【摘要】：目的:探討肺康寧合劑治療慢性阻塞性肺疾病肺動脈高壓患者臨床療效,并觀察其對內(nèi)皮素-1(ET-1)、D-二聚體的影響。方法:回顧性分析醫(yī)院52例慢性阻塞性肺疾病肺動脈高壓患者臨床診治情況,其中以常規(guī)治療者為對照組(21例);于常規(guī)治療基礎上采用肺康寧合劑治療者為治療組(31例)。檢測兩組患者治療前、后ET-1、D-二聚體及肺動脈壓變化,統(tǒng)計兩組臨床療效及臨床癥狀、體征(咳嗽、喘息、咳痰及哮鳴音)積分變化,觀察治療期間不良反應。結(jié)果:治療后,兩組患者ET-1、D-二聚體及肺動脈壓水平均較治療前顯著下降,治療組各項指標下降程度明顯優(yōu)于對照組,P0.05;兩組患者治療后咳嗽、喘息、咳痰及哮鳴音積分較治療前明顯下降,治療組各項癥狀、體征積分下降程度優(yōu)于對照組,P0.05。治療組治療總有效率90.32%,明顯高于對照組57.14%,P0.05;治療期間未見嚴重性不良反應。結(jié)論:采用肺康寧合劑治療慢性阻塞性肺疾病肺動脈高壓可顯著改善其肺動脈高壓及臨床各項癥狀、體征,提高臨床療效且具有一定安全性,然該藥物作用機制可能與其改善患者血管內(nèi)皮功能及高凝狀態(tài)有一定關系。
[Abstract]:Objective: To investigate the clinical efficacy of Corelle mixture in the treatment of chronic obstructive pulmonary disease with pulmonary hypertension and observe its effect on endothelin -1 (ET-1) and D- two dimer. Methods: the clinical diagnosis and treatment of 52 patients with chronic obstructive pulmonary disease with pulmonary hypertension were retrospectively analyzed. Among them, the conventional treatment group was 21 cases (control group). On the basis of routine treatment, the treatment group was treated with lung Corelle mixture (31 cases). The changes of ET-1 and D- two dimers and pulmonary arterial pressure in two groups before and after treatment were detected. The clinical efficacy and clinical symptoms, signs (cough, wheezing, expectoration and wheezing) integral changes of two groups were statistically analyzed, and adverse reactions during treatment were observed. Results: after treatment, two groups of patients with ET-1, two D- dimer and pulmonary artery pressure levels were significantly lower than before treatment, the index of the treatment group decreased significantly than the control group, P0.05; cough, wheezing, cough and wheeze score was significantly decreased than that before treatment in two groups of patients after treatment, the symptoms of the treatment group and signs integral decreased than the control group, P0.05. The total effective rate of treatment in the treatment group was 90.32%, which was significantly higher than that of the control group (57.14%), P0.05, and no severe adverse reaction was found during the treatment. Conclusion: the lung Corelle mixture in the treatment of chronic obstructive pulmonary disease and pulmonary arterial hypertension can significantly improve the pulmonary arterial pressure and clinical symptoms and signs, improve the clinical curative effect and has certain safety, so the drug mechanism may be related to the improvement of vascular endothelial function in patients with hypercoagulable state and have a certain relationship.
