【摘要】：目的：本文旨在通過對周惠芳教授的完整門診病案的回顧性數(shù)據(jù)挖掘,總結(jié)出導師在治療臨床常見婦科疾病時遣方用藥上的普遍規(guī)律,并重點突出地探討了導師治療黃體功能不全性不孕癥的獨到經(jīng)驗,以期為中醫(yī)藥治療該病提供新思路、新方法。方法：文獻綜述分為兩部分,第一部分論述了西醫(yī)學對黃體功能不全性不孕癥的認識概況,包括該病的病理生理、發(fā)病機制、診斷標準及其主要的治療方式；第二部分探討了中醫(yī)學對黃體功能不全性不孕癥病因病機的認識以及該病的辨治概況。經(jīng)驗總結(jié)部分筆者通過侍診學習；周惠芳教授親自講授；收集、分析病案,歸納經(jīng)驗；參考周惠芳教授所撰寫文章等方法,以收集、整理、錄入、分析病案資料為基礎(chǔ),結(jié)合傳統(tǒng)中醫(yī)經(jīng)典進行分析探討,對周惠芳教授治療婦科常見病的高頻藥物,常用方劑進行歸納總結(jié),并重點對其辨治黃體功能不全性不孕癥的臨證經(jīng)驗進行總結(jié)和探討。結(jié)果：①周惠芳教授臨床上最常使用的高頻藥物：淮山藥、菟絲子、牡丹皮、續(xù)斷、炒扁豆、山萸肉、煨木香、熟地黃、炒白芍、云茯苓等；②周惠芳教授治療臨床婦科疾病,最常選用的方劑：助孕湯、龜膠地黃湯、補腎促排卵湯、加減歸芍地黃湯等；③周惠芳教授認為黃體功能不全性不孕癥的核心病機是腎陽偏虛,夾有肝郁；④周惠芳教授治療黃體功能不全性不孕癥,有其獨到見解,具體特點總結(jié)如下：補腎調(diào)周,分期論治；辨證論治,專方專用；衷中參西,各取所長；調(diào)暢情志,規(guī)律生活。結(jié)論：周惠芳教授治療婦科疾病以補腎調(diào)周為根本大法,多選用補益腎陰腎陽、活血養(yǎng)血之品。周惠芳教授認為腎陽偏虛、夾有肝郁乃黃體功能不全性不孕癥的主要病機,治療上當以補腎調(diào)周為本,辯證論治、運用專方助孕合劑,酌情結(jié)合西醫(yī)、西藥,同時注重情志疏導,方能收獲良效。
[Abstract]:Objective: the purpose of this paper is to summarize the general rule of prescribing drugs in the treatment of common gynecological diseases by using retrospective data mining of the complete outpatient medical records of Professor Zhou Huifang. The unique experience of tutor in treating luteal insufficiency infertility was discussed in order to provide new ideas and methods for the treatment of this disease. Methods: literature review was divided into two parts. The first part discusses the understanding of luteal insufficiency infertility in western medicine, including pathophysiology, pathogenesis, diagnostic criteria and main treatment methods. The second part discusses the understanding of the etiology and pathogenesis of luteal insufficiency infertility and the general situation of differentiation and treatment of the disease in traditional Chinese medicine. Professor Zhou Huifang taught in person; Collect, analyze medical records, sum up experience; Referring to the articles written by Professor Zhou Huifang, based on collecting, collating, inputting and analyzing the medical records, and combining with the classic of traditional Chinese medicine, the high frequency drugs used by Professor Zhou Huifang for the treatment of gynecological diseases are discussed. The common prescriptions were summarized and the clinical experience of differentiation and treatment of luteal insufficiency infertility was summarized and discussed. Results Professor Huifang Zhou Huifang was the most frequently used high frequency medicine in clinical practice: Huai yam. Dodder, peony skin, Dipsacus, fried lentils, cornwood meat, simmering wood, cooked Rehmannia, fried white peony, cloud Poria cocos and so on; 2Professor Zhou Huifang's treatment of clinical gynecological diseases, the most commonly used prescriptions: the decoction of assisting pregnancy, the decoction of Guijiao Dihuang, the decoction of tonifying the Kidney and promoting ovulation, the decoction of adding and subtracting the decoction of Radix Paeoniae Dihuang and so on; 3Professor Zhou Huifang thought that the core pathogenesis of luteal insufficiency infertility was deficiency of kidney yang and liver depression. 4Professor Zhou Huifang in the treatment of luteal insufficiency infertility, has its own unique views, specific characteristics summarized as follows: tonifying the kidney and regulating week, treatment by stages; Treatment based on syndrome differentiation, special prescription; Zhongshen West, each to take advantage of; Conclusion: professor Zhou Huifang's treatment of gynecological diseases is based on tonifying the kidney and regulating the week, and more choose the product of tonifying kidney yin and kidney yang, promoting blood circulation and nourishing blood. Professor Zhou Huifang thinks that kidney yang is partial deficiency. There is liver depression is the main pathogenesis of luteal insufficiency infertility, treatment should be based on tonifying the kidney and regulating weeks, dialectical treatment, the use of special prescription to help pregnancy mixture, appropriate combination of Western medicine, Western medicine, at the same time pay attention to emotional dredging. We can reap good results.
