犬燒沖復(fù)合傷后海水浸泡對酸堿平衡的影響

【摘要】:目的:建立犬燒沖復(fù)合傷后海水浸泡的實驗?zāi)P?探討燒沖復(fù)合傷海水浸泡對犬酸堿平衡的影響,為海水浸泡早期救治提供理論依據(jù)。方法:20條健康成年雜種犬按隨機(jī)數(shù)字表法隨機(jī)分為兩組。對照組:致傷后直接觀察(n=10);浸泡組:致傷后置入21℃海水中浸泡4h(n=10),于傷前,浸泡后即刻(相當(dāng)于對照組傷后4h),浸泡后3h(相當(dāng)于對照組傷后7h),浸泡后6h(相當(dāng)于對照組傷后10h),浸泡后16h(相當(dāng)于對照組傷后20h),浸泡后24h(相當(dāng)于對照組傷后28h)測定血氣、酸堿平衡以及血流動力學(xué)指標(biāo)。結(jié)果:浸泡組出海后PaO2、BE、HCO3-、pH和PaCO2與對照組相比均明顯下降,且BE、pH、HCO3-和PaCO2呈進(jìn)行性下降;而PaO2隨時間延長呈恢復(fù)趨勢。對照組致傷前后無明顯變化。結(jié)論:海水浸泡可導(dǎo)致犬嚴(yán)重的低碳酸血癥和代謝性酸中毒,對體液內(nèi)環(huán)境的失衡有重要影響。
