【摘要】：目的:通過對張?jiān)厣�、著作及所處社會歷史背景、個人背景資料進(jìn)行匯集整理,系統(tǒng)總結(jié)并闡釋張?jiān)嘏K腑辨證理論內(nèi)涵及特色,明晰張?jiān)嘏K腑辨證理論形成、發(fā)展源流,探析張?jiān)貙W(xué)術(shù)思想,尤其是臟腑辨證理論及對后世醫(yī)家學(xué)術(shù)思想形成與臨床遣方用藥的指導(dǎo)意義。方法:第一,結(jié)合史學(xué)研究方法,歷史唯物主義認(rèn)為,物質(zhì)生活的生產(chǎn)方式?jīng)Q定社會生活、政治生活和精神生活的一般過程,社會存在決定社會意識;張?jiān)卦谄渌幍臍v史時代所經(jīng)歷的特殊生存環(huán)境中,不斷發(fā)展、形成了獨(dú)樹一幟的醫(yī)學(xué)理論和臨床經(jīng)驗(yàn),符合時代特征與人民需求。因此,追本溯源,在特定歷史環(huán)境中研究歷史人物及其學(xué)術(shù)思想尤為重要。第二,采用數(shù)據(jù)挖掘的方法探析張?jiān)貙W(xué)術(shù)理論在臨床中的應(yīng)用。第三,結(jié)合實(shí)地調(diào)研與文獻(xiàn)學(xué)的研究方法,全面搜集、考證古今相關(guān)史料、醫(yī)籍,綜合整理,對資料進(jìn)行匯總分類,以探究張?jiān)厣�、醫(yī)學(xué)理論的形成、特點(diǎn)及對后世醫(yī)學(xué)發(fā)展的影響。結(jié)果:第一,張?jiān)貙W(xué)術(shù)思想的形成,與時代背景、地理背景、學(xué)術(shù)淵源及個人品質(zhì)有緊密聯(lián)系。宋金元時期,戰(zhàn)亂頻發(fā)、疾病流行,人民疲于奔命,內(nèi)傷病較多;當(dāng)時醫(yī)學(xué)界抱守殘缺、泥古不化,醫(yī)家不思進(jìn)取;易州地區(qū)藥材資源豐富,為診療起到支撐作用;張?jiān)乇救饲诿闾?shí)、兼容并蓄、追求真理、力主創(chuàng)新;金元時期,醫(yī)學(xué)中興,百家爭鳴,學(xué)術(shù)交流頻繁,流派之間取長補(bǔ)短,有利于張氏學(xué)術(shù)思想的形成�？傊�,張?jiān)啬苋〉萌绱溯x煌的學(xué)術(shù)成就,與其所處的時代背景、地理環(huán)境及其良好的個人品質(zhì)密不可分,值得后人從多方面深入研究和探討。第二,張?jiān)貙W(xué)術(shù)思想對后世影響深遠(yuǎn)。其學(xué)術(shù)思想,首傳李杲和王好古,李杲又傳于羅天益。至明清,私淑張?jiān)�、李杲之學(xué)的有薛立齋、張介賓、李中梓諸家。之后,趙獻(xiàn)可私淑薛立齋,傳趙獻(xiàn)可之學(xué)者,有高鼓峰、呂留良、董廢翁諸家。李中梓之學(xué),先傳沈朗仲,次傳馬元儀。馬元儀再傳尤在涇。張?jiān)貙W(xué)術(shù)之傳承,以臟腑辨證理論為基礎(chǔ),逐步深入發(fā)展,形成了以脾胃學(xué)說和腎命學(xué)說的為核心的理論體系。第三,臟腑辨證是張?jiān)氐闹饕獙W(xué)術(shù)思想,對臨床診治有重要的現(xiàn)實(shí)意義。以臟腑辨證為原則,對皮肌炎的臟腑辨證及傳變規(guī)律和處方用藥規(guī)律進(jìn)行探討,可以將皮肌炎的臨床常見證型總結(jié)為:脾氣虛證、熱毒熾盛證、腎陰虛證、肺氣虛證和肝陰虛證五類主證,其中脾氣虛證所占比例最高(28.82%);進(jìn)而通過分析,篩選出方劑72首,藥物243種,得出了治療皮肌炎的常用藥物、用藥模式、用藥規(guī)則及藥物關(guān)聯(lián)的網(wǎng)絡(luò)化展示圖。結(jié)論:臟腑辨證是張?jiān)貙W(xué)術(shù)思想的核心,張?jiān)刈鳛榕K腑辨證理論的集大成者,確立了理法方藥完備的臟腑辨證論治體系;同時,臟腑用藥是其學(xué)術(shù)思想的一大特色,補(bǔ)充了臟腑辨證體系,對提高臨床療效起到十分重要的作用;張?jiān)貏?chuàng)立易水學(xué)派,經(jīng)代代傳承與發(fā)揮,逐步完善了臟腑辨證理論,并發(fā)展出腎為先天本、脾為后天本的學(xué)說,上述種種對中醫(yī)學(xué)的發(fā)展起到了巨大的作用。
[Abstract]:Objective: Based on Zhang Yuansu's life, works and social historical background, personal background data collection system, summarize and explain the elements of syndrome differentiation theory connotation and characteristics, a clear elements of syndrome differentiation theory, the origin and development of a element of academic thoughts, especially the viscera theory and on the later doctors academic thought formation prescription and clinical significance. Methods: first, combining the research methods of history, historical materialism believes that the mode of production of material life determines the general process of social life, political life and spiritual life, social existence determines social consciousness; development of special living environment, Zhang Yuansu's experiences in the historical era in the form, become an independent school medical theories and clinical experience, with the characteristics of the times and the needs of the people. Therefore, we in the specific historical environment research