基于正交設計法優(yōu)化穴位貼敷防治支氣管哮喘方案的研究

【摘要】:目的:研究過敏史、貼敷頻次、貼敷時間對穴位貼敷防治支氣管哮喘療效影響的主次因素,并優(yōu)化其治療方案。方法:收集80例確診為支氣管哮喘的患者作為研究對象進行正交設計試驗研究。藥物選用我科歷年的經(jīng)驗貼敷配方(白芥子、延胡索、生甘遂、細辛、麻黃、葶藶子、丁香、肉桂、牙皂按照2∶2∶1∶1∶1∶1∶1∶1∶1比例配成)。貼敷穴位取雙側(cè)肺俞、心俞、膈俞、腎俞。先按照有無過敏史分為兩大組(有過敏史40例,無過敏史40例),再將兩大組分別隨機各分為4組,每組10例。通過研究三因素兩水平,即有無過敏史(因素A:A_Ⅰ:有過敏史、A_Ⅱ:無過敏史)、不同貼敷頻次(因素B:B_Ⅰ:4次、B_Ⅱ:10次,即貼敷4次的組每10天貼1次,貼敷10次的組每4天貼敷1次)、不同貼敷時間(因素C:C_Ⅰ:4 h、C_Ⅱ:8 h)對各組患者治療前后同期哮喘發(fā)作次數(shù)和治療前與治療結(jié)束后6個月支氣管哮喘生存質(zhì)量量表(AQLQ)評分的差異篩選最優(yōu)方案。結(jié)果:(1)正交試驗表明,最優(yōu)方案為A_ⅠB_ⅡC_Ⅰ,即有過敏史的患者,貼敷10次,每次保留4 h。(2)因素B(貼敷頻次)、C(貼敷時間)為支氣管哮喘AQLQ評分的顯著影響因素(均P0.05)。(3)所有患者治療前后哮喘發(fā)作次數(shù)、AQLQ評分的比較表明不同因素水平的組合對哮喘發(fā)作次數(shù)和AQLQ評分的影響不同,除了1組、5組之外,其他各組均有顯著改善(P0.05,P0.01)。結(jié)論:穴位貼敷治療哮喘可明顯降低患者的發(fā)作次數(shù),改善患者的生活質(zhì)量;過敏史、貼敷頻次、貼敷時間對療效影響的主次關系為:貼敷頻次貼敷時間過敏史;最優(yōu)組合方案為:A_ⅠB_ⅡC_Ⅰ,即有過敏史的患者,貼敷10次,每次保留4 h。
[Abstract]:Objective: to study the influence of allergy history, frequency and time on the curative effect of acupoint application on bronchial asthma. Methods: 80 patients with bronchial asthma were selected as subjects for orthogonal design study. The medicine was applied to our department with the prescription of "White Mustard" and "Yanhusuo" in the past years.
