高溫高強度訓練對作戰(zhàn)訓練人員心臟的影響

【摘要】:目的分析高溫環(huán)境下高強度訓練對作戰(zhàn)訓練(作訓)人員心臟的影響。方法選擇解放軍252醫(yī)院于2015年7月1日~2016年8月31日收治的因高溫高強度訓練出現(xiàn)不適的作訓人員38例,均為男性,年齡18~23(20±1.8)歲。所有患者入院后均檢測血常規(guī)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、心肌肌鈣蛋白I(cTn I)、肌酐(Cr)、丙氨酸氨基轉(zhuǎn)移酶(ALT)、N末端腦鈉肽前體(NT-pro BNP)等,診斷并分析患者臨床癥狀。結(jié)果出現(xiàn)心臟相關(guān)癥狀胸悶、氣短12例(31.6%),心悸10例(26.3%)。此外,9例(23.7%)在出現(xiàn)不適癥狀時血壓偏低。12例(31.6%)出現(xiàn)心電圖改變,7例(18.4%)診斷為心律失常。5例(13.2%)出現(xiàn)心肌損傷,6例(15.8%)考慮存在多器官功能損害。20例(52.6%)診斷為中暑,其中6例(15.8%)同時存在肝腎功能受損,考慮為熱射病。患者入院后給予補液及相應對癥支持治療,均好轉(zhuǎn)出院,肝腎功能指標及心肌酶均恢復正常,無明顯后遺癥。結(jié)論高溫環(huán)境下的高強度訓練會引起作訓人員心臟等器官損傷,應科學合理設(shè)計訓練,使作訓人員提高對高溫環(huán)境高強度訓練的適應能力,減少心肌損傷,提高部隊戰(zhàn)斗力。
[Abstract]:Objective To analyze the effect of high-intensity training on the heart of the training in combat training (training) under high-temperature environment. Methods 38 cases of malaise in high-temperature and high-intensity training from July 1,2015 to August 31,2016 were selected to be male and 18-23 (20-1.8) years old. Blood routine, creatine kinase (CK), creatine kinase isoenzyme (CK-MB), cardiac troponin I (cTn I), myoglobin (Cr), alanine aminotransferase (ALT), and N-terminal brain natriuretic peptide (NT-pro BNP) were all detected after admission. Results The heart-related symptoms of chest distress, shortness of breath in 12 (31.6%) and palpitations (26.3%). In addition,9 (23.7%) had a low blood pressure in the presence of malaise.12 (31.6%) had an electrocardiogram change,7 (18.4%) had an arrhythmia,5 (13.2%) had myocardial injury,6 (15.8%) considered the presence of multiple organ dysfunction.20 (52.6%) were diagnosed with heatstroke, In 6 of them (15.8%), the function of liver and kidney was impaired, and it was considered to be a heat-emitting disease. After admission, the patient was given the fluid replacement and the corresponding symptomatic support treatment, and all the patients were discharged, the functional indexes of the liver and the kidney and the myocardial enzymes recovered to normal, and there was no apparent sequela. Conclusion The high-intensity training in the high-temperature environment can cause the injury of the heart and other organs of the training personnel, and the training should be scientifically and reasonably designed, so that the training personnel can improve the adaptability of high-strength training to the high-temperature environment, reduce the myocardial damage and improve the fighting capacity of the troops.
