【摘要】：目的：探索療效好、副作用小、有效提高生活質(zhì)量的橈骨莖突狹窄性腱鞘炎的針灸治療方法。通過針刺肌肉起止點(diǎn)、針刺阿是穴的方法,比較兩者之間的療效,并觀察其遠(yuǎn)期療效,為針刺肌肉起止點(diǎn)治療橈骨莖突狹窄性腱鞘炎提供依據(jù)。方法：于臺(tái)北市士林區(qū)崇生中醫(yī)診所,收集臨床橈骨莖突狹窄性腱鞘炎患者共60例,隨機(jī)分為肌肉起止點(diǎn)組(治療組)30例及阿是穴組(對(duì)照組)30例,兩組均每周治療3次,二周為一療程,共治療1個(gè)療程。1個(gè)療程后觀察兩組患者的臨床癥狀、體征變化,并對(duì)兩組進(jìn)行療效評(píng)定。對(duì)以上數(shù)據(jù)建立數(shù)據(jù)庫,運(yùn)用spss 22.0進(jìn)行統(tǒng)計(jì)分析。結(jié)果：經(jīng)統(tǒng)計(jì)學(xué)分析,治療前針刺肌肉起止點(diǎn)組(治療組)和針刺阿是穴組(對(duì)照組)兩組患者性別、年齡、病程、治療前周發(fā)作天數(shù)及主癥體征評(píng)分均無明顯差異(P0.05),具有可比性。1個(gè)療程后,針刺肌肉起止點(diǎn)組愈顯率為50.00%,總有效率為86.66%,針刺阿是穴組愈顯率為16.77%,總有效率為73.33%,經(jīng)檢驗(yàn)后兩組療效比較具有統(tǒng)計(jì)學(xué)意義(P0.05),說明針刺肌肉起止點(diǎn)組療效優(yōu)于針刺阿是穴組;兩組組內(nèi)治療前后癥狀及體征之評(píng)分結(jié)果比較,有統(tǒng)計(jì)學(xué)意義(P0.05),提示兩種療法治療前后癥狀及體征皆有明顯改善；兩組治療后之癥狀及體征評(píng)分比較,結(jié)果有統(tǒng)計(jì)學(xué)意義(P0.05),提示治療后針刺肌肉起止點(diǎn)組癥狀及體征之評(píng)分較針刺阿是穴組下降顯著。治療停止3個(gè)月后療效評(píng)定,針刺肌肉起止點(diǎn)組組愈顯率為46.66%,總有效率為90.00%,針刺阿是穴組愈顯率為10.34%,總有效率為48.28%,針刺肌肉起止點(diǎn)組總有效率優(yōu)于針刺阿是穴組,經(jīng)統(tǒng)計(jì)學(xué)分析,兩組療效具有統(tǒng)計(jì)學(xué)意義(P0.05),說明治療后3個(gè)月針刺肌肉起止點(diǎn)組之療效優(yōu)于針刺阿是穴組；組內(nèi)治療前及治療后3個(gè)月評(píng)分比較,結(jié)果有統(tǒng)計(jì)學(xué)意義(P0.05),提示治療后3個(gè)月較治療前之主癥及體征皆有改善；組內(nèi)治療后及治療后3個(gè)月評(píng)分比較,結(jié)果有統(tǒng)計(jì)學(xué)意義(P0.05),提示治療后3個(gè)月較治療1個(gè)月后之主癥及體征皆有復(fù)發(fā)情形；治療后3個(gè)月兩組評(píng)分進(jìn)行比較,結(jié)果有統(tǒng)計(jì)學(xué)意義(P0.05),提示治療3個(gè)月后針刺肌肉起止點(diǎn)組療效優(yōu)于針刺阿是穴組。結(jié)論：本課題經(jīng)臨床研究后,橈骨莖突狹窄性腱鞘炎經(jīng)肌肉起止點(diǎn)及阿是穴治療后皆有臨床療效,也都有一定的復(fù)發(fā)率,但肌肉起止點(diǎn)療法近期及遠(yuǎn)期療效皆優(yōu)于阿是穴療法,復(fù)發(fā)率也較低。肌肉起止點(diǎn)治療橈骨莖突狹窄性腱鞘炎值得推廣。
[Abstract]:Objective: To explore the method of acupuncture treatment of stenosing tenosynovitis of the radial styloid process has good curative effect, little side effect, effectively improve the quality of life. Through the method of acupuncture, acupuncture Ashi muscle starting and ending point, compare the effect between the two, and observe the long-term curative effect, provide the basis for acupuncture treatment of muscle enthesis radial styloid stenosing tenosynovitis. Methods: in the Shihlin District of Taipei city Chong students of Chinese medicine clinics, collect clinical de Quervain disease patients with a total of 60 cases were randomly divided into muscle enthesitis group (treatment group) 30 cases and Ashi group (control group) 30 cases, two groups were treated 3 times a week, two weeks for a course of treatment 1 course of treatment. After 1 courses of treatment, the clinical symptoms and signs of the two groups were observed, and the efficacy of the two groups was evaluated. A database is set up for the above data, and SPSS 22 is used for statistical analysis. Results: by statistical analysis, the treatment of acupuncture before the start points of the muscles group (treatment group) and acupuncture point group (control group) two groups of patients with gender, age, course of disease, treatment of the previous week there were no significant difference between the days of onset and main symptom score (P0.05), comparable. After 1 courses of acupuncture, muscle enthesis grouprevealing rate is 50%, the total effective rate was 86.66%, the acupuncture point grouprevealing rate is 16.77%, the total effective rate was 73.33%, after testing the efficacy of the two groups was statistically significant (P0.05), acupuncture muscle starting and ending point group is better than acupuncture Ashi group; in the two groups before and after treatment the score of symptoms and signs compared with statistical significance (P0.05), suggesting that the two therapy before and after treatment of symptoms and signs improved evidently; compared the symptoms and signs of the two groups after treatment the score, the results were statistically significant (P0.05), suggesting that after treatment the scores of acupuncture muscle starting and ending point the symptoms and signs of a group of acupuncture point group decreased significantly. Stop the treatment efficacy evaluation after 3 months, the starting and ending points of muscular acupuncture group group markedly effective rate was 46.66%, the total effective rate was 90%, the acupuncture point grouprevealing rate is 10.34%, the total effective rate was 48.28%, the starting and ending points of muscular acupuncture group total effectiveness is better than that of Acupuncture point group, by statistical analysis, with statistical significance the efficacy of the two groups (P0.05), that 3 months after the treatment of acupuncture point treatment group muscle starting and ending acupuncture Ashi group treatment group; 3 months score before and after treatment. The results were statistically significant (P0.05), suggesting that before 3 months after treatment compared with the main symptoms and signs are improved 3 months after the score; and in the treatment group after treatment, the results were statistically significant (P0.05), suggesting that after 3 month treatment compared with the treatment of the primary disease after 1 months and have signs of recurrence 3 months; compare the scores of the two groups after treatment, the results were statistically significant (P0.05), prompt After 3 months of treatment with acupuncture muscle starting and ending point group is better than acupuncture point group. Conclusion: the clinical study, radial styloid stenosing tenosynovitis by muscle starting and ending points and Ashi points after treatment has clinical curative effect, also have a certain recurrence rate, but the muscle from the short-term and long-term effect of therapy is better than acupuncture, and low recurrence rate. Muscle enthesis treatment of radial styloid stenosing tenosynovitis is worthy of promotion.
