【摘要】：目的:探討通陽益氣化痰逐瘀法聯(lián)合吳茱萸穴位貼敷治療早期2型糖尿病下肢血管病變(diabetic lower extremity vascular disease,DLEVD)的有效性及安全性,為中醫(yī)藥在2型糖尿病下肢血管病變早期防治領(lǐng)域提供新的思路。方法:將60例早期LDEVD患者隨機分為治療組和對照組。治療組給予導師依據(jù)通陽益氣、化痰逐瘀法自擬的中藥湯劑“通脈化痰湯”聯(lián)合吳茱萸粉穴位貼敷(涌泉穴);對照組予常規(guī)西藥治療。3個月為1個療程,采集患者治療前后癥狀積分、間歇性跛行的距離積分、空腹及餐后2h血糖水平、糖化血紅蛋白、下肢動脈血管彩超、ABI、血脂情況,并采用SPSS17.0進行統(tǒng)計學分析。結(jié)果:兩組治療后中醫(yī)癥候積分、跛行距離積分、血漿纖維蛋白原、血脂、下肢動脈彩色多普勒超聲積分較治療前均明顯改善(P0.05),除血脂兩組比較無統(tǒng)計學差異外(P0.05),其他觀察指標比較治療組均優(yōu)于對照組(P0.05),觀察過程中未見明顯不良反應(yīng)。結(jié)論:通陽益氣化痰逐瘀法聯(lián)合吳茱萸穴位貼敷可顯著改善DLEVD早期臨床癥狀體征及血脂水平、下肢動脈硬化及血流情況,減低血液粘稠度,縮小下肢動脈斑塊,安全性較高,值得進一步研究推廣。
[Abstract]:Objective: to explore the treatment of early type 2 diabetes mellitus (T2DM) with Tongyang Yiqi Huatan Zhuayu method combined with Evodia rutaecarpa acupoint application in the treatment of lower extremity vascular disease. Diabetic lower extremity vascular disease. The effectiveness and security of DLEVD. To provide a new idea for the early prevention and treatment of lower extremity vascular diseases in type 2 diabetes mellitus. Methods:. 60 cases of early LDEVD patients were randomly divided into treatment group and control group. The traditional Chinese medicine decoction "Tongmai Huatan decoction" combined with Evodia rutaecarpa powder acupoint application (Yongquan acupoint) was prepared by removing phlegm and removing blood stasis. The control group was treated with routine western medicine. Three months was a course of treatment. The symptom score, distance score of intermittent claudication, fasting and 2 h postprandial blood glucose level and glycosylated hemoglobin were collected before and after treatment. Results: the TCM symptom score, claudication distance integral, plasma fibrinogen, blood lipids were analyzed by SPSS17.0. Lower extremity artery color Doppler ultrasound scores were significantly improved than before treatment P0.05, except the two groups there was no significant difference in blood lipid (P0.05). The other observation indexes were better in the treatment group than in the control group (P0.05). Conclusion: the combination of Tongyang Yiqi and phlegm removing blood stasis method combined with Evodia rutaecarpa acupoint application can significantly improve the early clinical symptoms and signs of DLEVD and the level of blood lipids. The lower extremity arteriosclerosis and blood flow can reduce the blood viscosity and reduce the arterial plaque of lower extremity, which is worthy of further study and popularization.
