海軍部隊(duì)高強(qiáng)度軍事訓(xùn)練致腎臟損傷的調(diào)查分析

【摘要】:目的調(diào)查分析海軍部隊(duì)高強(qiáng)度軍事訓(xùn)練后腎臟損傷的發(fā)生情況。方法受試對(duì)象為1 320名進(jìn)行5km武裝越野跑的海軍部隊(duì)官兵,其中新兵568人,老兵752人。訓(xùn)練完成后即刻留取尿液,以干化學(xué)法進(jìn)行蛋白尿及血紅蛋白檢測(cè);尿液離心后行紅細(xì)胞鏡檢;ELISA法檢測(cè)尿N-乙酰-β-D-氨基葡萄糖苷水解酶(NAG)和視黃醇結(jié)合蛋白(RBP)水平;以酶偶聯(lián)法測(cè)定血肌酸激酶(CK)水平,以CK≥950U/L作為橫紋肌溶解癥診斷標(biāo)準(zhǔn)。結(jié)果訓(xùn)練后總體血尿發(fā)生率為2.1%,血紅蛋白尿發(fā)生率為7.4%,蛋白尿陽(yáng)性率為44%,尿NAG異常率為57.1%,尿RBP異常率為57.3%,且新兵均高于老兵(P0.05);各檢測(cè)項(xiàng)目訓(xùn)練結(jié)束24h后80%左右的官兵上述指標(biāo)恢復(fù)正常,訓(xùn)練結(jié)束48h后90%左右恢復(fù)正常;以上指標(biāo)復(fù)合異常(即至少有一項(xiàng)異常)發(fā)生率為78.2%。橫紋肌溶解癥發(fā)生率為0.3%,無(wú)急性腎功能衰竭出現(xiàn)。結(jié)論海軍部隊(duì)高強(qiáng)度軍事訓(xùn)練后腎臟損傷發(fā)生率較高,且新、老兵之間存在明顯差異,極少數(shù)官兵可發(fā)生橫紋肌溶解癥。
