肺癌合并輕度肺部感染尋求安全有效、經(jīng)濟(jì)方便的綜合療法,改善患者生活質(zhì)量。方法:采用隨機(jī)、對照原則,以非小細(xì)胞肺癌患者54例作為研究對象,通過觀察中醫(yī)癥候積分以及KPS評分,WBC、NEU%、PCT、CRP、ESR等指標(biāo)的變化,評價清金化痰湯治療肺癌合并輕度肺部感染的療效,為其推廣應(yīng)用提供臨床依據(jù)。結(jié)果:1.兩組對肺部感染的總療效:治療組的總有效率為96.2%,對照組的有效率為81.6%,兩組治療方案在療效方面均有效,治療組痊愈率為48.1%,無效為3.8%,對照組痊愈率為3.8%,無效率為18.4%,經(jīng)秩和檢驗,2=-3.869,P=0,P0.05,故兩組在療效方面的差異有統(tǒng)計學(xué)意義,治療組肺部感染的療效優(yōu)于對照組。2.兩組對痰熱郁肺證患者中醫(yī)癥狀都有改善作用:(1)清金化痰湯組中醫(yī)癥候療效改善情況為,痊愈率為33.3%,無效為0,有效率為100%,對照組有效率為85.2%,痊愈率為0,兩組療效經(jīng)秩和檢驗,差異具有統(tǒng)計學(xué)意義,治療組中醫(yī)癥候的改善優(yōu)于對照組。(2)經(jīng)治療后兩組均能改善患者的痰量、痰色、痰質(zhì)、氣喘、口干、發(fā)熱、舌脈。經(jīng)組間比較,在改善各個中醫(yī)癥狀方面,治療組優(yōu)于對照組,(p0.05),具有統(tǒng)計學(xué)意義。在癥狀總積分改善上,治療組優(yōu)于對照組,(p0.05),具有統(tǒng)計學(xué)意義。3.兩組均能降低白細(xì)胞、中性粒百分比,降鈣素原、C反應(yīng)蛋白、血沉等感染指標(biāo),在降低感染指標(biāo)方面,清金化痰湯組降低幅度更大,(p0.05),具有統(tǒng)計學(xué)意義。4.兩組均能提高肺癌合并輕度肺部感染患者的KPS評分(p0.05),具有統(tǒng)計學(xué)意義;兩組組間比較差異無統(tǒng)計學(xué)意義(p0.05)。5.在治療過程中,兩組均無不良事件發(fā)生。結(jié)論:清金化痰湯能改善肺癌合并輕度肺部感染患者咳嗽咳痰、氣喘、口干、發(fā)熱等癥狀,減少患者肺部Up音,改善舌脈,降低其感染指標(biāo),提高KPS評分,具有明顯療效,且治療過程中無不良事件發(fā)生,故清金化痰湯治療肺癌合并輕度肺部感染有效且安全,早期應(yīng)用可防止肺部感染進(jìn)一步進(jìn)展,從而對肺癌合并肺部感染的治療也是有利的。
[Abstract]:Objective: to observe and evaluate the clinical effect of Qingjin Huatan decoction (QJHuatang) in the treatment of lung cancer with mild pulmonary infection, and to seek safe, effective, economical and convenient comprehensive therapy for lung cancer complicated with mild pulmonary infection. Methods: 54 patients with non-small cell lung cancer (NSCLC) were studied on the basis of randomization and control principle. The TCM symptom score and KPS score were observed. To evaluate the efficacy of Qingjin Huatan decoction in the treatment of lung cancer with mild pulmonary infection. Results the total curative effect of the two groups on pulmonary infection: the total effective rate of the treatment group was 96. 2 and the effective rate of the control group was 81.6%. The cure rate was 48.1 in the treatment group, 3.8in the invalid group, 3.8 in the control group, and 18.4in the control group. The difference between the two groups in the curative effect was statistically significant. The therapeutic effect of pulmonary infection in the treatment group is better than that in the control group. 2. The two groups have improved the symptoms of traditional Chinese medicine in the patients with phlegm-heat stagnation lung syndrome. The curative effect of Qingjin Huatan decoction group is improved. The cure rate is 33.3%. The ineffective rate was 0, the effective rate was 100 and the control group was 85.2 percent and the cure rate was 0. The difference between the two groups was statistically significant by rank sum test. The improvement of TCM symptoms in the treatment group was better than that in the control group.) after treatment, both groups could improve the amount of phlegm, the color of phlegm, the quality of phlegm, asthma, dry mouth, fever and tongue pulse. In improving the symptoms of traditional Chinese medicine, the treatment group is better than the control group (P 0.05), with statistical significance. In the improvement of the total symptom score, the treatment group is better than the control group (P 0.05). Both groups can reduce leukocyte, neutrophil percentage, calcitonin proto-C-reactive protein, erythrocyte sedimentation rate and other infection indicators, in terms of reducing the infection index, Qingjinhuatan decoction group decreased more. The KPS score of lung cancer patients with mild pulmonary infection was significantly higher than that of control group (P 0.05). There was no significant difference between the two groups. There were no adverse events in the two groups. Conclusion: Qingjin Huatan decoction can improve cough and expectoration in patients with lung cancer and mild pulmonary infection. Asthma, dry mouth, fever and other symptoms, reduce the patients' lung up sound, improve the tongue vein, reduce its infection index, improve the KPS score, have obvious curative effect, and there are no adverse events in the course of treatment. So Qingjin Huatan decoction is effective and safe in the treatment of lung cancer with mild pulmonary infection, and early application can prevent further progress of lung infection, which is also beneficial to the treatment of lung cancer with pulmonary infection.
