【摘要】：目的:通過對李澤光教授治療原發(fā)性干燥綜合征(Primary Sjogren's sy ndrome,pSS)的病案收集、方藥整理以及用藥數(shù)據(jù)分析,總結(jié)出李澤光教授治療原發(fā)性干燥綜合征的用藥規(guī)律及學(xué)術(shù)思想。方法:整理2015年-2017年李澤光教授治療原發(fā)性干燥綜合征門診初診病案300例,將其方藥采用頻數(shù)分析、聚類分析、因子分析等方法進行歸納與分析。結(jié)果:1.頻數(shù)分析結(jié)果:300則處方用藥共197味,總使用頻次為6347次。其中使用頻數(shù)前46味的中藥,頻數(shù)均在30次以上,累積頻率達80.40%,是治療原發(fā)性干燥綜合征的主要藥物,使用頻數(shù)前20味的中藥,頻數(shù)均在100次以上,累計頻率達60.49%,是治療原發(fā)性干燥綜合征的核心藥物;四氣統(tǒng)計中,寒性藥使用頻率最高,微寒性次之,其次為平性和溫性,熱性與大寒性藥物基本沒有出現(xiàn)。五味統(tǒng)計中,甘味藥物出現(xiàn)頻率最高,苦味次之,其次為辛、酸、澀、咸、淡。歸經(jīng)統(tǒng)計中,以歸肝、肺、胃經(jīng)的藥物頻率最高。藥物類別統(tǒng)計中,以清熱藥使用頻率最高,其次分別是補虛藥、活血化瘀藥、祛風(fēng)濕藥、解表藥、利水滲濕藥。2.聚類分析結(jié)果:本研究對頻數(shù)在30次以上,總使用頻率80.40%以上的46味中藥進行聚類分析,得出治療原發(fā)性干燥綜合征的常用藥對8個,3味藥組合5個及多味藥物組合4個,多為導(dǎo)師針對原發(fā)性干燥綜合征口干、眼干而設(shè)的滋陰潤燥、清熱解毒藥對、藥組,它們分別從不同角度體現(xiàn)了導(dǎo)師臨證組方思維特點、組方原則等。3.因子分析結(jié)果:本研究對頻數(shù)在30次以上,總使用頻率80.40%以上的46味中藥進行因子分析,得出公因子14個,從多個角度反映了原發(fā)性干燥綜合征的內(nèi)在病機及導(dǎo)師的治法治則等。結(jié)論:1.生地黃、秦皮、蘆根、白芍、菊花、忍冬藤、金銀花、麥冬、甘草、郁金、茯苓、烏梅、百合、北沙參、雞血藤、合歡皮、延胡索、白術(shù)、威靈仙、陳皮是導(dǎo)師治療原發(fā)性干燥綜合征的核心藥物。2.清熱藥、補虛藥、活血化瘀藥、解表藥、祛風(fēng)濕藥是導(dǎo)師治療原發(fā)性干燥綜合征的基本組成類別。3.酸甘化陰、清熱解毒、活血化瘀是導(dǎo)師治療原發(fā)性干燥綜合征最常用的用藥法則。
[Abstract]:Objective: To summarize professor Li Zeguang's medication rules and academic thoughts in treating primary Sjogren's syndrome by collecting professor Li Zeguang's Primary Sjogren's sy ndrome (pSS), collecting prescription data and analyzing drug data. Methods: in 2015 -2017, 300 cases of primary Sjogren's syndrome in the outpatient clinic were treated in 2015. Results: 1. frequency analysis results: 300 prescriptions were 197 flavors, and the total use frequency was 6347 times. The use of traditional Chinese medicine 46 kinds of frequency, the frequency was more than 30 times, the cumulative frequency of 80.40%, is the main drug for the treatment of primary Sjogren syndrome, the use of traditional Chinese medicine 20 kinds of frequency, frequency of more than 100 times, the cumulative frequency of 60.49%, is the core of drug treatment of primary Sjogren syndrome statistics; four, the highest frequency of use of cold medicine, micro cold times, followed by flat and warm, hot and cold drugs does not appear. Five kinds of statistics, the highest frequency of bitter sweet medicine, the second, followed by spicy, sour and astringent and salty, pale. By statistics, in the liver, lung and stomach medicine highest frequency. In the statistics of drug categories, the use frequency of antipyretic drugs is the highest, followed by supplementing deficiency drugs, activating blood and removing blood stasis drugs, removing wind and wet drugs, solving table drugs, and leothing and wetting drugs. The results of this study: 2. clustering frequency in more than 30 times, more than 80.40% of the total frequency of use of 46 traditional Chinese medicines by cluster analysis, the commonly used drugs for the treatment of primary Sjogren syndrome in 8, 3 drug combinations of 5 drugs and combination of the 4, is designed for primary teachers dry dry dry mouth, dry eye syndrome, Ziyin Runzao, detoxification of drugs and drug group, they were from different angle reflects the characteristics of thinking, the teacher certificate prescription prescription principle etc.. 3. factor analysis results: in this study, we conducted factor analysis for 46 Chinese herbs with frequency above 30 times and 80.40% of total frequency, and got 14 common factors, which reflected the intrinsic pathogenesis of primary Sjogren syndrome and the governors' treatment principles from multiple perspectives. Conclusion: 1. radix rehmanniae, cortex Fraxini, reed rhizome, Radix Paeoniae Alba, chrysanthemum, honeysuckle, honeysuckle, licorice, ophiopogon root, turmeric, Poria, plum, lily, Glehnia root, Caulis Spatholobi, Albizzia julibrissin, Atractylodes, Rhizoma Corydalis, Radix Clematidis, tangerine peel is the core drug mentor treatment of primary Sjogren syndrome. The basic composition of the tutor's treatment of primary Sjogren syndrome is 2. antipyretic drugs, supplementing deficiency drugs, activating blood circulation and removing stasis medicine, resolving the table medicine and removing wind and damp medicine. 3. sour and sweet Yin, detoxification, blood stasis is the mentor treatment of primary Sjogren syndrome is the most commonly used drug law.
