不同復(fù)溫方式對犬腹部開放傷合并海水浸泡救治時血流動力學(xué)的影響

【摘要】:目的 探討不同復(fù)溫方式對犬腹部開放傷合并海水浸泡救治時血流動力學(xué)的影響。方法 成年雜種犬 30只制作成腹部開放傷動物模型 ,傷后均浸泡人工海水中 ,2h后打撈出水 ,隨機(jī)分為 3組。自然復(fù)溫組 (n=10 ) :將動物移至溫暖環(huán)境 ,靠機(jī)體自身產(chǎn)熱自發(fā)復(fù)溫 ;體表復(fù)溫組 (n=10 ) :用電熱毯體表加溫 ;體中心復(fù)溫組 (n=10 ) :1/3張鹽水 4 2℃水浴 2 0min后 ,腹腔用 1/2張鹽水持續(xù)灌洗 2h。觀察不同復(fù)溫方式對犬腹部開放傷合并海水浸泡救治時體溫、心率、平均動脈壓、中心靜脈壓和心輸出量的影響。結(jié)果 自然復(fù)溫組復(fù)溫效果不滿意 ,救治 6h后體溫 34 8℃ ,未恢復(fù)正常體溫 ,血壓提升困難、心動過速、心輸出量降低。體表復(fù)溫組復(fù)溫效果優(yōu)于前組 ,6h后體溫 36 9℃ ,接近致傷前體溫 ,但血壓、心率和心輸出量也出現(xiàn)同前組類似的表現(xiàn) ,只是程度較輕。體中心復(fù)溫組復(fù)溫效果滿意 ,救治后 4h體溫、平均動脈壓、中心靜脈壓和心輸出量基本恢復(fù)正常。結(jié)論 腹部開放傷海水浸泡后體溫過低的救治時最好采用體中心復(fù)溫方式。
