【摘要】：目的探討多排螺旋CT對胃平滑肌瘤與胃神經(jīng)鞘瘤的鑒別診斷價(jià)值。方法回顧性分析我院經(jīng)手術(shù)病理及免疫組化證實(shí),CT資料完整的22例胃平滑肌瘤及16例胃神經(jīng)鞘瘤,采用兩樣本t檢驗(yàn)分析瘤體的長徑、長短徑之比數(shù)據(jù)、靜脈期CT值并繪制ROC曲線得到最佳診斷值。采用Fisher’s確切概率法分析發(fā)病部位、生長方式、形態(tài)、粘膜強(qiáng)化、鈣化、強(qiáng)化程度。結(jié)果與胃神經(jīng)鞘瘤相比,胃平滑肌瘤好發(fā)于賁門,多向腔內(nèi)生長,增強(qiáng)掃描輕度強(qiáng)化,長徑≤3.2cm,長徑/短徑1.2,靜脈期CT值65HU(P0.05)。用長徑≤3.2cm、長徑/短徑1.2、靜脈期CT值≤63HU作為標(biāo)準(zhǔn),敏感度依次為86.4%、81.82%、95.24%,特異度依次為62.5%、81.25%、56.24%。結(jié)論使用以上特異的CT標(biāo)準(zhǔn)鑒別胃平滑肌瘤與胃神經(jīng)鞘瘤有很高的準(zhǔn)確性。
[Abstract]:Objective to evaluate the value of multi-slice spiral CT in differential diagnosis of gastric myoma and gastric schwannoma. Methods 22 cases of gastric smooth myoma and 16 cases of gastric schwannoma confirmed by operation, pathology and immunohistochemistry in our hospital were analyzed retrospectively. the long diameter, the ratio of length to short diameter, the CT value of venous phase and the best diagnostic value of ROC curve were analyzed by two sample t test. Fisher's exact probability method was used to analyze the location, growth mode, morphology, mucous membrane enhancement, calcification and enhancement degree of the disease.
