多艙室爆炸比格犬致傷特點(diǎn)與傷情分析

【摘要】:目的探索多艙室爆炸比格犬的致傷特點(diǎn)并進(jìn)行傷情分析。方法健康成年雄性比格犬48只,分別置于當(dāng)艙和鄰艙(n=24),于裝藥量為0.75kg和3.50kg TNT的艙室(當(dāng)艙)進(jìn)行靜爆,觀察爆后即刻至傷后24h動物存活情況、各項(xiàng)生命體征的改變、臟器形態(tài)學(xué)改變及各種致傷類型的發(fā)生率,并進(jìn)行神經(jīng)功能評分。結(jié)果比格犬現(xiàn)場死亡20只,傷后24h內(nèi)死亡9只,死亡發(fā)生率為60.42%(29/48),其中當(dāng)艙死亡發(fā)生率為79.17%(19/24),鄰艙死亡發(fā)生率為41.67%(10/24)。形態(tài)學(xué)改變主要表現(xiàn)為顱骨彈片穿通傷,腦組織以及肺、心、胃腸道、肝、腎等器官不同程度淤血和出血,軟組織挫傷或體腔破裂缺損,肢體斷離缺失,肢體骨折,嚴(yán)重者甚至發(fā)生空腔和實(shí)質(zhì)臟器穿孔或破裂。破片傷、沖擊波傷與沖擊波破片復(fù)合傷的致死率分別為27.59%(8/29)、17.24%(5/29)和55.17%(16/29)。結(jié)論艙室內(nèi)爆炸性武器所致破片傷與沖擊波復(fù)合傷具有發(fā)生率高、傷情重、致死率高等特點(diǎn),應(yīng)盡可能地尋找遮擋物體,從而減少被殺傷的機(jī)會,進(jìn)而降低死亡發(fā)生率。其早期救治的重點(diǎn)是及時(shí)有效地處理多發(fā)傷;液體復(fù)蘇時(shí)最好在血流動力學(xué)監(jiān)測條件下進(jìn)行,以免進(jìn)一步加重病情。
[Abstract]:Objective to explore the injury characteristics of multi-compartment explosive Beagle and analyze the injury condition.Methods 48 healthy adult male Beagle dogs were placed in the cabin and adjacent cabin respectively, and were detonated in the cabin containing 0.75kg and 3.50kg TNT. The survival of the animals was observed immediately after the explosion and 24 hours after the injury, and the changes of the vital signs were observed.Morphological changes of organs and incidence of various types of injury, and neurological function score.Results 20 Beagle dogs died in the field, 9 died within 24 hours after injury, and the death rate was 60.42%. The mortality rate of the second compartment was 79.17 / 19 / 24, and that of the adjacent cabin was 41.67 / 24 / 10 / 24.The main morphological changes were skull shrapnel puncture injury, brain tissue and lung, heart, gastrointestinal tract, liver, kidney, and other organs of different degrees of congestion and bleeding, soft tissue contusion or body cavity rupture defect, limb broken loss, limb fracture,In severe cases, the cavity and parenchyma organ even perforated or ruptured.The fatality rates of fragment injury, shock wave injury and blast wave fragment combined injury were 27.599 / 29 and 17.24 / 29, respectively, and 55.17% of 16 / 29 respectively.Conclusion the compound injury caused by explosive weapons in cabin has the characteristics of high incidence, serious injury and high mortality. It is necessary to look for occluded objects as far as possible so as to reduce the chance of being killed and then reduce the incidence of death.The emphasis of early treatment is to deal with multiple injuries in time and effectively, and fluid resuscitation is best carried out under the condition of hemodynamic monitoring, so as to avoid further exacerbation of the disease.
