低碘流率多層螺旋CT冠狀動脈造影對斑塊診斷價值的研究

【摘要】:目的探討低碘流率多層螺旋CT(MSCT)冠狀動脈造影對斑塊診斷的價值。方法收集2013年6月—2014年12月深圳市第四(福田)人民醫(yī)院行低碘流率MSCT冠狀動脈造影的冠心病患者30例為低碘流率組,另選擇同時期行正常碘流率MSCT冠狀動脈造影的冠心病患者30例為對照組,在相同掃描條件及對比劑濃度下,分別用1 480 mg I/s及1 850 mg I/s碘流率(注射流率分別為4 ml/s及5 ml/s)進行檢查,利用工作站軟件進行后處理圖像,比較兩組圖像質(zhì)量及斑塊指標(biāo)、對比劑總量、注射時間。結(jié)果對照組與低碘流率組信號-噪音比(SNR)、對比噪音比(CNR)、圖像質(zhì)量、圖像銳利程度、斑塊面積(PA)、斑塊負(fù)荷(PB)、斑塊偏心指數(shù)、狹窄程度(SD)比較,差異均無統(tǒng)計學(xué)意義(P0.05)。對照組對比劑總量為(68.5±4.9)ml,低碘流率組對比劑總量為(56.8±6.3)ml,兩組比較差異有統(tǒng)計學(xué)意義(t=8.067,P0.001)。對照組注射時間為(14.4±1.5)s,低碘流率組注射時間為(14.0±0.9)s,兩組比較差異無統(tǒng)計學(xué)意義(t=1.282,P=0.205)。結(jié)論使用低碘流率進行MSCT冠狀動脈造影,不僅可以達到診斷要求的圖像質(zhì)量,還可以減少對比劑使用,減少對比劑腎病等不良反應(yīng)的發(fā)生,具有一定的臨床應(yīng)用價值。
