犬沖燒復(fù)合傷合并海水浸泡的血流動(dòng)力學(xué)研究

【摘要】:目的 建立犬沖擊傷復(fù)合燒傷合并海水浸泡的實(shí)驗(yàn)?zāi)P?,觀察實(shí)驗(yàn)犬血流動(dòng)力學(xué)特點(diǎn)并探討相關(guān)機(jī)制。方法 將肺部輕度沖擊傷合并 10 %Ⅱ度體表燒傷犬 17只隨機(jī)分為浸泡組 (n =10 )和沖燒組 (n =7)。浸泡組實(shí)驗(yàn)犬浸泡于海水中 (水溫 2 3℃ ,pH8 0 ,鹽度 2 2~ 2 5 3) 4h后 ,觀察傷前及浸泡出水后 2 4h內(nèi)血流動(dòng)力學(xué)變化 ,抽取靜脈血測(cè)定丙二醛 (MDA) ,進(jìn)行血細(xì)菌培養(yǎng) ,實(shí)驗(yàn)結(jié)束后觀察心、肺病理變化。沖燒組除不浸泡外 ,余處理同浸泡組。結(jié)果 浸泡組實(shí)驗(yàn)犬體溫、心臟指數(shù) (CI)、平均動(dòng)脈壓 (MAP)、心率(HR)較沖燒組明顯下降 ,周圍血管阻力指數(shù) (SVRI)、肺血管阻力指數(shù) (PVRI)明顯升高 ,血漿MDA變化與CI呈負(fù)相關(guān) ,在浸泡出水后6h內(nèi)血流動(dòng)力學(xué)紊亂最明顯。浸泡組傷后血中出現(xiàn)腸道菌的時(shí)間較沖燒組提前。浸泡組的心、肺病理損傷程度重于沖燒組。結(jié)論 海水浸泡明顯加重沖燒復(fù)合傷血流動(dòng)力學(xué)紊亂 ,浸泡后 6h內(nèi)為最明顯時(shí)相段。這可能與浸泡后體溫下降、炎癥反應(yīng)和脂質(zhì)過氧化物反應(yīng)增強(qiáng)有關(guān)。