The historical figures and their academic thought is particularly important. Second, the application of data mining method in clinical academic Zhang elements. Third research methods, combined with field research and literature comprehensive collection, textual research and related historical materials, medical books, a comprehensive summary of the data collation, classification, in order to explore the elements of Zhang's life and the formation of medical theory, characteristics and influence of medical development. Results: first, to form the elements of academic thought, and the historical background and geographical background, is closely related to the academic origin and personal qualities. Song Jin and Yuan Dynasty, frequent wars, epidemics, people's injury was more constantly on the run, the medical community hold; incomplete, rigid, doctors no; easy abundant medicinal resources area, supporting role in diagnosis and treatment; Zhang Yuansu himself diligently steadfast, the pursuit of truth, for fully inclusive and equitable, Jinyuan period of innovation, Medical ZTE, academic communication, School of contention of a hundred schools of thought, between each other, is conducive to the formation of Zhang's academic thoughts. In a word, Zhang elements can achieve such a brilliant academic achievement, with the background of the times, geographical environment and good personal qualities are inseparable from many aspects, worthy of further research and discussion. The second elements, Zhang the academic thought of far-reaching influence on the later generations. His academic thought, the first Chuan Li Gao and Wang Haogu, Li Gao came to Saul Tianyi. The principle of syndrome differentiation, dermatomyositis and viscera transmission rules and prescription drug laws are discussed, the common clinical syndromes of dermatomyositis were summarized as: spleen deficiency syndrome, heat toxin syndrome, kidney yin deficiency, Qi deficiency and liver yin deficiency syndrome five principal syndromes of spleen deficiency syndrome which has the highest proportion (28.82%); and then through the analysis, screening out 72 prescriptions, 243 kinds of drugs, the commonly used drugs, drug treatment of dermatomyositis model, network medication and drug the associated display chart. Conclusion: syndrome differentiation is the core of Zhang Yuansu's academic thought, Zhang Yuansu as a master of syndrome differentiation theory of the viscera prescription on the treatment of complete system; at the same time, the medication is viscera A major feature of his academic thought, added the Zang Fu system, is very important to improve the clinical efficacy; Zhang Yuansu founded Yishui school, with the play from generation to generation, and gradually improve the viscera theory, and the development of the kidney is the congenital and the spleen of the theory, the development of TCM played a huge role.
