【摘要】：目的:通過分析清熱化濕和解少陽(yáng)法對(duì)熱痹型成人Still病的臨床效果,評(píng)價(jià)清熱化濕和解少陽(yáng)法在熱痹型成人Still病中的有效性,為醫(yī)治成人Still病提供新的中醫(yī)治療思維。方法:將30例符合本課題研究方案的熱痹型納入者,隨機(jī)分為治療組15例,口服中藥自擬方150ml,日兩次口服;聯(lián)合洛索洛芬鈉片60mg,日三次口服;來氟米特20mg,日一次口服。對(duì)照組15例,予洛索洛芬鈉片60mg,日三次口服;來氟米特20mg,日一次口服。兩組周期均是12周,并對(duì)其進(jìn)行臨床觀察。治療前,通過計(jì)算兩組受試者的性別、年齡、病程等基本信息,無統(tǒng)計(jì)學(xué)意義,具有可比性。通過軟件分析治療前后兩組患者中醫(yī)證候積分及生化指標(biāo)WBC、ESR、CPR、SF等變化,進(jìn)而評(píng)判兩種方案的臨床有效率。結(jié)果:1.主要中醫(yī)癥狀比較:治療組在改善患者發(fā)熱、皮疹、咽痛、口渴、關(guān)節(jié)疼痛、關(guān)節(jié)腫脹、關(guān)節(jié)發(fā)熱及肌肉酸軟等方面優(yōu)于對(duì)照組,差異具有統(tǒng)計(jì)學(xué)意義(p0.05)。2.生化指標(biāo)比較:治療組在降低WBC、ESR、CPR、SF等方面結(jié)果顯示,治療組優(yōu)于對(duì)照組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。3.兩組總有效率比較:治療組總有效率為93.33%;對(duì)照組總有效率為60.00%。治療組優(yōu)于對(duì)照組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:1.試驗(yàn)結(jié)果表明,通過清熱化濕和解少陽(yáng)法,以法立方明顯改善患者發(fā)熱、皮疹、咽痛、口渴、關(guān)節(jié)疼痛、關(guān)節(jié)腫脹、關(guān)節(jié)發(fā)熱及肌肉酸軟等中醫(yī)癥狀,同時(shí)臨床療效確切。2.試驗(yàn)結(jié)果表明,通過清熱化濕和解少陽(yáng)法,以法立方也能明顯降低患者的白細(xì)胞計(jì)數(shù)、血沉、C反應(yīng)蛋白、血清鐵蛋白水平等生化指標(biāo)。
[Abstract]:Objective: to evaluate the effectiveness of the method of clearing away heat and removing dampness and reducing yang in the treatment of Still disease of adults with heat arthralgia by analyzing the clinical effect of the method of clearing away heat and removing dampness and reducing yang. In order to treat adult Still's disease, 30 cases of heat arthralgia were randomly divided into treatment group (n = 15) and oral Chinese medicine prescription (150 ml). Twice a day; In combination with losuprofen sodium tablets 60 mg, oral three times a day; Leflunomide 20 mg, once a day, control group 15 cases, Losoprofen sodium tablets 60 mg, oral three times a day; Leflunomide 20mg, once a day. The two groups of cycle is 12 weeks, and clinical observation. Before treatment, by calculating the sex, age, course of disease and other basic information, there is no statistical significance. Through the software analysis of the two groups of patients before and after treatment of TCM syndromes integral and biochemical indicators such as the changes of ESRP CPRSF and so on. Results: 1. Comparison of main TCM symptoms: the treatment group improved fever, rash, pharynx, thirst, joint pain, joint swelling. The joint fever and muscle weakness were better than the control group, the difference was statistically significant (P 0.05). Comparison of biochemical indexes: the treatment group in the reduction of WBCU ESR-CPR. The results of SF and other aspects showed that the treatment group was superior to the control group, the difference was statistically significant (P 0.05) .3.The total effective rate of the two groups was 93.33; The total effective rate of the control group was 60.00.The treatment group was superior to the control group, the difference was statistically significant (P 0.05). The symptoms of fever, rash, pharynx, thirst, joint pain, joint swelling, joint fever and muscle weakness were obviously improved by French cube. By the method of clearing away heat and removing dampness and eliminating yang, the white blood cell count, erythrocyte sedimentation rate C reactive protein, serum ferritin level and other biochemical indexes were also significantly decreased.
