【摘要】：目的研究高原低氧環(huán)境對(duì)急速進(jìn)藏新兵心功能及血流動(dòng)力學(xué)變化的影響 ,為指導(dǎo)部隊(duì)科學(xué)制訂高原訓(xùn)練計(jì)劃 ,預(yù)防和減輕高原反應(yīng)提供科學(xué)依據(jù)。方法隨機(jī)選擇 6 0名乘飛機(jī)進(jìn)藏的內(nèi)地新兵 ,應(yīng)用彩色多普勒超聲心動(dòng)圖于進(jìn)藏后第 1、2、3、4、12周分別進(jìn)行 2 7項(xiàng)心功能及心內(nèi)血流參數(shù) ,包括左心室容量、整體收縮功能及舒張功能、心室內(nèi)徑及室壁厚度、各瓣口峰值流速等的檢測(cè)。根據(jù)體表面積將所測(cè)數(shù)值與平原對(duì)照值進(jìn)行對(duì)比研究。結(jié)果 (1)進(jìn)藏第 1周即出現(xiàn)左心室收縮功能指標(biāo)下降 ,第 4周達(dá)到最低值 (P 0 .0 1) ,直至第 12周仍未恢復(fù)到平原對(duì)照水平 (P 0 .0 5 ) ;(2 )左心室舒張功能始終無(wú)明顯變化 ;(3)反映心臟結(jié)構(gòu)的指標(biāo) (LVDs、LVPWd、RVDd、PAD)隨進(jìn)藏時(shí)間延長(zhǎng)呈逐漸增厚增寬趨勢(shì) ;(4 )進(jìn)藏第 1周始主動(dòng)脈瓣峰值流速下降 ,肺動(dòng)脈瓣及三尖瓣前向血流峰值流速持續(xù)加快 (P 0 .0 5 ) ;隨訓(xùn)練強(qiáng)度加大兩瓣口返流出現(xiàn)率由進(jìn)藏第 1周的 5 3.3%增加到 95 .0 % ,返流量亦隨之增多。結(jié)論高原低氧環(huán)境對(duì)急速進(jìn)藏新兵早期心功能及血流動(dòng)力學(xué)的影響以左心室收縮功能下降及右心室前負(fù)荷加重為主要特征 ,這種影響在進(jìn)藏后第 4周內(nèi)最為明顯 ,說(shuō)明心功能在第 1個(gè)月內(nèi)處于調(diào)節(jié)性適應(yīng) ,此間不宜進(jìn)行強(qiáng)度訓(xùn)練。多普勒超聲心動(dòng)圖
[Abstract]:Objective to study the effects of hypoxia on cardiac function and hemodynamics in recruits rapid change, as the guiding force of scientific formulation of altitude training, and provide scientific basis for prevention to alleviate altitude sickness. Methods 60 recruits got into Tibet by plane, using color Doppler echocardiography in Tibet after 1,2,3,4,12 weeks respectively for blood flow parameters 27 heart and heart function, including left ventricular volume and systolic function, diastolic function, ventricular wall thickness and ventricular diameter detection, peak flow velocity of each valve and so on. According to the surface area of the measured value and plain control comparative study value. Results (1) Tibet observed in the first week of left ventricular systolic function the index fell fourth weeks, reached the lowest value (P 0.01), until twelfth weeks did not recover to the control level plain (P 0.05); (2) the left ventricular diastolic function has no The obvious change; (3) reflect the heart structure index (LVDs, LVPWd, RVDd, PAD) with Tibet time increased thickening widened trend; (4) to the first weeks of aortic peak velocity decreased, pulmonary valve and tricuspid valve three to peak flow velocity before continued to accelerate with (P 0.05); the training intensity increase two mitral regurgitation rate increased from 53.3% to 95% to first weeks, back flow also increased. Conclusion: the effect of high altitude hypoxia environment on rapidly recruits early heart function and hemodynamics in left ventricular systolic function decreased and right heart ventricular preload aggravated as the main feature, this effect in Tibet fourth weeks is the most obvious, that heart function in the regulation of adaptation in first months, here is not suitable for strength training.
