【摘要】：隨著社會的發(fā)展，社會競爭的不斷加劇，人們對醫(yī)療水平要求的不斷提高，護理工作面臨著巨大的職業(yè)壓力。軍隊醫(yī)院護士作為護理隊伍中一個特殊群體，由軍人護士、文職護士和聘用護士組成，共同擔負著軍隊衛(wèi)勤保障的重任，她們既要承擔高負荷的臨床工作，又要面對軍隊復雜的醫(yī)療工作環(huán)境，長期的壓力源刺激會直接影響她們的心理健康，進而影響其生活、工作效率和質量。面對壓力采取不同的應對方式也會對心理健康產(chǎn)生影響。 目的： 本研究旨在通過調查某軍隊醫(yī)院護士工作壓力、應對方式和心理健康狀況的現(xiàn)狀，探討心理健康狀況與工作壓力、應對方式的內在關系，找出影響心理健康狀況的因素，提出提升心理健康狀況的對策，為提升軍隊醫(yī)院護士的心理健康狀況提供理論參考依據(jù)。 方法： 采用隨機抽樣的方法，選用自編的一般情況調查表、護士工作壓力源量表、簡易應對方式問卷、癥狀自評量表對某軍隊三級甲等醫(yī)院266名護士進行調查；通過對工作壓力、應對方式和心理健康狀況進行Pearson相關分析，獲得三者之間的內在關系；通過對心理健康狀況可能的影響因素進行多元線性回歸分析，獲得影響心理健康狀況的因素。建立Epidata數(shù)據(jù)庫，使用SPSS17.0軟件包進行統(tǒng)計分析。 結果： 1.某軍隊醫(yī)院護士SCL-90總分≥160分者162人，占總人數(shù)比例為60.90%。其中，，總分在160～199分的有140人，占52.63%，總分在200～239分的有18人，占6.77%，總分≥240分的有4人，占1.50%。某軍隊醫(yī)院護士SCL-90各因子得分均高于中國正常女性常模，差異有統(tǒng)計學意義。不同科室護士SCL-90總分差異有統(tǒng)計學意義，急診科護士得分最高，其次是外科、ICU。 2.某軍隊醫(yī)院護士工作壓力源得分：總均分為2.080.14。時間分配及工作量方面為2.550.33，病人護理方面為2.180.18，護理專業(yè)及工作方面為2.150.27，管理及人際關系方面為1.740.25，工作環(huán)境及儀器設備方面為1.740.45。 工作量太大在所有壓力源項目中位于首位。不同年齡、科室護士工作壓力總分差異有統(tǒng)計學意義。36-48歲護士工作壓力最大，兒科、婦產(chǎn)科、外科護士工作壓力位于前三位。 3.某軍隊醫(yī)院護士應對方式與常模比較，消極應對方式得分低于常模組，差異具有統(tǒng)計學意義。護士最常采用的應對方式為盡量看到事物好的一面。不同科室護士在消極和積極應對方式上差異均有統(tǒng)計學意義。 4.某軍隊醫(yī)院護士SCL-90總分與壓力總分呈正相關，與消極應對方式呈顯著正相關。 5.多元線性逐步回歸分析結果顯示，消極應對方式、管理與人際關系兩個因素進入SCL-90總分的回歸方程。 結論： 1.某軍隊醫(yī)院護士心理健康狀況與中國正常女性常模相比較差，工作壓力處于中度水平，采取消極應對方式少于常模。 2.工作壓力越大，心理健康狀況越差；采取消極應對方式越少，心理健康狀況越好。 3.影響某軍隊醫(yī)院護士心理健康狀況的因素有消極應對方式和管理及人際關系，消極應對方式對護士心理健康狀況影響最大。
[Abstract]:With the development of the society and the intensification of social competition and the continuous improvement of the medical level, the nursing work is facing enormous occupational pressure. As a special group in the nursing team, the military hospital nurses are composed of military nurses, civilian nurses and employed nurses, and they are responsible for the task of military medical support. To undertake high load clinical work and to face the complex medical environment of the army, long-term stressor will directly affect their mental health, and then affect their life, work efficiency and quality. Different coping styles in the face of pressure will also affect mental health.
Objective:
The purpose of this study is to investigate the status of working pressure, coping style and mental health status of nurses in a military hospital, to explore the internal relationship between psychological health and work pressure and coping style, to find out the factors that affect the mental health status, and to put forward the countermeasures to improve the mental health status in order to improve the psychological health of the military hospital nurses. The situation provides a theoretical reference.
