【摘要】：目的:探討在抗擊傳染性非典型肺炎的應(yīng)激狀態(tài)下,參戰(zhàn)一線士兵心理衛(wèi)生狀況,并分析不同年齡及不同崗位士兵間癥狀因子評(píng)分的差異特征。方法:于2003-05/06選擇北京解放軍小湯山醫(yī)院男性戰(zhàn)士141人(包括機(jī)關(guān)士兵)為觀察對(duì)象進(jìn)行調(diào)查。采用癥狀自評(píng)量表對(duì)其心理狀態(tài)進(jìn)行調(diào)查。該量表共有90個(gè)項(xiàng)目并分為軀體化、強(qiáng)迫癥狀、人際關(guān)系、抑郁、焦慮、敵對(duì)、恐怖、偏執(zhí)狂、精神病性等9個(gè)癥狀因子分和總癥狀指數(shù),采用1～5級(jí)評(píng)分。評(píng)估標(biāo)準(zhǔn):單項(xiàng)因子分及總癥狀指數(shù)達(dá)到3分以上定為陽(yáng)性,提示可能存在明顯的心理問(wèn)題。在測(cè)試前采用統(tǒng)一的指導(dǎo)語(yǔ),讓士兵根據(jù)自己的實(shí)際情況做出真實(shí)的自我評(píng)定,獨(dú)立完成。心理檢測(cè)由經(jīng)培訓(xùn)的專(zhuān)業(yè)人員進(jìn)行。并與國(guó)內(nèi)青年常模進(jìn)行比較。結(jié)果:所有戰(zhàn)士均完成測(cè)試,全部進(jìn)入結(jié)果分析。①參戰(zhàn)士兵癥狀自評(píng)量表陽(yáng)性癥狀人數(shù)分布:參戰(zhàn)士兵癥狀自評(píng)量表陽(yáng)性癥狀人數(shù)依次為軀體化1人(0.7%)、強(qiáng)迫癥狀13人(9.2%)、人際關(guān)系敏感10人(7.1%)、抑郁7人(5.0%)、焦慮3人(2.1%)、敵對(duì)5人(3.6%)、恐怖9人(6.4%)、偏執(zhí)10人(7.1%)、精神病性5人(3.6%)。其中有一項(xiàng)以上分?jǐn)?shù)達(dá)到3分者共30人,占調(diào)查人數(shù)的21.6%。②參戰(zhàn)士兵癥狀自評(píng)量表平均分最高的因子為強(qiáng)迫癥狀,其次為人際關(guān)系敏感,最低的因子則是精神病性,中間由高到低依次是敵對(duì)、偏執(zhí)、憂郁、焦慮、軀體化和恐懼。除人際關(guān)系敏感因子外,參戰(zhàn)士兵癥狀自評(píng)量表各因子分均高于國(guó)內(nèi)青年組常模,差異有顯著性。③機(jī)關(guān)兵的總癥狀指數(shù)和各因子分與非機(jī)關(guān)兵比較,前者的人際關(guān)系敏感因子分顯著高于后者。④第2年軍齡的戰(zhàn)士總癥狀指數(shù)及人際關(guān)系、抑郁、恐懼、偏執(zhí)等因子分顯著高于第4年軍齡的戰(zhàn)士。第2年軍齡的戰(zhàn)士恐怖因子分也明顯高于第1年軍齡者(t=2.37,P0.05)。結(jié)論:①參戰(zhàn)一線士兵癥狀自評(píng)量表的總分、總均分、陽(yáng)性項(xiàng)目數(shù)、軀體化癥狀、人際關(guān)系敏感、抑郁、焦慮、敵對(duì)、恐怖、偏執(zhí)狂、精神病性等標(biāo)準(zhǔn)分顯著高于常模組,具有較高的心理異常發(fā)生率。②不同崗位,不同年齡士兵的心理健康評(píng)分有差異。
[Abstract]:Objective: to explore the mental health status of soldiers in the first line of war under the stress state of fighting SARS, and to analyze the different characteristics of symptom factor scores among soldiers of different ages and positions. Methods: 141 male soldiers (including government soldiers) in Xiaotangshan Hospital of Beijing PLA were investigated in June 2003. Symptom Checklist-90 (SCL-90) was used to investigate their mental state. There were 90 items in the scale, including somatization, obsessive compulsion, interpersonal relationship, depression, anxiety, hostility, phobia, paranoia, psychosis, and total symptom index. Evaluation criteria: single factor score and total symptom index above 3 points as positive, suggesting that there may be obvious psychological problems. Before the test, the soldiers can make a true self-assessment according to their actual situation and complete it independently. Psychological tests are performed by trained professionals. And compared with the domestic youth norm. Results: all the soldiers completed the test, all entered into the result analysis. 1 the distribution of the positive symptoms of the soldiers participating in the war: the number of the positive symptoms of the soldiers participating in the war was 1 somatization (0.7%). There were 13 patients (9.2%) with obsessive-compulsive symptoms, 10 (7.1%) with interpersonal sensitivity, 7 (5.0%) with depression, 3 (2.1%) with anxiety, 5 (3.6%) with hostility, 9 (6.4%) with terror. Paranoid 10 (7.1%), psychotic 5 (3.6%). Among them, 30 people had a score of 3 or more, accounting for 21.6.2% of the respondents. The highest score of the SCL-90 was obsessive-compulsive symptom, the second was interpersonal sensitivity, and the lowest factor was psychosis, the highest score was obsessive-compulsive symptom, the second was interpersonal sensitivity, and the lowest was psychosis. From high to low are hostility, paranoia, depression, anxiety, somatization, and fear. With the exception of interpersonal sensitivity factors, the scores of all the factors in the SCL-90 were higher than those in the national youth group, and the difference was significant. 3 the total symptom index and the factor scores of the Organ soldiers were compared with those of the non-organ soldiers. The scores of interpersonal sensitivity factors of the former were significantly higher than those of the latter. 4 the scores of the total symptom index, interpersonal relationship, depression, fear and paranoid ideation of the soldiers in the second year of military service were significantly higher than those of the soldiers in the fourth year. The scores of terror factor of the soldiers in the second year were also higher than those in the first year (t = 2.37, P 0.05). Conclusion: 1 the total score, mean score, positive items, somatization symptoms, interpersonal sensitivity, depression, anxiety, hostility, phobia, paranoia and psychosis of the soldiers in the first line of war were significantly higher than those in the normal model group. There was a higher incidence of psychological abnormality. 2 there were differences in mental health scores of soldiers in different positions and different ages.
