【摘要】：目的:觀察腕踝針聯(lián)合靜脈自控鎮(zhèn)痛泵(patient controlled intravenous analgesia,PCIA)和單純PCIA治療異位妊娠腹腔鏡術(shù)后疼痛的臨床療效差異。方法:將符合該試驗(yàn)標(biāo)準(zhǔn)的98例患者按照隨機(jī)數(shù)字表法分為觀察組和對照組,每組49例。兩組均采用了全身靜吸復(fù)合麻醉,對照組僅采用PCIA術(shù)后鎮(zhèn)痛,觀察組采用腕踝針雙側(cè)踝部1、2區(qū)聯(lián)合PCIA治療。分別于手術(shù)后1 h、2 h、6 h、12 h、24 h、36 h、48 h采用視覺模擬量表(visual analogue scale,VAS)對兩組患者切口疼痛情況進(jìn)行評分,并比較兩組鎮(zhèn)痛總有效率及術(shù)后48 h內(nèi)不良反應(yīng)的發(fā)生率。結(jié)果:觀察組手術(shù)后6 h、12 h、24 h切口痛VAS評分低于對照組(均P0.01),其余時(shí)間點(diǎn)兩組評分差異均無統(tǒng)計(jì)學(xué)意義(均P0.05)。觀察組總有效率為98.0%(48/49),對照組總有效率為83.7%(41/49),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組不良反應(yīng)發(fā)生率為12.2%(6/49),對照組為69.4%(34/49),差異有統(tǒng)計(jì)學(xué)意義(P0.01)。結(jié)論:腕踝針配合PCIA能有效緩解腹腔鏡探查術(shù)后疼痛,降低術(shù)后不良反應(yīng)發(fā)生率。
[Abstract]:Objective: to observe the patient controlled intravenous analgesia with carpal ankle acupuncture combined with patient-controlled intravenous analgesia pump. Methods: 98 patients with ectopic pregnancy were divided into observation group and control group according to random digital table method. 49 cases in each group were treated with general intravenous inhalation combined anesthesia, the control group only received postoperative analgesia with PCIA, and the observation group with wrist ankle acupuncture on both sides of malleolus 1. Area 2 combined with PCIA were treated at 1 h, 2 h, 6 h, 12 h, 24 h and 36 h, respectively. Visual analogue scale (VAS) was used to evaluate the incision pain in both groups at 48 h. The total effective rate of analgesia and the incidence of adverse reactions within 48 hours after operation were compared between the two groups. The VAS score of 24 h incision pain was lower than that of the control group (all P 0.01). There was no significant difference in scores between the two groups at other time points (all P 0.05). The total effective rate of the observation group was 98.0% and the total effective rate of the control group was 83.741 / 49. The incidence of adverse reactions in the observation group was 12. 2 / 49 and that in the control group was 69. 4 / 49). Conclusion: wrist and ankle acupuncture combined with PCIA can effectively relieve postoperative pain and reduce the incidence of adverse reactions after laparoscopic exploration.
