解放軍小湯山醫(yī)院患者及一線醫(yī)護(hù)人員心理干預(yù)模式研究

【摘要】:目的 1、對(duì)解放軍小湯山醫(yī)院SARS患者實(shí)施整體心理危機(jī)干預(yù)措施并評(píng)估療效和影響因素,探索對(duì)危機(jī)人群進(jìn)行臨床心理干預(yù)的有效模式。 2、對(duì)解放軍小湯山醫(yī)院一線醫(yī)護(hù)人員進(jìn)行心理干預(yù)并評(píng)估療效,探索對(duì)急性應(yīng)激事件下的人員進(jìn)行臨床心理干預(yù)的有效模式。 方法 1、680名SARS患者(其中男性379名,女性301名),對(duì)483名干預(yù)組患者進(jìn)行綜合心理干預(yù),包括電話心理咨詢(主動(dòng)和被動(dòng))、文字材料、自我調(diào)適以及面對(duì)面危機(jī)干預(yù)(主要由臨床醫(yī)務(wù)人員和心理醫(yī)生實(shí)施)。采用SCL-90對(duì)干預(yù)組和對(duì)照組進(jìn)行入院時(shí)和出院前的心理健康狀況評(píng)估,并采用自編的“心理干預(yù)效果評(píng)定表”對(duì)各種干預(yù)方法的干預(yù)效果進(jìn)行評(píng)定。 2、輪休期SARS一線醫(yī)護(hù)人員265人,對(duì)實(shí)驗(yàn)組131名一線醫(yī)護(hù)人員進(jìn)行心理行為訓(xùn)練,對(duì)照組134人不進(jìn)行心理行為訓(xùn)練,用SCL-90評(píng)估干預(yù)效果。 結(jié)果 1、實(shí)驗(yàn)組和對(duì)照組患者入院時(shí)SCL-90各因子分值均明顯高于地方常模的分值,差異有顯著性;入院時(shí)干預(yù)組和對(duì)照組的SCL-90各因子均分沒(méi)有明顯差異,而出院時(shí)干預(yù)組和對(duì)照組患者SCL-90各因子均分相比,在多個(gè)因子上干預(yù)組均分均顯著低于對(duì)照組(t=2.1~5.7,P0.01~0.05),兩組人員的各因子得分仍然顯著高于地方常模。各種干預(yù)方式中患者主動(dòng)電話咨詢的有效率最高(95.7%),文字材料的有效率最低(19.5%),經(jīng)過(guò)培訓(xùn)后的臨床主管醫(yī)生與心理醫(yī)生進(jìn)行心理干預(yù)的干預(yù)有效率都很高(80%),兩者之間的有效率比較無(wú)顯著性差異;主動(dòng)求助、被動(dòng)求助以及文字干預(yù)對(duì)文化程度較低的
[Abstract]:Purpose. 1. To carry out the whole psychological crisis intervention to SARS patients in Xiaotangshan Hospital of PLA, to evaluate the curative effect and the influencing factors, and to explore the effective mode of clinical psychological intervention to the people in crisis. 2. Psychological intervention and evaluation of the curative effect of first-line medical staff in Xiaotangshan Hospital of PLA were carried out to explore the effective mode of clinical psychological intervention for the personnel under acute stress events. Method. 1,680 patients with SARS (1,680 males and 301females) underwent comprehensive psychological intervention, including telephone counseling (active and passive, text materials). Self adjustment and face to face crisis intervention (mainly carried out by clinical medical staff and psychiatrist. Mental health status of intervention group and control group were evaluated by SCL-90 at admission and before discharge. The psychological intervention effect evaluation table was used to evaluate the intervention effect of various intervention methods. 2. During the rest period, 265 first-line medical staff of SARS were trained in psychological behavior in the experimental group, while those in the control group were not trained in psychological behavior. SCL-90 was used to evaluate the effect of intervention. Results. 1. The scores of SCL-90 factors in the experimental group and the control group were significantly higher than those of the local norm at admission, and there was no significant difference between the intervention group and the control group at admission. When discharged from hospital, the mean scores of SCL-90 factors in the intervention group and the control group were compared. In many factors, the average score of intervention group was significantly lower than that of control group. The scores of each factor of the two groups were still significantly higher than that of the local norm. The highest effective rate of patient active telephone consultation was 95.7% and the effective text material was effective. The lowest rate was 19.5%. The effective rate of psychological intervention between the clinical doctor and the psychologist after training was very high, and there was no significant difference between the two. Active, passive and written interventions were associated with lower literacy
