【摘要】:目的:在“腎腦相濟(jì)”理論的指導(dǎo)下,觀察“補(bǔ)腎益精填髓”的電針療法對(duì)“天癸竭”圍絕經(jīng)期抑郁癥的作用,并探討其作用機(jī)制,為臨床治療圍絕經(jīng)期抑郁癥提供指導(dǎo)和實(shí)驗(yàn)依據(jù)。材料與方法:選取17周齡健康雌性SD大鼠120只,隨機(jī)分為正常組、模型組、藥物組、電針組和假手術(shù)組,每組各24只。2只一籠正常飼養(yǎng)。1.行為學(xué)指標(biāo)檢測:各組大鼠在造模開始前先進(jìn)行行為學(xué)指標(biāo)的觀測,行為學(xué)指標(biāo)有曠場實(shí)驗(yàn)中總移動(dòng)距離和曠場中心區(qū)域停留時(shí)間;糖水偏好實(shí)驗(yàn)中糖水消耗量、總液體消耗量和糖水消耗率;強(qiáng)迫游泳實(shí)驗(yàn)中大鼠在水中靜止時(shí)間。2.制備圍絕經(jīng)期抑郁癥模型:正常組大鼠不接受造模,正常飼養(yǎng)。模型組、藥物組和電針組大鼠摘除雙側(cè)卵巢去勢,假手術(shù)組不切除卵巢,只在卵巢附近切除與卵巢等大的脂肪。手術(shù)后大鼠恢復(fù)兩周,之后進(jìn)行連續(xù)5天的陰道涂片,確定手術(shù)組大鼠無動(dòng)情周期。認(rèn)為切除卵巢去勢成功。之后正常組大鼠正常飼養(yǎng)不接受刺激,模型組、藥物組、電針組和假手術(shù)組的大鼠均接受為期21天的慢性不可預(yù)見溫和刺激(CUMS)。刺激包括:冰水游泳、熱水游泳、禁水禁食、夾尾、束縛限制行動(dòng)、傾斜籠子和晝夜顛倒。每種刺激隨機(jī)出現(xiàn),且每種刺激在21天中出現(xiàn)次數(shù)不少于3次。21天刺激結(jié)束后,通過行為學(xué)指標(biāo)的變化確定大鼠圍絕經(jīng)期抑郁癥模型造模是否成功。3.電針治療:正常組大鼠正常喂養(yǎng),不接受治療。電針組和假手術(shù)組的大鼠接受28天的電針治療,根據(jù)“腎腦相濟(jì)”理論,選取百會(huì)、腎俞、三陰交穴,每日電針治療1次,持續(xù)20分鐘。藥物組大鼠選用三環(huán)類抗抑郁藥物鹽酸氯米帕明作為對(duì)照,用鹽酸氯米帕明灌胃,每日灌胃1次。治療結(jié)束后通過行為學(xué)指標(biāo)的變化評(píng)價(jià)電針對(duì)于圍絕經(jīng)期抑郁癥大鼠的治療效果。4.行為學(xué)指標(biāo)觀察結(jié)束后,麻醉處死所有大鼠取材,大鼠大腦石蠟切片用于觀察大鼠海馬結(jié)構(gòu)和神經(jīng)細(xì)胞形態(tài)變化;分離大鼠海馬組織,用酶聯(lián)免疫吸附法檢測海馬組織中腦源性神經(jīng)營養(yǎng)因子(BDNF)、酪氨酸激酶受體B(Trk B)、c AMP反應(yīng)元件結(jié)合蛋白(CREB)、B細(xì)胞淋巴瘤2(Bcl-2)和Bcl-2相關(guān)X蛋白(Bax)的含量,并用實(shí)時(shí)熒光定量PCR的方法檢測海馬組織內(nèi)BDNF、Trk B、CREB、Bcl-2和Bax的m RNA表達(dá)量。結(jié)果:1.造模開始前各組大鼠在曠場實(shí)驗(yàn)中總移動(dòng)距離和中心區(qū)域停留時(shí)間無差異(p0.05)。糖水實(shí)驗(yàn)中,糖水消耗量、總液體消耗量和糖水消耗率無差異(p0.05);強(qiáng)迫游泳實(shí)驗(yàn)中靜止不動(dòng)時(shí)間無差異(p0.05)。2.雙側(cè)卵巢摘除術(shù)后,除正常組和假手術(shù)組外,各組大鼠陰道涂片均無動(dòng)情周期。3.經(jīng)過21天慢性不可預(yù)見溫和刺激后,曠場實(shí)驗(yàn)結(jié)果:與正常組相比,其余各組大鼠的總移動(dòng)距離各組均小于正常組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。其余各組大鼠曠場中心區(qū)域停留時(shí)間各組均小于正常組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。4.經(jīng)過21天慢性不可預(yù)見溫和刺激后,糖水實(shí)驗(yàn)結(jié)果:與正常組比較,其余各組大鼠糖水消耗率均小于正常組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。與模型組比較,假手術(shù)組大鼠糖水消耗率高于模型組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。5.經(jīng)過21天慢性不可預(yù)見溫和刺激后,強(qiáng)迫游泳實(shí)驗(yàn)結(jié)果:與正常組比較,其余各組大鼠水中靜止時(shí)間均大于正常組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。6.