高原高寒戰(zhàn)時(shí)環(huán)境肢體火器傷后應(yīng)激反應(yīng)的特點(diǎn)

【摘要】:目的 探討高原高寒環(huán)境下肢體火器傷后全身應(yīng)激反應(yīng)的特點(diǎn)。方法 將2 4頭豬隨機(jī)分為高原戰(zhàn)時(shí)致傷組(GZ)、高原平時(shí)致傷組(GP)和平原平時(shí)致傷組(PP) ,每組8只、復(fù)制肢體火器傷模型。分別于傷前、傷后10、3 0min、2、6、12、2 4、48、72、12 0h采靜脈血測定血漿氫化可的松和腎上腺素。結(jié)果 3組豬血漿氫化可的松含量均于傷后10min明顯升高、3 0min達(dá)高峰,高原戰(zhàn)時(shí)致傷組血漿氫化可的松含量增高幅度最大,約為高原平時(shí)致傷組的2倍、平原平時(shí)致傷組的4倍;高原戰(zhàn)時(shí)致傷組血漿腎上腺素含量于傷后10min明顯升高并達(dá)高峰,約為傷前的40倍、高原平時(shí)致傷組的1 5倍、平原平時(shí)致傷組的1 8倍。平原平時(shí)致傷組血漿氫化可的松和腎上腺素含量均在傷后48h恢復(fù)到正常水平,高原平時(shí)致傷組在傷后72h時(shí)恢復(fù)到正常水平,高原戰(zhàn)時(shí)致傷組在傷前血漿氫化可的松和腎上腺素含量就大大高于其它兩組,一直到傷后12 0h仍然保持在較高水平。結(jié)論 高原高寒環(huán)境戰(zhàn)時(shí)豬肢體槍彈傷后全身應(yīng)激反應(yīng)明顯重于高原平時(shí)致傷組和平原平時(shí)致傷組,而且持續(xù)的時(shí)間長。
[Abstract]:Objective to investigate the characteristics of systemic stress response after firearm injury in high altitude and cold environment. Methods Twenty-four pigs were randomly divided into two groups: (GZ), group (n = 8), (GP) group (n = 8) and plain group (n = 8). Plasma hydrocortisone and epinephrine were measured in venous blood samples for 120 hours before and 10 30 minutes after injury. Results the levels of plasma hydrocortisone in the three groups were significantly higher than those in the post-injury group, and reached the peak at 3 0min. The plasma hydrocortisone content in the high altitude wartime injury group was the highest, which was about twice as high as that in the plateau peacetime injury group. The level of injury in plain was 4 times as high as that in normal time. The plasma epinephrine content in the high altitude wartime injury group was significantly higher than that before the injury and reached the peak, about 40 times as high as before, 1.5 times as much as the plateau peacetime injury group, and 18 times as high as the plain peacetime injury group. The plasma levels of hydrocortisone and epinephrine returned to normal level in plain normal injury group at 48 h after injury, and returned to normal level at 72 h after injury in plateau injury group. The plasma levels of hydrocortisone and epinephrine in the high altitude wartime group were significantly higher than those in the other two groups, and remained at a relatively high level until 120 hours after injury. Conclusion the whole body stress response after gunshot wound of pig limbs in high altitude and cold environment was significantly more serious than that in plateau injury group and plain injury group, and lasted for a long time.
