【摘要】： 2009年4月,中共中央、國務(wù)院《關(guān)于深化醫(yī)藥衛(wèi)生體制改革的意見》正式出臺。新醫(yī)改既給軍隊醫(yī)院帶來發(fā)展機遇,也給軍隊醫(yī)院提出嚴峻挑戰(zhàn)。當(dāng)前,如何適應(yīng)我國醫(yī)療改革形勢和我軍特色軍事變革的雙重要求,科學(xué)地回答建設(shè)什么樣的醫(yī)院、怎樣建設(shè)醫(yī)院和為誰建設(shè)醫(yī)院,從而更好地為經(jīng)濟社會發(fā)展服務(wù)、為廣大官兵和人民群眾健康服務(wù),是軍隊醫(yī)院迫切需要解決的難題。因此,提出軍隊醫(yī)院可持續(xù)發(fā)展模式研究的課題,對化解軍隊醫(yī)院發(fā)展中的現(xiàn)實矛盾和歷史問題,推動軍隊醫(yī)院步入可持續(xù)發(fā)展的良性軌道具有重要意義。 本文以軍隊醫(yī)院作為研究對象,堅持理論聯(lián)系實際,探討了軍隊醫(yī)院發(fā)展的現(xiàn)狀與特點,指出了軍隊醫(yī)院發(fā)展面臨的問題。針對這些問題,提出了軍隊醫(yī)院發(fā)展模式的設(shè)計思路,旨在為構(gòu)建新時期軍隊醫(yī)院可持續(xù)發(fā)展模式做出理論探索。 作者認為,軍隊醫(yī)院發(fā)展模式有三種類型:計劃模式、市場模式和綜合集成模式。計劃模式突出公益性目標(biāo)的實現(xiàn),但存在資金補償壓力,必然導(dǎo)致醫(yī)院發(fā)展效率低下。市場模式突出跨越式目標(biāo)的實現(xiàn),但因政府公共職能缺失,容易滋生看病難和看病貴問題。鑒于軍隊醫(yī)院兼有衛(wèi)勤保障、公共產(chǎn)品與醫(yī)療市場主體的特征,綜合集成模式強調(diào)政府主導(dǎo)、市場補充和社會參與三種力量的合成,既突出了公益性目標(biāo)與跨越式目標(biāo)的同步實現(xiàn),適應(yīng)了當(dāng)今世界醫(yī)院發(fā)展的趨勢,又兼顧了軍事效益、社會效益和經(jīng)濟效益的綜合平衡,切合了目前我國國情和我軍實際。因此,綜合集成模式是軍隊醫(yī)院可持續(xù)發(fā)展的必然選擇。為此,本文提出了從宏觀層面發(fā)揮政府公共職能作用、從微觀層面加強軍隊醫(yī)院戰(zhàn)略研究的建議。
[Abstract]:In April 2009, the CPC Central Committee, the State Council on deepening the reform of the medical and health system was formally issued. The new medical reform not only brings the development opportunity to the military hospital, but also puts forward the severe challenge to the military hospital. At present, how to adapt to the dual requirements of medical reform in our country and the military reform with our military characteristics, scientifically answer what kind of hospital to build, how to build the hospital and for whom to build the hospital, so as to better serve the economic and social development. It is an urgent problem for military hospitals to serve the health of officers and soldiers and the masses. Therefore, it is of great significance to put forward the research on the sustainable development model of military hospitals, to resolve the realistic contradictions and historical problems in the development of military hospitals, and to promote the military hospitals to step into the benign track of sustainable development. This paper takes the military hospital as the research object, adheres to the theory and practice, discusses the present situation and characteristics of the military hospital development, and points out the problems facing the military hospital development. Aiming at these problems, this paper puts forward the design idea of military hospital development mode, which aims to make a theoretical exploration for the construction of military hospital sustainable development model in the new period. The author believes that there are three types of military hospital development model: planning model, market model and integrated integration model. The plan model emphasizes the realization of the public welfare goal, but there is the pressure of fund compensation, which will inevitably lead to the low efficiency of hospital development. The market model emphasizes the realization of the leap-forward goal, but because of the lack of public function of the government, it is easy to breed the problem of difficult and expensive to see a doctor. In view of the fact that military hospitals have the characteristics of medical support, public goods and the main body of the medical market, the integrated integration model emphasizes the combination of three forces: government leading, market supplement and social participation. It not only highlights the simultaneous realization of public welfare goals and leap-forward goals, adapts to the development trend of hospitals in the world today, but also gives consideration to the comprehensive balance of military benefits, social benefits and economic benefits, and accords with the current situation of our country and the reality of our army. Therefore, the integrated model is an inevitable choice for the sustainable development of military hospitals. Therefore, this paper puts forward some suggestions on how to give full play to the public function of government and how to strengthen the strategic research of military hospitals at the micro level.
