中醫(yī)預(yù)防干預(yù)對(duì)高原駐訓(xùn)預(yù)備役男性軍人中醫(yī)體質(zhì)及相關(guān)指標(biāo)影響的初步研究

【摘要】:目的:觀察和研究高原駐訓(xùn)對(duì)預(yù)備役男性軍人脈搏、血壓、血氧飽和度、肺活量、血液成分(白細(xì)胞、紅細(xì)胞、血紅蛋白)、骨密度、肝腎功能(AST、ALT、CR、BUN)等臨床指標(biāo)的影響。同時(shí),觀察高原駐訓(xùn)對(duì)中醫(yī)體質(zhì)的影響,進(jìn)而探討高原駐訓(xùn)對(duì)預(yù)備役軍人身體機(jī)能的影響機(jī)制,為預(yù)備役軍人科學(xué)進(jìn)行高原訓(xùn)練提供中醫(yī)預(yù)防調(diào)養(yǎng)方案和醫(yī)學(xué)指導(dǎo),使參訓(xùn)人員盡快達(dá)到高原習(xí)服(即生活在高原缺氧環(huán)境中一段時(shí)間后,通過自身生理調(diào)節(jié),能適應(yīng)高原缺氧狀態(tài),缺氧初期的各種癥狀可明顯減輕),進(jìn)一步提高預(yù)備役部隊(duì)的戰(zhàn)斗力,為人員的身體健康和生命安全服務(wù)。方法:選取赴高原駐訓(xùn)的某部300名年齡跨度18-45歲,平均年齡(21.84±1.82)歲的預(yù)備役男性軍人作為調(diào)查研究對(duì)象,并于集結(jié)開展適應(yīng)性訓(xùn)練后,經(jīng)過長途機(jī)動(dòng)進(jìn)入高原(海拔跨度在1875米-4252米之間,)到達(dá)海拔4208米的某軍事訓(xùn)練基地,按照現(xiàn)役部隊(duì)統(tǒng)一要求,集中管理,統(tǒng)一訓(xùn)練及食宿保障,進(jìn)行為期60天的高原駐訓(xùn),進(jìn)高原前及進(jìn)高原后第7天和駐訓(xùn)結(jié)束時(shí)(第60天)各進(jìn)行中醫(yī)體質(zhì)辨識(shí)1次,同時(shí)各檢查記錄脈搏、血壓、肺活量、血氧飽和度、血液成分(白細(xì)胞、紅細(xì)胞、血紅蛋白)、骨密度、肝腎功能(AST、ALT、Cr、BUN)等臨床指標(biāo),觀察高原駐訓(xùn)前后各項(xiàng)指標(biāo)的變化情況后將統(tǒng)計(jì)結(jié)果進(jìn)行組間比較。將本次駐訓(xùn)中高原病的發(fā)生率設(shè)置為觀察組,與相同條件下文獻(xiàn)報(bào)道的部隊(duì)高原駐訓(xùn)中高原病發(fā)病率的結(jié)果為對(duì)照組進(jìn)行比較。觀察組在常規(guī)高原衛(wèi)勤保障基礎(chǔ)上根據(jù)中醫(yī)體質(zhì)辨識(shí)結(jié)果給予相應(yīng)中醫(yī)預(yù)防保健措施干預(yù),對(duì)照組實(shí)施常規(guī)衛(wèi)勤保障,比較兩組不同保障條件下高原病的發(fā)病率。結(jié)果:1.完成本研究的研究對(duì)象樣本依從性100%。2.1觀察組與對(duì)照組比較:輕度高原病發(fā)生率比較,P0.01,具有極顯著性差異;中度高原病發(fā)生率比較,P0.01具有極顯著性差異;重度高原病例發(fā)生率比較,P0.05,具有顯著性差異。觀察組通過療效判定為優(yōu)秀,對(duì)照組通過療效判定為中等。2.2通過對(duì)駐訓(xùn)人員各時(shí)期的中醫(yī)體質(zhì)辨識(shí),所有參訓(xùn)人員均為平和質(zhì)。高原駐訓(xùn)后第7天與進(jìn)高原前中醫(yī)體質(zhì)轉(zhuǎn)化分比較,P0.05,顯示無明顯差異;高原駐訓(xùn)60天后與進(jìn)高原前比較,P0.01,具有極顯著性差異。2.3預(yù)備役軍人高原駐訓(xùn)7天后與進(jìn)高原前脈搏、血氧飽和度、白細(xì)胞、血紅蛋白、紅細(xì)胞等指標(biāo)比較,P0.01,具有極顯著性差異;血壓比較,P0.05,具有顯著性差異;肺活量、骨密度、肝功能、腎功能比較,P0.05,顯示無明顯差異。2.4預(yù)備役軍人高原駐訓(xùn)60天后與進(jìn)高原前白細(xì)胞、血紅蛋白、紅細(xì)胞比較,P0.01,具有極顯著性差異;脈搏、血氧飽和度、肺活量、骨密度比較,P0.05,具有顯著性差異;血壓、肝功能、腎功能比較,P0.05,顯示無明顯差異。結(jié)論:高原駐訓(xùn)時(shí)在常規(guī)衛(wèi)勤保障基礎(chǔ)上通過中醫(yī)體質(zhì)辨識(shí)加用中醫(yī)預(yù)防保健干預(yù)措施能降低高原反應(yīng)的發(fā)病率,療效優(yōu)于高原常規(guī)衛(wèi)勤保障。且通過中醫(yī)體質(zhì)辨識(shí)給予個(gè)體化的中醫(yī)預(yù)防保健措施對(duì)駐訓(xùn)預(yù)備役軍人的中醫(yī)體質(zhì)及其相關(guān)指標(biāo)有明顯的改善作用,駐訓(xùn)后預(yù)備役軍人相關(guān)生理指標(biāo)優(yōu)于駐訓(xùn)前。表明該方法對(duì)提升預(yù)備役軍人身體素質(zhì)具有明顯促進(jìn)作用。方法簡便易行、安全有效,值得在今后的高原訓(xùn)練中進(jìn)一步加以研究和推廣運(yùn)用。
[Abstract]:Objective: To observe and study the influence of high altitude training on the pulse, blood pressure, blood oxygen saturation, vital capacity, blood components (white blood cells, red blood cells, hemoglobin), bone mineral density, AST, ALT, CR, BUN, and other clinical indexes of male soldiers in reserve service. The mechanism of the influence of human body function is to provide the prevention and regulation scheme of traditional Chinese medicine and medical guidance for the plateau training of the reserve servicemen, so that the trainees can reach the plateau as soon as possible (that is, after a period of time in the anoxic environment of the plateau, through their own physiological regulation, they can adapt to the state of high anoxia, and the symptoms of the early anoxia can be obvious. On the basis of physical constitution identification plus traditional Chinese medicine preventive health intervention measures, the incidence of plateau response can be reduced, and the curative effect is superior to the plateau routine medical support. The physiological indexes of the post training reserve servicemen are better than those before the training. It shows that the method has an obvious promoting effect on improving the physical quality of the reserve servicemen. The method is simple and convenient and safe and effective. It is worth further studying and popularizing in the future plateau training.
