【摘要】：目的:咳嗽變異性哮喘(Cough variant asthma,CVA),是哮喘的一種特殊類型,而且是一種呼吸系統(tǒng)疾病的常見病�？人允荂VA的主要或唯一的臨床表現(xiàn),很多人由于咳嗽持續(xù)發(fā)生或者反復發(fā)作,導致咳嗽遷延不愈,最終引發(fā)咳嗽變異性哮喘。近年來咳嗽變異性哮喘的發(fā)病率持續(xù)增高的趨勢。目前現(xiàn)代醫(yī)學主要以抗炎藥、支氣管擴張藥、抗過敏藥物來應用對癥治療,但其療效尚不理想,并且對人體的副作用較多,停藥后容易復發(fā)。西醫(yī)對于本病的治療具有一定的局限性。而中醫(yī)藥的治療根據(jù)審證論治,辨清證候,審明病因病機,采取針對性的治則從根本上治愈并改善患者的整體癥狀,提高生活質(zhì)量。因此運用中醫(yī)藥治療該病日益受到關(guān)注。導師楊道文教授在其二十多年的臨床工作中,對于咳嗽變異性哮喘的認識及治療積累了豐富的經(jīng)驗。"益氣疏風方"為楊道文教授根據(jù)多年的臨床經(jīng)驗所創(chuàng),方藥基本組成為黃芪、白術(shù)、蜂房、蟬蛻、僵蠶、炙麻黃、苦杏仁,本著益氣扶正以治本,疏風祛邪以治標,標本兼治的治療原則。本研究旨在觀察運用益氣疏風方加減治療辨證為氣虛風動證的咳嗽變異性哮喘患者,探討益氣疏風方治療咳嗽變異性哮喘的臨床療效,以期為治療本病提供一條新的診治思路。方法:本論文第一部分通過文獻檢索、整理歸納的方式,總結(jié)古代醫(yī)家以及現(xiàn)代醫(yī)家對咳嗽變異性哮喘的相關(guān)研究,主要包括本病的中醫(yī)病名,病因病機,不同的診治方法和臨床經(jīng)驗四大部分。第二部分是采用自身對照的方法,對20例辨證為氣虛風動證的咳嗽變異性哮喘患者運用"益氣疏風方"進行治療,重點觀察患者的咳嗽、咽癢、惡風、自汗、神疲乏力等癥狀與體征,分別記錄于治療前、治療后評分,應用統(tǒng)計軟件SPSS20.0對評分進行分析,觀察并評估"益氣疏風方"加減對咳嗽變異性哮喘的臨床療效。結(jié)果:運用益氣疏風方治療氣虛風動證的咳嗽變異性哮喘患者,經(jīng)治療前與治療后癥狀評分比較,有顯著統(tǒng)計學意義(P0.01),可以明顯改善患者的臨床癥狀。其中臨床治愈1例,顯效4例,有效13例,無效2例,治療總有效率為90%。結(jié)論:益氣疏風方加減對改善咳嗽變異性哮喘氣虛風動證患者的臨床癥狀及體征療效確切,取得了顯著臨床效果,具有重要的臨床指導意義與使用價值。由此證實益氣疏風法的有效性及適應范圍,并為臨床提供了一條新的診治思路。
[Abstract]:Objective: cough variant asthma (Cough variant, asthma, CVA) is a special type of asthma, and is a common disease of respiratory system disease. The clinical manifestations of cough is mainly or only CVA, a lot of people because of persistent cough or recurrent cough, leading to delayed healing, eventually cause cough variant asthma in recent years. Cough variant asthma incidence trend. Modern medicine mainly anti-inflammatory drugs and bronchodilators, anti allergic drug application to symptomatic treatment, but the effect is not ideal, and more side effects on the human body, easy to relapse after drug withdrawal. Western medicine has limitations the treatment for the disease. The treatment of Chinese medicine according to the trial on the treatment of differentiating syndromes, etiology and pathogenesis of the Ming, take targeted treatment fundamentally cure and improve overall symptoms, improve the quality of life and therefore. The use of Chinese medicine treatment of the disease has been paid more and more attention. Professor Yang Daowen in the more than 20 years of clinical work, for the understanding and treatment of cough variant asthma has accumulated rich experience. "Qi Shufeng party" created by Professor Yang Daowen based on years of clinical experience, the basic recipe consists of Astragalus, Atractylodes, hive chantui, Jiangcan, ephedra, bitter almond, the spirit of Invigorating Qi to cure, dispelling wind expelling the symptoms, the treatment principle of tackling the problem. The purpose of this study is to observe the use of Yiqi Shufeng Decoction for treating the syndrome of qi deficiency and wind srirring cough variant asthma patients, to investigate the effects of Yiqi Shufeng Decoction in the treatment of the clinical curative effect of cough variant asthma, in order to provide a new treatment strategy for the treatment of this disease.