經(jīng)過28天的電針和藥物治療后,曠場實(shí)驗(yàn)結(jié)果:與正常組比較,模型組大鼠的總移動(dòng)距離小于正常組,中心區(qū)域停留時(shí)間小于正常組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。與模型組比較,藥物組、電針組和假手術(shù)組的總移動(dòng)距離大于模型組,曠場中心區(qū)域停留時(shí)間大于模型組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。7.經(jīng)過28天的電針和藥物治療后,糖水實(shí)驗(yàn)結(jié)果:與正常組比較,其余各組大鼠糖水消耗率均少于正常組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。與模型組比較,假手術(shù)組、電針組和藥物組的糖水消耗率高于模型組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。與電針組比較,假手術(shù)組糖水消耗率高于電針組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。8.經(jīng)過28天的電針和藥物治療后,強(qiáng)迫游泳實(shí)驗(yàn)結(jié)果:與正常組比較,模型組大鼠靜止時(shí)間大于正常組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。與模型組比較,藥物組、電針組和假手術(shù)組的靜止時(shí)間小于模型組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。9.經(jīng)過28天的電針和藥物治療后,在光鏡下觀察各組大鼠海馬石蠟切片,正常組大鼠海馬CA3區(qū)錐體細(xì)胞層和齒狀回區(qū)顆粒細(xì)胞層細(xì)胞數(shù)量多、排列整齊,神經(jīng)元結(jié)構(gòu)完整,胞核及胞質(zhì)染色均勻、清晰。尼氏染色可見神經(jīng)元胞質(zhì)內(nèi)含有豐富的尼氏體,呈紫藍(lán)色,神經(jīng)元細(xì)胞軸突樹突形態(tài)結(jié)構(gòu)完整。模型組大鼠海馬CA3區(qū)細(xì)胞數(shù)量少,細(xì)胞間隙增大,排列疏松不齊,甚至出現(xiàn)細(xì)胞層中斷,尼氏染色可見神經(jīng)元細(xì)胞核消失,細(xì)胞質(zhì)萎縮、出現(xiàn)空洞,軸突樹突結(jié)構(gòu)不完整,甚至出現(xiàn)細(xì)胞結(jié)構(gòu)消失。藥物組、電針組和假手術(shù)組的情況介于兩者之間。10.經(jīng)過28天的電針和藥物治療后,海馬組織酶聯(lián)免疫吸附法結(jié)果:與正常組相比,模型組大鼠海馬BDNF蛋白含量明顯降低,差異有統(tǒng)計(jì)學(xué)意義(p0.05);與模型組比較,藥物組、電針組和假手術(shù)組均高于模型組,差異有統(tǒng)計(jì)學(xué)意義(p0.05);與電針組比較,假手術(shù)組的數(shù)值高于正常組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。與正常組相比,模型組大鼠海馬Trk RNA相對(duì)表達(dá)量低于電針組,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。結(jié)論:1.去勢造模結(jié)合慢性不可預(yù)見溫和刺激確立圍絕經(jīng)期抑郁癥大鼠模型。2.基于“腎腦相濟(jì)”理論的電針治療能改善圍絕經(jīng)期大鼠的抑郁癥狀,實(shí)驗(yàn)結(jié)果與臨床吻合,且為臨床提供實(shí)驗(yàn)依據(jù)。3.假手術(shù)組和電針組行為學(xué)指標(biāo)結(jié)果略有不同,說明“天癸竭”在圍絕經(jīng)期抑郁癥的發(fā)病和轉(zhuǎn)歸中有著重要作用。4.圍絕經(jīng)期抑郁癥大鼠“髓海空虛”的生物學(xué)基礎(chǔ)可能是大腦海馬神經(jīng)元細(xì)胞數(shù)量減少和細(xì)胞結(jié)構(gòu)破壞,且與海馬神經(jīng)元細(xì)胞凋亡增加、海馬神經(jīng)發(fā)生降低有關(guān)。5.電針能高提高大鼠海馬內(nèi)BDNF、Trk B和CREB的含量,促進(jìn)海馬神經(jīng)發(fā)生,還能提高Bcl-2和Bax的比例,抑制神經(jīng)元細(xì)胞凋亡,電針“補(bǔ)腎益精填髓”的作用是通過促進(jìn)神經(jīng)發(fā)生和抑制細(xì)胞凋亡實(shí)現(xiàn)的。6.模型組大鼠海馬組織中Trk B m RNA相對(duì)表達(dá)量高但Trk B蛋白含量低,說明圍絕經(jīng)期抑郁癥大鼠體內(nèi)某些因素妨礙了Trk B基因的表達(dá),這可能是慢性不可預(yù)見溫和刺激造成海馬中Trk B和BDNF含量減少的原因。
